AZ Podiatrist Discusses Common Causes of Foot Cramps
“Pain is the body’s best sign that something is going awry,” says Sondema Tarr, DPM, a board-certified podiatrist and foot surgeon at Direct Podiatry Arizona in Tempe. In most cases, there are easily fixable reasons behind your cramping feet. It commonly means that you’re low on potassium and other electrolytes. Electrolytes are essential minerals, such as sodium, magnesium, and potassium, that you get from the foods you eat and liquids you drink. “These molecules have electrical charges, and they help the body send important messages to the muscles,” explains Dr. Tarr.
|
|
Dr. Sondema Tarr |
Dr. Tarr adds that recurring cramps can sometimes be a sign of circulation issues, but there would usually be other signs along with the toe cramps, including cramps in your calves and upper legs. In some cases, cramps could be a sign of neuropathy or peripheral arterial disease (PAD), which is usually related to diabetes or stroke, she says.

Carbon-Plated Shoes Best for Experienced Runners: DC Podiatrist
Deployed with strategy and intention, carbon-plated shoes can be a game-changing addition to an experienced runner’s shoe rotation. Yes, the word "experienced" is key. Lee Firestone, DPM, a podiatrist and certified running coach, emphasizes that runners should already have strong running form—including a fast cadence, a slight forward lean, and a strong knee drive to avoid heel striking—in order to wear the shoes, as well as strong feet, ankles, and glutes.
|
|
Dr. Lee Firestone |
Some experts believe that the shoes’ extreme energy return could overtax the tendons and ligaments in the midfoot, or lead to plantar fasciitis or bone stress injuries. Dr. Firestone says that the high stack in most carbon shoes could lead to instability, leaving you at a greater risk of ankle sprains.

Student Loan Concepts to Know During Tax Season
With tax season approaching, many people are focused on gathering all the tax forms to make sure they don’t miss the April deadline; however, few will pause to inquire how their tax filing could impact their student loan payment. Fortunately, there are many ways that borrowers can get creative with income reporting come tax season that could impact their student loan payments substantially.
10B. Filing separately from spouse - For those of you in one of the lucky few “community property” states, you also have another lever we can pull on for you. AZ, CA, ID, LA, NV, NM, TX, WA, and WI are all community property states which means that all income that is earned annually by you and your spouse is legally considered community property. This means that when you file your taxes separately, instead of simply being able to isolate your income from your spouse’s, you can also submit additional paperwork (Form 8958) to have them draw that line right down the middle.
As an example, if you are making $300k and your spouse is making $100k, in a normal filing taxes separate situation, you could isolate your $300k of income and must report that to the Department of Education for your payment's sake. But in a community property state, you get to cut your household income of $400k directly down the middle.[3] You can report it as if you made $200k and your spouse made $200k. This means that when you go to report your income, you get to report the $200k instead of the $300k of income! This can be HUGE savings for clients and the bigger discrepancy between your and your spouse’s income, the more impact this will have on your situation.

Query: Difficulty With Assisted Living Facilities
I have been having a problem billing Medicare Advantage plans (especially Aetna Medicare and United Healthcare Medicare) for my services in assisted living facilities. I bill them using the new CMS guidelines procedure code CPT 99348 since CPT 99335 are deleted in 2023 and using POS code 13 for assisted living facilities. They have been denying claims stating the POS code 13 is wrong for CPT 99348. This is only happening with Medicare Advantage plans. I practice in New Jersey.
Does anyone have any insight how this should be billed? Maybe POS code 12 instead of 13? Also, if you have to use PS code 12 for Medicare Advantage plans, what should be placed in box 30 on HCFA 1500 form? Should the facility address be the patient home address if using POS 12?
PM News Subscriber
Response: You are billing this correctly using the new CMS Guidelines. This is a computer issue that needs to be addressed by Aetna and their system.

| RESPONSES/COMMENTS (CLINICAL) - PART 1A | |
From: Michael Orosz, DPM
I’ve recently started using a product from PediFix called Visco-Gel Hammertoe Crutch. These are especially effective for those patients with long toes that plantarflex over a crest pad.

| RESPONSES/COMMENTS (CLINICAL) - PART 1B | |
From: Edith James, CPed
Pedifix’s Pediplast made into a custom toe separator could be the solution. Custom toe separators can be made to lift and prevent rubbing between toes plus significantly reduce contact with ground forces. These are formed directly to the foot, then cure in minutes. These last very well; some last for more than a year.
These combined with cushioned, moisture-wicking high tech fiber socks have improved the health of many patients. Note: thicker socks usually require more room in footwear, of course. Personally, if current socks are inadequate protection, my presentation of solutions starts with socks since they’re the first opportunity to help. Custom toe separators, protective cushioned socks, orthoses, then fit footwear are all that are needed here.
Edith James, CPed, St. Louis, MO
