CA Podiatrist Discusses Home Remedies for Sweaty or Itchy Feet
 
It’s not just the stench; sweaty feet can also cause cracked heels, lizard-like skin, and even fungal infections. Each of your feet has about 250,000 sweat glands, which can churn out half a pint of sweat every day. To minimize the amount of perspiration that reaches the surface, mist your feet with an antiperspirant/deodorant spray every morning and night, says Wenjay Sung, DPM, a podiatrist and foot-and-ankle surgeon in California; the metals in antiperspirant will block your sweat ducts.

Itchy feet? Try a vinegar bath. Once an infection develops, Dr. Sung swears by a foot bath of malt or white vinegar. It won’t cure the infection, but will soothe the irritating itch, he says.

WY Podiatrist Participates In Wind City Running Expo
 
Saturday saw the first Wind City Running Expo at Wind City Physical Therapy. Specialists in biomechanics, nutrition, sports gear, and podiatry were all on hand to help both beginners and experts navigate training plans and manage injuries, including the dreaded ingrown toenails and plantar fasciitis. “It’s just important to have a very, very gradual training program,” said podiatrist Michael Wilkinson, DPM. “Most injuries happen in running when they do too much, too fast.”

Dr. Wilkinson said arch taping, night splints, cortisones injections, and stretching were all ways to treat various injuries, but added that the “number one [treatment] is rest.

Aussie Podiatrist Sees Surge in Overuse Injuries
 
Podiatrists and physiotherapists are reporting a surge of patients suffering overuse syndrome after taking up exercise during the pandemic, often as an antidote to lockdown lethargy. Jeffery Jenkins, a podiatrist and director of OnePointHealth in Penrith, said there had been huge demand for services once people came out of the most severe lockdown. “Everyone started exercising,” he said. The biggest cause of lower limb pain was sudden overuse. “When bodies aren’t used to it, it causes arch pain, plantar fasciitis, shin splints, and Achilles tendonitis.”

Research by the Australian Podiatry Association conducted last October found that 70 percent of Australians wore less supportive shoes than usual or went barefoot when working at home. One in three reported more pain or discomfort in their feet and lower limbs, and had reduced walking or exercise as a result.

UK Podiatrist Converts Public Loo Into State-of-the-Art Clinic 
 
An abandoned public loo has been given a new lease on life as a state-of-the-art health clinic after it was fully converted by its new owner. The abandoned public building has been fully refitted and rebranded as Active Health under the leadership of podiatrist David Cashley.

The former loo is staffed by four podiatrists, two physiotherapists, and a massage therapist, with a chiropractor soon to join them. “We can use things like our gait analysis machine to take a look at how they’re walking and suggest ways that we can make things easier,” he said.

ON THE INTERNATIONAL LECTURE CURCUIT
CA Podiatrist Keynotes at International Foot and Ankle Biomechanics Symposium
 
Dr. David G. Armstrong, Professor of Surgery at the Keck School of Medicine of University of Southern California joined physicians, surgeons, engineers, and scientists from around the world for the International Foot and Ankle Biomechanics conference. Originally scheduled for Rio De Janiero, Brazil in April of 2020, the symposium- known as iFAB was rescheduled in a virtual format.

Armstrong, who gave the lecture outside of the high-risk clinics at the National Rehabilitation Center at Rancho Los Amigos, gave a keynote to participants from 5 continents. It was particularly poignant that the lecture be given at Rancho, as much early work in gait analysis and biomechanics is credited to clinician scientists from that institution for over more than 50 years.

RE: Effect of Low-Intensity vs High-Intensity Home-Based Walking Exercise in Patients with PAD

A study was reported by McDermott, ML, et al. in the Journal of the American Medical Association regarding supervised high-intensity walking exercise that induces ischemic leg symptoms and that this is the first-line therapy for people with lower-extremity peripheral artery disease (PAD), but adherence is poor. A multicenter randomized clinical trial included 305 participants (i.e., 159 men and 146 women, mean age 60 years old) with PAD employing low-intensity exercise, high-intensity exercise, and non-exercise control resulting in a mean 12-month change in 6-minute walk distance of −6.4 m, 34.5 m, and −15.1 m, respectively.
 
The study concluded that low-intensity exercise was significantly less effective than high-intensity exercise. The authors concluded that the results do not support the use of low-intensity home-based walking exercise for improving objectively measured walking performance in patients with PAD (JAMA. 2021;325(13):1266-1276) .

The following is not directed particularly at Dr. Sigle, and I realize that  reporters can sometimes publish information out of context. The Internet is now full of foot surgeons advertising the newer minimally invasive bunion procedures. Many of these advertisements claim that patients will have a faster recovery with the minimally invasive techniques. To my knowledge, there is no significant evidence that this is true. There are studies that indicate patients may have less pain in the initial post-op period with the minimally invasive techniques, but I have not been able to find a peer-reviewed article that says anything about the recovery being faster.
 
I have done a fair number of minimally invasive bunionectomies. That is to say enough of them to start seeing and managing complications in my own patients. I offer the minimally invasive option when I feel it is appropriate for the patient. It is by no means a panacea and there are many situations where it is not, in my opinion, the best technique for a given patient. My post-op protocol for these cases is much the same as for my open procedures. As a profession, I think we need to be careful that what we promise is what we can deliver and what is evidenced to be true.

Dr Ribotsky's comment about medical errors that he caught hit home for me. For many years, I was a caregiver for my late parents and both had multiple visits to at least two hospitals. I vividly remember arguing with one attending who wanted to order a CT scan with contrast for my mother who was suffering with advanced renal disease. I had to alert her renal specialist who came in and thankfully prevented the test.
 
Errors are the norm, not the rarity in our medical system and arrogance makes it worse. The only future solution to these problems is if hospitals could computerize many medical decisions in a way that a red flag would be raised if a physician was about to order a treatment that is not in the patient's best interest.

I read with interest Dr. Ribotsky’s response about records today. He is correct in that the charts are filled with needless information. I have performed chart reviews for med mal cases and it is common for the same note to be copied and pasted throughout the chart, adding little to what was going on with the patient's care. Ultimately, many times, it leads to disastrous outcomes for the patients and the doctor when they face a malpractice claim. Perhaps the good old days were the best.
