Rural Patients Need Access to Specialty Care: ID Podiatrist
Lost Rivers Medical Center is a tiny brick hospital nestled on the snowy hills above this remote town in central Idaho. Lost Rivers has only one full-time doctor and its emergency room has just three beds – not much bigger than a summer camp infirmary. Dr. Tim Tomlinson, a podiatrist who lives in Twin Falls and drives 100 miles once a week, spent a recent morning seeing a lineup of patients. One was a man who had to have a toe amputated after a horse stepped on his foot, another a diabetic who needed a skin graft checked on his foot.

Tomlinson said he has gotten paid late before, and he has seen the hospital nearly shut down more than once. But he keeps coming because he has developed a practice, and considers it important that patients have access to specialty care. Lost Rivers isn’t unique in its difficulties, he noted. “All those small towns are struggling as young people move out, leaving mostly old people,” he said. “That puts a drain on the hospitals.” Patients are living longer with chronic diseases now, so the demand for elderly care is only going to increase. If not the rural clinics and hospitals, Tomlinson said, “who’s going to deliver it?”

Dr. Storts graduated from the Temple University School of Podiatric Medicine. He completed his residency at New York Methodist Hospital in Brooklyn, where he focused on reconstructive foot and ankle surgery and limb salvage. Storts has authored several scholarly publications in academic journals and has lectured on topics related to foot and ankle surgery.

Cutting Podiatry Services Will Not Save Money - Canadian Podiatrist
Cutting podiatry services in Saskatchewan, Canada is projected to save the provincial coffers $1.2 million. Many patients requiring podiatry services are diabetics with chronic foot and ankle wounds, said Dr. Ata Stationwala, a Moose Jaw podiatrist and president of the Saskatchewan College of Podiatrists. “I don’t think there is a full understanding of what we do and the impact it’s going to have on the health and well-being of residents,” Stationwala said. Treating ulcers requires regular follow-ups, Stationwala said. “If they don’t heal or if they continue to get infected, that’s what leads to possible amputation of the limbs,” he said.

He’s concerned that increased fees will deter people from getting the care they need and this inevitably mean more visits to emergency rooms for preventable foot and ankle complications. “Many of these patients will end up with lower limb amputations — amputations that could have been completely prevented if the services had been maintained,” Stationwala said. “What’s the cost to the system of one amputation?”

The American Academy of Podiatric Sports Medicine (AAPSM) President Amol Saxena, DPM met with Drs. Dominic Catanese and Eric Walters, and the residents of the Montefiore Hospital Residency program. Dr. Saxena was also able to meet with students from the New York College of Podiatric Medicine (NYCPM).

Third Transatlantic Conference a Great Success
The Third Transatlantic Wound Science and Podiatric Medicine Conference, an annual conference put on by partners NYCPM and the National University of Ireland (NUI), Galway, was by all measures a successful and enjoyable conference. This year, 300 attendees from the U.S., Ireland, and Europe experienced two days packed with cutting-edge presentations, workshops, and panel discussions from early morning until evening.

NYCPM’s VP for Academic Affairs and Academic Dean Michael J. Trepal, DPM, co-chaired the conference with Caroline McIntosh, PhD, MChs, Head of Podiatric Medicine, NUI Galway. NYCPM presenters included Eileen Chusid, PhD; Robert A. Eckles, DPM, MPH; Anthony R. Iorio, DPM, MPH; Susan Rice, DPM, as well as Dr. Trepal, who said “we look forward to the 4th Transatlantic Conference in March 2018, which will take place as part of Superbones East.”

Query: Missed First Appointments
I have a problem with patients missing their first appointments and wonder if it is acceptable to run a credit card to hold this first appointment so that it isn't missed without notice. Would this be an acceptable practice? Is there anyone else doing this? Are there any other ways to keep the new patient missed appointment from happening?
All my patients sign a waiver which allows me to bill them for their missed appointments, once they are an established patient, and this works well to keep the missed rate down.

I have never seen the dilute 4% dehydrated alcohol version available commercially. It is usually supplied in the 98% version of ethanol and is listed as Dehydrated Alcohol for Injection, USP. I think it is still available online or through Moore Medical Supplies.

The responsibility for controlling one's medical cost rests with the patient (beneficiary). For medications I use on a regular basis (90-day prescriptions), I research the cost. My sources are the website for my prescription plan and GoodRx.com. When I ask my doctor for a refill, I have the pharmacy where I want the Rx to go and make the request. Additionally, I have the app for GoodRx.com on my phone just in case my doctor wants to give to me a new Rx so I can explore the "CASH" price prior to getting the prescription.
I also look up the cost of prescriptions. I am able to show the patient a cash price for that drug as well as the different prices at the different pharmacies in the area. The patient can then decide what pharmacy they want based on cost. I am a little surprised that the State of NY requires only ePrescribe; I thought that was related to CMS, not to the state.
Disclosure: I have no financial interest in goodrx.com. There are also many discount Rx services.
