DC Podiatrist Provides Tips to Avoid Blisters and Lost Toenails
Lee Firestone, a DC podiatrist, running coach, and longtime runner, recommends wearing tight acrylic socks and a tried-and-true running shoe, and keeping your toenails short. (He sees plenty of blisters and lost toenails in the medical tent during races.)

If it’s particularly hot, lace your shoes higher to prevent your foot from moving around in your shoe, which can cause too much skin friction. “We often get blisters running downhill because the foot is moving forward in the shoe,” Firestone says.

CA Podiatrist Publishes Book
Richard L. Blake, DPM is a practicing podiatrist in San Francisco, CA, at the famed Center For Sports Medicine, St. Francis Memorial Hospital. Secrets to Keep Moving: A Guide from a Podiatrist is a mixture of the art and science of that medical practice. The emphasis of the book is on the conservative treatments of injuries of the foot, ankle, and lower leg. Dr. Blake emphasizes conservative, and when appropriate, simple approaches to these injuries. He tries hard to establish a healing environment for the patient at the first visit, and then works the patients through all the phases of rehabilitation.
|
|
Dr. Richard Blake |
This book begins by covering some general topics like what rules not to break, then discusses general rehabilitation principles, general treatments of pain, bio-mechanical approaches to injury, and gait evaluation. He then describes some of the general rules of treatment concerning tendinitis, nerve injuries, and fractures. In the second part of the book, Dr. Blake uses many of these general principles and begins to describe top treatments for injuries in the front of the foot, mid part of the foot, heel area, ankle, Achilles tendon area, and lower leg not involving the knee or above.

Primary Culprit for Bromhidrosis is Old Shoes: MD Podiatrist
According to podiatrist Benjamin Kleinman, DPM of the Baltimore Podiatry Group, the most mundane culprit of toe-curling foot stink is old shoes. "The first thing I ask a patient who comes in with foot odor is 'How old are your shoes?' Most people will say, 'Oh, they're in good shape,' but then I find out they're over a year old," he says. Shoes that are past their due date are a breeding ground for smelly bacteria. Toss 'em.
|
|
Dr. Benjamin Kleinman |
To prevent sweat in the first place, you can use an anti-perspirant. The same stuff you swipe on under your arms will work, but a spray like Dove Dry Spray is a little easier to apply than solids. Kleinman doesn't recommend using powders not specifically designed for your feet to absorb moisture and cut the scent, since certain bacteria or fungi can use them for food.

NJ Podiatrist's License Suspended For Prescribing Pain Meds
The NJ State Board of Medical Examiners has temporarily suspended the license of a Passaic County podiatrist amid allegations he prescribed painkillers and other pills to patients for years without legitimate medical purpose. In addition to pain pills like oxycodone, Dr. Anthony Enrico Jr. is accused of prescribing drugs like Xanax, Ambien, and Lunesta for conditions unrelated to podiatry, Attorney General Christopher S. Porrino and the state Division of Consumer Affairs have alleged.

White House Finalizes ACA Rule to Strengthen Individual Market
The White House has finalized a rule that aims to appease insurers looking for some stability in the individual marketplace before they must submit rates for 2018. The rule limits when consumers can gain coverage outside of open enrollment periods, shifts authority to states to determine whether health plans have adequate provider networks, lets insurers potentially pay a lower percentage of consumers' medical costs, and allows insurers to refuse to cover persons who haven't paid their premiums.
The rule, however, does not address the insurance industry's major concern regarding continued enforcement of the individual mandate. It does continue and even increase subsidies for people who buy coverage on the Affordable Care Act's individual coverage exchanges.

Query: The Use of CPT 20500
Is CPT 20500 (injection of sinus tract; therapeutic [separate procedure]) only to be used if dealing with a sinus tract, or can it be used as a therapeutic injection for other pathology?
Barry Feinstein, DPM, North Hollywood, CA
Response: I think the use of CPT 20500 is limited to injection of a sinus tract. If you inject something other than a sinus tract, you should code the injection procedure specific to the type of injection performed.

| RESPONSES/COMMENTS (NON-CLINICAL) - PART 1A | |
From: Ivar Roth, DPM, MPH
Missed first appointments were a continuing source of irritation in my practice. We now take a credit card from all first time patients and bill a nominal amount to make sure the card is real. This amount is deducted from their visit if they come in. We send a confirmation email immediately after they call with a copy of our policy so there is NO miscommunication.
This has worked well for us. Those who refuse to give us a credit card do not get an appointment. This is a real simple solution. We only charge $75 if they fail to show up or give notice. This policy is really to keep the new patient mindful that our time is valuable when they book an appointment.
Ivar Roth, DPM, MPH, Newport Beach, CA

| RESPONSES/COMMENTS (NON-CLINICAL) - PART 1B | |
From: Paul Busman, DPM, RN
I wouldn't require a credit card guarantee. Right off the bat, even before you'd established a doctor-patient relationship, you'd have shown distrust for that new patient. You've also suggested that getting their money is more important than taking good care of them. Missed appointments are a fact of life.
A better way would be to call that potential new patient the day before the appointment and remind them of the date and time, and tell them how much you're looking forward for the chance to care for them.
Paul Busman, DPM, RN, Frederick, MD
