PA Podiatrist Has Advice for Aging Feet
 
Taking a few moments to tend to your sole health can minimize age-related issues, according to Chestnut Hill Hospital’s vice president and chief medical officer, podiatrist John Scanlon, DPM. “The feet are your foundation and absolutely play into your quality of life as we age,” Dr. Scanlon said. “If you develop foot issues, it affects you physically, emotionally, and spiritually.”

Scanlon, who at 61 still plays a pickup basketball game once a week to keep in shape, has had ankle problems as an athlete. While some issues are genetic, he believes being proactive can do a lot to improve your foot health in your senior years. “Good foot health is essential to a good quality of life,” said Scanlon. “Exercise is the key. There is no magic formula. You’ve got to get off your derriere and start moving, as long as your doctor gives you clearance.”

MA Podiatrist Discusses Treatments for Morton's Neuroma
 
April serves as Foot Health Awareness Month, and a good reminder to assess any ailments of these body parts on which we are estimated to walk 115,000 miles in our lifetime, the equivalent of about four times around the world. Foot issues are often associated with aging and certain chronic diseases, but Dr. Christopher Peteros’ patient population has ranged in age from children to adults into their 90s during his more than 25 years in practice.

A common foot condition seen is a neuroma, sometimes called a Morton’s neuroma, “which is a sharp pain between the third and fourth toes most often” and caused by tissue that becomes thick around a nerve leading to a toe. “Morton’s neuroma treatment many include a cortisone shot to shrink the nerve or alcohol injections to damage the nerve and make it stop feeling pain, or nerve decompression surgery where I make a small, half-inch incision that can be done somewhat noninvasively,” Peteros said.

David Sullivan, DPM and Athletico Physical Therapy, along with the Westfield High School boys lacrosse team announced that the organizations will partner with Changing Footprints to conduct a “Spring Cleaning with Meaning” shoe drive from April 22 to May 10.

How to Protect your Reputation Against Defamatory Online Reviews - Part 5
 
Doctors, dentists, psychologists, clinical laboratories, and other medical providers have a right to defend their reputations and livelihoods against reviewers who spread false and damaging claims. Fortunately, there are a variety of legal options available to combat defamatory online reviews.
 
5. Increase positive reviews. Make a sustained effort to increase the number of positive reviews. Asking satisfied patients to post truthful online reviews can increase a provider’s overall rating and may dilute the effect of one older negative comment compared to more recent positive reviews.

Does Medicare still cover shockwave treatment for plantar fasciitis CPT 28890 (high energy in outpatient hospital setting). I remember I used to do this back in the early 2000s but the payment was low.

Response: CPT code 28890 is: Extracorporeal shock wave, high energy, performed by a physician, requiring anesthesia other than local, including ultrasound guidance, involving the plantar fascia.
 
Medicare still covers and reimburses this CPT code/procedure in both an office setting and in a facility setting, as well. The approximate reimbursement in an office setting is $357.00. The approximate reimbursement in a facility setting is $249.00.

It is with sadness that I notify PM News readers of the passing of Ronald E. Cangiano, DPM, age 81 on April 14, 2019. He was a past President of the New Jersey Podiatric Medical Society and practiced in northern NJ for 30 years. He was a 1960 graduate of the Illinois College of Podiatric Medicine. He resided in NYC and Miami, Florida and succumbed to recurrent prostate cancer.

While I  agree with Dr. Blum for private insurers - in California, the patient’s Social Security number is a requirement when billing California Workers Compensation Insurance. Many if not all of the CA Workers Compensation Insurance Companies may not process your bill if the patient’s Social Security number is not on the claim since this information is matched to the employer’s employment records.
 
For those people who actually do not have a Social Security number, you are required to use 999-99-9999. But you cannot use it if the injured worker simply does not give you their SS# or forgot it. The insurance company will electronically compare the Social Security number on your bill (whether e-bill or paper billed) with the records of the employer. If they do not match, you will get a rejection notice.

At the expense of insulting friends and colleagues who have a vested interest in balance braces, allow me to give my personal experiences with them. When these products first became available, I attended many sponsored lectures on them and it was exciting. Having lost a grandparent as a result of a fall, I wanted to do anything and everything to help my patients prevent unnecessary falls, so I dispensed many pairs of balance braces to patients. My findings were that the patients who used them did well and reported back greater stability and less falls.
 
The problem is that the devices are bulky, often hard for an elderly patient to put them on, and most of the patients stopped using them. Some were open and honest about it. Others said they were using them but when they came into my office for some "mysterious" reasons they never had them on. Yes, I truly believe they have value but the rate of compliance in my patient population was not there. I am, however, open to revisiting the issue of balance braces if they can be rendered more comfortable so that my patients will use them.

We tried this for a short period since it dovetailed well with our MIPS reporting. I found them to be poorly tolerated. The AFOs are cumbersome and don't fit easily into patients' shoes. This is extremely problematic in a deconditioned patient. I did have one patient with a reducible equinovarus deformity who found the AFO helpful, but the feedback was lukewarm to unfavorable. You will need to spend time responding to patients' complaints and returning the AFOs to the lab for adjustments. If you really want to help your fall risk patients, a PT referral can go a long way. Establish a relationship with a home nursing agency to set up home PT if needed. Your patients will return to you with a spring in their step.

In response to the post by Ernest L. Isaacson, DPM:  In the last sentence the reference should have been to the LOW pressure environment of an airplane, not high pressure.

Dr. Isaacson is only partially right in his comments about wearing compression socks during air flight. He doesn’t explain the full reason for wearing compression socks, which should be stockings above the calf. The more important reason is that pooling or stagnation of blood could lead to a DVT or a PE, a real medical emergency, not just a cosmetic fat foot. 
 
I usually suggest to my patients that in addition to wearing compression stockings, they also consult with their medical doctor about taking an 81mg aspirin just before they fly to minimize the possibility of these problems.
