2 TX Podiatrists Reappointed to Podiatric Medical Examiners Advisory Board
Governor Abbott has reappointed Cory Brown, DPM and Travis A. Motley, DPM to the Podiatric Medical Examiners Advisory Board for terms set to expire on February 1, 2029. Dr. Brown is a partner at West Texas Health Clinic. He is a member and Texas delegate to the American Podiatric Medical Association, past president of the Texas Podiatric Medical Association, and an associate member of the American College of Foot and Ankle Surgeons.

MI Podiatrist Targeted by People Posing as DEA Agents
The Drug Enforcement Administration (DEA) has issued a warning that callers are posing as DEA agents. John Evans, DPM, a podiatrist in Allen Park, said he received a distributing call at his office on March 27. The message was supposedly from a DEA agent investigating him. "There was an abandoned car that was found in southeast Texas that had over 50,000 tablets of controlled substances in it," Dr. Evans said.

The caller claimed when they traced the car, it led investigators to two homes in southern Texas filled with drugs, cash, guns and his medical information and name. That night, Evans looked up the agents' names on LinkedIn and messaged one of them. Thankfully, that agent got a message through to the DEA office in Detroit. They contacted Evans to let him know the callers were imposters.

Flat Foot Types Least Likely to Benefit from Going Barefoot: NJ Podiatrist
Dana Canuso, DPM, a board-certified podiatrist and podiatric surgeon, told Shape that "Our home-based lifestyle has caused a significant change in the shoes we wear." She added that "Many of us have shifted from sneakers and boots to slippers and [being] barefoot, and this change significantly impacts many aspects of foot health." We don't really need to buy office footwear like high heels or trendy shoes as much anymore. Instead, people can stock up on comfy slippers and socks. However, as Canuso noted, this change largely affects your feet.

Despite the many findings that encourage being barefoot, full-time shoelessness isn't the best option for everyone. Before you go chucking out your shoe collection, it's important to note that not all people will benefit from going barefoot. You might want to check out your foot type. In conversation with Shape, Dr. Canuso warned that "flat foot type[s] are predisposed to more injury from not wearing shoes since they're already lacking the support needed to promote a neutral foot position."

Enovis Expands Foot and Ankle Portfolio with Knotless Repair System
Enovis has announced the expansion of its foot and ankle product line with the introduction of its Enofix with Constrictor technology. The company said the knotless syndesmotic repair system offers an improved solution for surgeons treating syndesmotic injuries, which commonly result from ankle fractures. The new system demonstrates superior fixation under cyclic loading, according to Enovis.

Cigna Physicians Deny Claims en masse Without Reading Them
Cigna physicians deny large batches of claims without reviewing them first, a process that may save the company millions of dollars every year when members don't appeal, ProPublica reported March 25. According to the report, an internal claims system at Cigna allows medical directors to deny a claim without opening the patient's file. The report claims that Cigna physicians denied more than 300,000 claims over two months in 2022 through the system, which equated to 1.2 seconds of review per claim on average. "We literally click and submit," a former Cigna physician told ProPublica. "It takes all of 10 seconds to do 50 at a time."
Many states require medical directors to review patient files and coverage policies before denying claims for medical reasons. ProPublica alleges that Cigna has bypassed these steps by having a computer algorithm complete the review and then having physicians sign off on groups of denied claims.

Student Loan Concepts to Know During Tax Season
With tax season approaching, many people are focused on gathering all the tax forms to make sure they don’t miss the April deadline; however, few will pause to inquire how their tax filing could impact their student loan payment. Fortunately, there are many ways that borrowers can get creative with income reporting come tax season that could impact their student loan payments substantially.
10A. Filing separately from spouse - For married clients, the tax filing status is incredibly important. Another perk to the rule that you must report off your AGI on the tax return is that we are able to have you file separately from your spouse to isolate only your income for reporting's sake. This is incredibly important for folks who have a spouse who has a high income that you don’t want to be included in your monthly payment calculation. As an example, if you have a spouse that is making $150k/yr, filing taxes separate could save you upwards of $1250/mo ($15k per year!) on your student loan payment in some instances.
Although this is a big number, filing taxes separate comes with its own list of pros and cons and so it will be important to have your student loan advisor connect with your CPA to evaluate the options. If you save more by filing your taxes jointly (on the tax side) than you do by filing separately (on the student loan side) then it is likely not worth it to file separately. There are also tax benefits that you could be missing out on by filing separately but at the end of the day, it is simply a math problem. By running both scenarios, the answer will be clear as day

Dr. Gurnick, in his response to my post, states: "I think that an x-ray is an x-ray and should be billed with an x-ray CPT code. An image obtained with a fluoroscopy machine, whether static, dynamic, non-weight-bearing or somewhat weight-bearing is still a fluoroscopic image, and probably should be billed as such." He states that I should get a legal opinion before informing readers of this being my billing protocol.
1. Dr. Gurnick, has provided his opinion here on billing digital flat plate images. Does he not need the same legal advice he is admonishing me to obtain?
2. Fluoroscopy, CPT code 76000 bills via RVU 1.24. Two radiographic views of the foot, 73620 bill at RVU 0.812. Three views of the foot, 73630 at RVU 0.983. So, if we bill such images as fluoroscopy per Dr. Gurnick's suggestion, we are billing at a higher rate. As such, are we not in more "jeopardy" by billing a static image as fluoroscopy?
3. The distinction between radiographs and fluoroscopy has changed with changes in technology. Radiographs are static images while fluoroscopy provided real-time images and allowed observation of motion. The advent of digital radiography led to the use of imaging receptors. Flat panel receptors have been added to many models of fluoroscopy to allow digital radiographs to be taken. It does not matter that a "fluoroscopy" machine was used to take such images as the images are still static radiographic images.
