NC Podiatrist Volunteers in COVID-19 Vaccination Effort
Gretchen A. Lawrence, DPM of Foothills Podiatry in Shelby, North Carolina volunteered on April 10 at UNC Asheville Reuters Center. The Janssen COVID-19 Vaccine, a company of Johnson and Johnson, and the third vaccine approved by the U.S. Food and Drug Administration for emergency use in the prevention of the corona virus was distributed.

Dr. Lawrence educated the participants of potential side-effects of this vaccine and ways to report those effects back to the FDA and CDC through the FDA/CDC Vaccine Adverse Event Reporting System (VAERS). She assisted a patient with anxiety and syncope and assisted a patient who lost consciousness and developed nausea and vomiting after the injection. She also helped a patient with severe swelling and arranged for further emergency medical treatment via transport to the hospital for further management.

NY Podiatrist Discusses Heel Pain
You swing your legs out of bed, hit the floor with your feet and —ouch! — your heels are hurting. "Heel pain is one of the top five reasons that patients go to a podiatrist," Nelya Lobkova, DPM, a podiatrist at Step Up Footcare in New York, tells LIVESTRONG.com. The heel is a weight-bearing bone; constant repeated stress of walking barefoot at home, standing at a desk, or taking up running as a new hobby are a few reasons why foot docs have been seeing an uptick in heel pain problems lately.

As for why heel pain strikes in the morning? When you're resting in bed, the plantar fascia —the fibrous connective tissue that runs from heel to toe along the bottom of your foot — is contracted, Dr. Lobkova says. "The first step out of bed aggressively wakes up and stretches the fascia, causing pain and micro-tearing where the [tissue] inserts in the heel," she says. Then, the heel hardly has a chance to recover as you constantly move about during the day.

Superior Court Rules in Favor of PA Podiatrist in Malpractice Suit
The Pennsylvania Superior Court on Friday refused to overturn a jury verdict in favor of a podiatrist who allegedly fouled up a surgery on a patient, saying the jury had plenty of evidence to support the conclusion that the doctor was not negligent. A three-judge panel affirmed the trial court's decision not to reconsider the verdict in Nicole Heyer's suit against Dr. Rosemarie Rynkiewicz and Achilles Foot Center PC, rejecting Heyer's argument that she had never given informed consent prior to the surgery.
Rynkiewicz performed the surgery in June 2010 to deal with a lesion on the bottom of Heyer's fifth metatarsal head, or the bone at the base of her pinkie toe, and nearly a year later, Heyer complained that her pinkie toe was overlapping with the next toe over. She sued in July 2012, alleging negligence and lack of informed consent. Based on that testimony and evidence in the record, the appeals panel found that the jury had plenty of evidence to conclude that Rynkiewicz had obtained informed consent and was not negligent in treating Heyer.

Query: United Healthcare and Proof of Delivery
How can I bill United Healthcare for L3000 without showing proof of delivery? Can I bill the patient upfront for orthotics and then reimburse the patient if and when UHC pays for them?

Response: All DMEPOS under Medicare requires Written Proof of Delivery (WPOD). If UHC is now requiring WPOD (as stated in the foot orthotic policy), then you cannot bill for these items prior to delivery. If you are participating with UHC, then no, in most cases, you cannot bill the patient for the device unless you have performed your due diligence and ascertained that it is not covered. You can have the patient sign a financial liability statement stipulating their financial responsibility should the device not be covered.

RE: Methods of Diagnosing Onychomycosis

The most prevalent nail condition worldwide is onychomycosis, with 10 million physician visits for nail fungal infections according to a National Ambulatory Medical Care Survey from 2007 to 2016, more than double the number of all other nail diagnoses combined. Diagnosing onychomycosis using clinical examination alone has poor sensitivity and specificity and may lead to progression of disease and unwanted side-effects from inappropriate therapy. A useful adjunct is dermoscopy but it is still inferior compared to mycologic testing.
Classical methods of diagnosis include potassium hydroxide staining with microscopy, fungal culture, and histopathology. Polymerase chain reaction is a newer technique with wide accessibility and excellent sensitivity and specificity. Artificial intelligence recently has been studied for the diagnosis of onychomycosis but mycologic confirmation is still necessary before initiating treatment (Shari R and Lipner M; Cutis. 2021 March;107(03):113-114).

I read with such excitement this week's survey regarding how people enter and generate clinical notes for their practice. But it begs us to re-evaluate why we do this. When I was a student, we were all taught that you generated significant information in your medical records so that if one day, an accident happened and you could not see your patients, any other competent physician could pick up your notes and be able to treat the patient competently without skipping a beat.
Over the last few decades, there seems to be a significant paradigm shift. We started transcribing notes to justify billing and third-party reimbursement and most recently, we added on top of that medical malpractice protection. For example, it’s crystal clear that not every DPM checks for pulses on every patient on every visit but it’s important that you document this for reimbursement and legal protection. Today, as a patient, I get copies of beautifully transcribed and computer-generated notes that go on for pages that have nothing to do with why I am being treated. It seems that the data has become noise and is of rare value to the practitioner.
I have witnessed multiple errors because the important information is so difficult to find within the notes. For example, allergies, drug interactions, and contraindications have disappeared from a simple sticker on the front of the chart; thus, this is no longer top of mind for the physician. For example, when I was on a blood thinner post DVT/PE, I was shocked how many times I was offered an NSAID. Does anyone think that the care of the patient has gotten better since the change in notetaking over the last decade or two?
