IA Podiatrist Among First Responders to Boston Marathon Massacre

A doctor from Iowa was among the first emergency responders when two bombs went off Monday near the finish line of the Boston Marathon. The blasts killed three people and injured at least 175 others. Des Moines podiatrist Dr. Kirk Neustrom was working at the finish line medical tent, as he has done the last 19 years, when he heard “a loud boom.” He stepped outside just a couple of seconds later. “I looked down the street and saw a big plume and thought ‘huh,’ I mean it sounded like a cannon and I thought that was strange,” Neustrom says. “And then, about 10 to 15 seconds later, we saw and heard the next one and I immediately knew it was a bomb.

So, I immediately headed toward where the bombs went off because everyone knows if you’ve got bombs going off, you’re going to have people needing medical attention.” The 53-year-old Neustrom, a Navy veteran, helped with basic triage. One of the first victims he encountered was badly wounded. “A gentleman with his legs blown off below the knees. He had no legs. That was probably the most gruesome thing that I saw,” Neustrom says.

UK Podiatrist Discusses Children's Shoe Fitting

Podiatrist Emma Supple recently answered some common questions about children’s foot health: Q: Why do I need to get my children’s shoes fitted?

Emma says: “Growing feet need room to grow, and children’s feet grow in variable bursts so being vigilant and awareness is key. It is recommended that children get their feet measured every 6-8 weeks before they begin school, and then every 3-4 months once they begin school. Even if children haven’t outgrown their shoes, they should be replaced if the shoes are worn out. If children aren’t wearing correctly fitted shoes, they can create or sustain foot deformities that may have a significant impact on their adult lives due to foot problems.”

Former AK Podiatrist Presents at HIMSS Meeting

Dr. Steven J. Kaniadakis, a former practicing podiatrist from Alaska, recently became the first podiatric surgeon ever to be an invited presenter at the prestigious HIMSS organization, a cause-based, not-for-profit organization exclusively focused on providing global leadership for the optimal use of information technology (IT) and management systems for the betterment of healthcare. Kaniadakis presented his software, an electronic super-bill invention.

Forty thousand people attended, including former President Bill Clinton, who gave the keynote address, and former White House Official, Karl Rove. The super-bill that  Kaniadakis developed is designed to enable users to use virtually any EHR (EMR) and practice management or billing/coding software, so that users are not limited to using one EMR system.

IL Podiatrist Among Those Arrested in Chicago Hospital Kickback Case

After an investigation involving witnesses wearing wires inside a hospital, federal agents have arrested the owner-CEO of Sacred Heart Hospital in Chicago and five other people on charges of running an extensive kickback conspiracy in which physicians were bribed with $225,000 in cash and other payments to refer Medicare patients for treatment.

Among those arrested Tuesday was Edward J. Novak, 58, the owner and chief executive of the 96-bed stand-alone for-profit hospital on Chicago's West Side. Authorities also detained Roy Payawal, 64, chief financial officer and executive vice president at the hospital, and four doctors who practiced there: Dr. Venkateswara “V.R.” Kuchipudi, 66; Dr. Percy Conrad May, Jr., 75; Dr. Subir Maitra, 73; and Dr. Shanin Moshiri, 57. Moshiri, a podiatrist, allegedly received $86,000 for purporting to teach podiatry students according to prosecutors.

Fee Data Puts Brakes on Docs' Test Orders: Study

It has long been an article of faith among certain healthcare reformers that soaring national healthcare costs can be constrained by making patients more aware of the cost of their own episodes of care. Price awareness by physicians could have similar effects, according to a team of researchers from the Johns Hopkins University School of Medicine, reporting in a six-page article in the online edition of JAMA Internal Medicine.

Their work, “Impact of Providing Fee Data on Laboratory Test Ordering: A Controlled Clinical Trial,” shows that presenting lab test costs to providers at the time the tests are ordered resulted in “a modest decrease” in test orders and costs. “Adoption of this intervention may reduce the number of inappropriately ordered diagnostic tests.”

It may be time for an IM injection of 40 to 60 mgs of Kenalog. However, before you do that, the patient should be patch-tested for shoe chemical allergies. It is quite possible that with the bazooka treatment the patient is getting, exposure to the primary allergen continues unabated. That should be addressed. You can send the patient to an allergist or do the testing yourself with the T.R.U.E. Patch testing kit.

I suggest avoiding the potent topical steroids as their mechanism of action is vasoconstriction. I have had success with Pimecrolimus cream (a steroid-free medication applied to the skin). Also, there is a causal relationship with eczema and low levels of Omega 3s, so I would try some OTC or Rx Omega 3.

EMR Compatibility with Dragon Voice Recognition (John Scheffel, DPM)

I have been using Dragon dictation 11.5 to supplement Practice Fusion EMR. I upgraded to 12.5 and when it wouldn't work, I contacted Dragon. They say it works with everything except any EMR program, and the fix is to upgrade to Dragon Medical for $1,600. Needless to say, I went back to 11.5, which by the way, only works with older versions of Adobe Reader, not the new versions. Nuance/Dragon put a "bomb" in their 12.5 version so they can sell their $1,600 medical version.

Dragon Voice Recognition (and other dictation solutions) can be an extremely valuable tool to help with your documentation. Whether it is your only method of dictation, or used as a supplement to another charting or dictation solution, it provides you with a great way to express yourself in your notes.

As said before, you should contact your current, or future, EMR/Charting Vendor, and ask about implementation and ease-of-use. For example, the QNotes Office Solution from Quick Notes is a truly simple EMR that works extremely well with voice recognition, whether or not you use the portable Quick Notes iPad/Touchscreen. Simply find a patient, add a new note, and dictate! It’s as easy as that!

He has always answered my questions promptly, professionally, and accurately. If he was unsure of an answer, he would research it and get back to me in a timely manner.

Dr. Gastwirth has attended many, many podiatric meetings throughout the country and always made himself available to answer any questions from the membership at any time. Like Dr. Barry Block, he has his finger on the pulse of podiatry and is knowledgeable in so many areas of our profession. He is a true asset and icon, we are all lucky he serves in his capacity.

In reading the plethora of scientific information over the years regarding the uses of Epsom salt, I was surprised to read Dr. Bowman’s comments regarding this salt. Any salt, including magnesium sulfate, is anhydrous, causes drying of the skin, and absorbs fluid from wounds and ulcers. However, I have never found any science that demonstrates that warm Epsom salt soaks have any benefit over warm table salt soaks of equal molecular concentration. I would appreciate being enlightened with non-anecdotal facts.

I received over twenty emails from these stranded applicants. I too think that they should go to residencies and offer their services for no pay. Sometimes, residents drop out for reasons of all kinds and you will already be there. There are many other professions who intern for a year or two without pay. As long as you get that certificate, with equal training, it is worth it. Believe me, I made $7,800 my first year and $12,000 the second year.

If you have loans from school, you can apply for a two-year hardship deferment. That is what I did, and then I got two more years at interest only. It is do-able. Be proactive, strong, and persevere. I promise you will make it. To the class of 2013, I give you all my best wishes. I know how hard you all worked.
