Compare at Least 3 Different Running Shoes Before Buying: MN Podiatrist
Ask any running coach or podiatrist, and they’ll tell you the best way to pick a cushioned running shoe is to try on a few options and decide which feels best to you. “The best evidence for how to select a shoe to minimize risk of injury or pain is to try on shoes by running in them, compare at least three different shoes, and select the one that is most comfortable to you,” says Paul Langer, DPM, a board-certified podiatrist and fellow of the American Academy of Podiatric Sports Medicine.

Dr. Langer encourages his patients not to be too loyal to specific shoe brands when picking out a new pair of sneakers. “There are an overwhelming number of choices out there, and limiting people to one brand or specific models robs them of the opportunity to prioritize their own comfort,” he says. “On top of that, no brand has proven to be superior to others in terms of comfort or injury reduction.”

UK Podiatry Student Switches from Hospitality to Healthcare
It will soon be last orders for one local pub owner as she prepares to move from hospitality to healing heels. Since 2016, Claire Eyles has co-run The Wheatsheaf in Braunston with husband Chris after many years working in the hospitality sector. Now, Claire is jumping professional ships to a radically different discipline – podiatry, which covers healthcare related to the lower limbs.

TX Podiatrist Convicted for $8.5M Compounding Fraud Scheme
A federal jury convicted two Texas men today for their role in a scheme to fraudulently bill TRICARE – the healthcare program for U.S. service members and their families – for compounded creams that were medically unnecessary and procured through kickbacks and bribes. According to court documents and evidence presented at trial, Brian Carpenter was a podiatrist who signed prescriptions for compounded pain and scar creams for TRICARE beneficiaries to whom he never spoke and whom he never examined or treated. Jerry Lee Hawrylak, a pharmacist, recruited Carpenter to sign the prescriptions and recruited TRICARE beneficiaries to accept the medically unnecessary creams.

VA Surveillance & Contact Precautions Reduced MRSA Infections during Covid
During the COVID-19 pandemic, Veterans Affairs facilities that practiced active surveillance and contact precautions reported fewer healthcare-associated MRSA infections, especially in ICUs. Brian McCauley, DPM, MHSA, the national Veterans Affairs multidrug-resistant organisms program manager, discussed the VA’s MRSA prevention initiative, which was created in 2007 to reduce healthcare-associated MRSA infections across all 130 acute-care VA facilities as well as long-term care facilities. Additionally, the VA suspended several precautions like active surveillance screening to conserve PPE resources for COVID-19 testing in facilities. McCauley assessed the effects of suspending active surveillance and contact precautions for patients colonized or infected with MRSA in a prospective cohort study from July 2020 to June 2022 at 123 acute-care VA medical facilities.
Dr. McCauley said that even after accounting for monthly COVID-19 admissions, the relationship between facility policy and MRSA HAI rates in ICUs or non-ICUs did not change, adding that “MRSA HAI rates increased as the number of interventions decreased”. Therefore, it is suggested that the impact of these precautions is greater in ICUs compared with non-ICUs and the differences in rates between ICUs and non-ICUs may be a point for policy decisions.

Student Loan Concepts to Know During Tax Season
With tax season approaching, many people are focused on gathering all the tax forms to make sure they don’t miss the April deadline; however, few will pause to inquire how their tax filing could impact their student loan payment. Fortunately, there are many ways that borrowers can get creative with income reporting come tax season that could impact their student loan payments substantially.
11A. How could filing an extension help you save money? - If you are playing the game right, we typically like to see our clients reporting based off their income from 2 years in arrears. This typically can be a lucrative approach going through a major income change as you can effectively delay the impact of that income change for a couple of years. Since you must report off your most recently completed tax return though, there are some instances that filing a tax extension could be beneficial for you. As an example, if you file an extension on your 2022 taxes and don’t file them until October 2023, if we go to report your income to the Department of Education in September of 2023, your most recently completed tax return would still be from the year 2021 (hence the two years and arrears).
Since this is your most recently completed tax return and it is within 2 years, this would allow you to make payments based off that 2021 income for 12 subsequent months after you report that income. This means that you could be making payments based off your 2021 income all the way through September of 2024. With the recent COVID extensions, you may no longer need to file extensions but depending on your situation, it could be substantially beneficial.

1. Cut 1/4” foam roll with adhesive backing into a 1.5”x 1 3/4” rectangle. Cut the edge of the longer side at a 45 degree angle. Peel back the paper backing and roll it from the unbeveled area towards the beveled edge. Take 1/2” tubular gauze and place the roll inside. Take the roll that is now inside the gauze and put it in your hand under the middle three fingers in the sulcus area and let the loose ends of the gauze hang from both sides. Bring the two loose ends together to meet on the back of your middle finger and pinch them together. Slide this off your hand while holding the area you pinched and cut the ends just under the pinched area in the gauze. Take a kidney-shaped mole skin and put the two ends head-to-head on the sticky side of the mole skin and fold over the mole skin, trapping the ends of the gauze. Place over the middle three toes of the patient with the crest placed in the sulcus area.
2. Punch a hole in the patient's shoe insole underneath the tip of that toe and plug with PPT.
3. Make an accommodative triple density custom orthotic and make sure the lab does not remove the impression in the forefoot and toe areas so that the device will have the exact contours of the sulcus and toes. Then have them punch a hole below that toe tip and fill it with PPT before covering with a Plastazote top cover.

RE: These DME Updates Go Into Effect Immediately

Medicare has made several changes which are imminent when it comes to DME. For both sets of codes below, you will need to have a documented face-to-face examination of the patient which stipulates the patient's needs for these specific devices. This should not be problematic for patients we are treating with these DMEPOS.
1) The vendor who provides your patients with osteogenesis stimulators must have a complete Written Order Prior to Delivery (WPOD) to the patient. That is, the order must be signed and dated and in their system prior to the date of the patient's signature on their delivery ticket. Otherwise, the vendor providing them with the bone stimulator will not be paid. Be on the lookout for more scrupulous documentation requirements from every bone stimulator company you work with, and be understanding of their needs.
2) Podiatrists who are providing their own patients with AFO devices, a WPOD prior to delivery for certain AFOs (L1940 L1951, L1970) must also have documentation in the chart which satisfies the WOPD requirements. At a minimum, this includes a generic description of the item, date of need, number of items (usually one), patient information, your name (please make it legible), and your NPI.
For these custom orders, your laboratory order along with the other information noted should suffice, along with the medical chart documentation supporting medical necessity. The note and laboratory order must be dated prior to the date of delivery. Written Proof of Delivery requirements remain unchanged.

RE: Shoe Crease Protectors

In the interest of keeping podiatrists informed of new shoe trends, and their potential negative biomechanical implications, it has recently come to my attention that a relatively new shoe product is being marketed and widely used by many younger individuals in their expensive sneakers (think Air Jordan). This widely-used shoe product is called a “crease protector” and may be made of either a plastic sleeve that fits inside the forefoot region of the shoe, or may even be a foam piece placed on the dorsal aspect of the forefoot in order to prevent a dorsal “crease” developing over time in the vamp of the shoe. Those of you who have children who are pre-teens or teenagers, probably already have encountered these products.
I first learned about this product last night from my 12 year-old granddaughter. I was very proud that she could demonstrate the various odd gait patterns that her pre-teen and teenage friends demonstrate when wearing these “crease protectors” in their shoes which prevent them from undergoing normal propulsive gait mechanics. She said that these are widely used in her middle school by many of her friends and she assumes that they are also popular in high schoolers who also want to keep their expensive, leather-uppered shoes looking “new”.
Even though I have not yet seen any issues from wearing these plastic devices inside shoes which restrict normal metatarsophalangeal joint dorsiflexion during gait, it may be a factor which we may want to consider if unusual symptoms are occurring in younger individuals who have no other biomechanical explanation. And, unfortunately, this is **not** an April fool’s joke.

A recent qui tam whistleblower case from a former Cigna medical director alleges that Cigna medical directors and their subordinates routinely and en mass denied claim appeals (upwards of 50-60) per minute without ever even opening up the files. A few weeks ago UHC announced they would only accept claim appeals via their portal (AKA electronically). Does anyone here smell a rat? Is anyone here surprised?
I'd suggest that the Federal gov't require all insurance companies to be required to have routine unannounced forensic examination of their software so it can be determined how long a file is open by the auditor, CMD, etc. and how long it takes to write a report etc. for any claim sent to appeal. Furthermore, those forensics must be available to the OIG on a routine basis! The penalties for not following acceptable protocols need to be commensurate with the crime and not a slap on the wrist!
