IL Podiatrist Starts $100,000 Scholarship Program for 4th Year Students
 
Sev Hrywnak DPM, MD chair and founder of Project HUGS (Helping Under-insured Get Surgery) has started a scholarship program for 4th year podiatric medical students. “The current system places a financial strain on 4th year students who wish to visit residency programs or spend a month at a program. Additional financial resources will help level out the playing field and benefit those who do not have the means," stated Hrywnak.

Students are encouraged to apply during the end of their 3rd year. Scholarships will range from $1,000 to $5,000 dollars depending on the students' financial situation. "The cost of medical education has made the return on the investment more difficult and hopefully in the future a more logical and fair system will be put in place," added Hrywnak.

Salon Pedicures Can Expose You to the Flu: CA Podiatrist
 
While not linked to the actual process of getting a pedicure, close and personal environments, like the nail salon, can lead to the spread of the flu. "If workers are not wearing gloves, not washing hands in between clients, or seeing clients while they're sick, it could spread the virus," warns Dana Cozzetto, DPM, a podiatrist with Mercy Medical Group in Carmichael, CA.

"Remember, the flu virus can live for up to eight hours, so an infected customer could spread the virus to you if the chairs are not wiped down in between customers."

UK Podiatrist Uses Lower Level Laser Therapy (LLLT)
 
Lower level laser therapy (LLLT) is a recent addition to foot treatments. Podiatrist Margaret Dabbs advises it for a range of painful or inflammatory conditions including bunions, plantar fasciitis (heel pain), tendonitis, and wear and tear.

A colleague with a catalogue of chronic foot problems was so impressed with the Margaret Dabbs medical pedicure that she is booking in for the course of three to six laser treatments, as well as a three-monthly medi-pedi for routine upkeep. "My fungal nails look so much better and the gentle laser eased my arch pain," she says.

Over 400 Attend DERMfoot
 
DERMfoot 2018 launched this past weekend in Baltimore's Inner Harbor to a record crowd of over 400 podiatrists, dermatologists, and industry professionals with a keen interest in dermatology of the lower extremity. Joel Morse DPM and Annette Joyce, DPM celebrated their third year as meeting chairs. Council for Nail Disorders President, Nat Jellinek, MD and past president of the American Academy of Dermatology, Clay Cockerell, MD conducted a lively open panel "DERMfanatics" discussion on melanoma and nail biopsy.

Areas of of focus included regenerative medicine, skin and nail surgery, dermoscopy, inflammatory disorders, chemotherapy, wound care, and pediatrics. Standing room only biopsy cadaver workshops were taught by dermatology pioneers, "Dock "Dockery, DPM and Bryan Markinson, DPM. A poignant and uplifting keynote speech was delivered by Bradley Bakotic DPM, DO, which detailed Dr. Bakotic's lifelong journey into the fields of podiatry and dermatopathology along with his vision of an "even brighter future" for tomorrow's foot and ankle specialists.

Overall Tax Strategies for Physicians - Part 1
 
Now that the tax-filing deadline is here, the die is cast on last year's revenues, refunds, and amounts owed. However, smart planning throughout the year and effective management of expenses and deductions can leave physicians and medical practices better equipped for next year's filing season. Here are some tips that can help physicians maximize revenues and sail smoothly through tax season into next year.
 
1. Be Proactive -"Talk to your advisor or CPA during the year. Proactice planning can start after your tax return is filed, to make next year a better tax year. [Consider[ retirement plans and contribution planning, equipment purchase timing, better savings estimate for next April. With the new tax law and new withholding rates, pre-planning is even more important. Grouping large expenses in alternate years may help you beat the 199A thresholds. I think that Lamisil 250 mg Qd x 3 months with LFT monitoring is a good treatment option along with topical Jublia with or without a total nail avulsion  Discuss all possible side-effects with the child's parents .

Part of wisdom is knowing what you do not know. Refer this child to a pediatric dermatologist. By virtue of the inquiry, it is likely you are not experienced in this area. You have completed your job by making an appropriate referral to someone with more experience with this pathology.

The cause of the dystrophy of this nail will never be found. If it was present at birth, maybe the trauma was intrauterine or it's even possible that the nail matrix was oxygen deprived; whatever the exact cause, the nail matrix now outputs an abnormal nail plate. The nail plate is not only abnormal in appearance but in all probability is and always will be susceptible to fungal, saprophytic, and bacterial infections. 
 
In my experience, the use of systemic or topical antifungal agents will not cause a normal appearing nail to grow. There may be some who advocate steroid injections into the nail matrix, but in my experience, they are of little benefit. I understand the psychological effect that an abnormal nail may have on her, but the only solution would be to remove the nail and use cosmetic nail replacements periodically, or routinely file the nail to a normal thickness and paint the nail. Sometimes, conditions are not resolvable.

Dr. Forman wrote: “I received notification from Medicare that 33% of my visits submitted included an E/M charge. I was told it was above the average.” To me, there are two issues here: 1) do you really want to go toe-to-toe with Medicare defending your E&Ms and 2) I am surprised that 33% is above average. I know that some doctors will challenge Medicare personally and spend a lot of time and effort defending their claims. If your office is equipped to handle this, then go for it. The 33% number seems low and may be something that APMA could look into. 
 
That said, there may be a lot of practices where “non-medically licensed” personnel are trimming toenails, corns, and calluses, and these offices may simply have a high turnover of procedures without examinations. On a side note, if a PA (physican assistant) or NP (registered nurse practitioner) submits a bill to Medicare, it is paid at a lower fee profile than if submitted by an MD, DO, or DPM; however, when “non-medically licensed” personnel perform routine foot care, that procedure is billed as if the DPM personally performed the procedure. Even though Dr. Forman may be practicing good medicine, it appears that the numbers may simply be against him.

I have tons of patients sent to me for "routine care" from large managed care clinics in my area. I often hear from these patients that they were seen by the podiatrists in their clinic and told by them, "I'm a surgeon. I don't trim toenails or calluses!" and that they often don't even look at their feet. They refer them out to the few clinics in my area (population >1 million) that do "routine care". 
 
In most all cases, I do a full lower extremity exam for these "routine care" patients. Many of the elderly have had knee or hip replacements, so I routinely measure for leg-length discrepancies, excessive pronation, collapsing or collapsed medial column, and I have them stand and walk. As a result, we fit...
