IAFS Appoints MA Podiatrist Scientific Chair for Their Annual Meeting
 
The International Aesthetic Foot Society (IAFS) has announced that Adam Landsman, DPM, PhD will serve as the scientific chair for this year's annual meeting to be held in New York City on October 5 & 6, 2018. The conference this year will feature plastic and orthopedic physicians from Spain, along with physicians from around the U.S. The lectures will be focused on a variety of procedures that emphasize an aesthetic approach to solving various mechanical, functional, and painful conditions of the foot.

Lectures will include some of the latest minimally invasive and percutaneous procedures that emphasize curative surgeries with aesthetically pleasing results. In addition to bone and tendon surgery, the program will include the use of biologics and fillers to promote healing and reduce pain. Various innovative injection techniques, such as "Pillows for Your Feet", will be demonstrated live by Suzanne Levine, DPM, who developed the procedure for patients with metatarsalgia.  Also, this year, a simulcast option will be available.

AAFAO Holds Inaugural Meeting
 
The American Academy of Foot and Ankle Osteosynthesis (AAFAO) held its inaugural meeting at its new location in the Dallas/Fort Worth (DFW) area on April 5-7, 2018. Its first-rate resident education program showcased cutting-edge surgical procedures and timely topics in foot and ankle reconstruction. The program provided both comprehensive and advanced didactic courses and hands-on workshops for 200 professionals, residents, and fellows-in-training. Lively debates on controversial topics were well received.

Early feedback was distinctly positive regarding the academic program and new location. “We’re really excited to be the premier educational organization for residents and fellows,” stated Dr. Jack Schuberth, Chairman of the AAFAO board. “We will have the opportunity to provide these courses two more times in 2018, such that all residents and fellows can be accommodated.” The organization plans to expand its educational programs in 2018-2019 to include arthroscopy, cadaveric surgery, and an online lecture series.

Bianco Bros. Continue Instrument Sharpening Business Tradition
 
For decades Joe Bianco estimates he has sharpened between 50 and 100 hairdressing scissor, and surgical, gynecological, orthodontic and podiatric, and other instruments a day. On this Monday afternoon, we find Joe’s twin brother, Vincent, sharpening a knife on a sit-down grinding wheel built during the First World War.

The Biancos receive blunt objects (“paper cutters and lawn mower blades,” Joe said. “We’ll get the occasional sword swallower in.”) from all over this city, state, country and as far away as Milan and Rome, and in their Dyker Heights shop return to those tools their edges. Vincent and Joe own and run this operation with their brother Dominick. Their dad, John, a Brooklyn hair stylist and salon owner, purchased the sharpening business in 1972. The Bianco brothers continue sharpening in their father’s memory but also build and sell their own brand of scissors, snippers, and other instruments under the name Bianco Brothers.

Best Ways to Play the Prior Authorization Game - Part 3
 
There is nothing more frustrating to physicians than knowing a patient needs a certain diagnostic test or medication and having them not be able to get it because their health insurance company won’t cover it. All too often, many services require prior-authorization (PA). How can this prior-authorization game be played for better odds of winning?
 
 
3. Learn what is needed to get an approval. One example is that with certain insurance companies, every time I order an MRI of the lower back, they want the patient to have had a plain x-ray first. Why? There is no evidence that x-rays are a good test to diagnose back problems. In fact, if I am looking for a herniated disc, a plain x-ray will not show it but rather an MRI is needed. I know this but sometimes the only way to get the patient to get the test is to do it anyway.

We wanted to respond to a post in the March 22, 2018 edition of PM News by Dr. Craig Breslauer, where a Cartiva Facebook page was referenced. Cartiva, Inc. does not have a corporate presence on any social media forum, including Facebook. We have never asked or endorsed a podiatric or orthopedic surgeon or employee to participate on our behalf as a moderator on any social media site. 
 
We receive thousands of contacts each year from patients suffering with painful osteoarthritis of the great toe, inquiring if a Cartiva synthetic cartilage implant could be an option for them. We refer these patients to foot and ankle surgeons nearest to them who have been trained on Cartiva, without regard to specialty. Our belief is that a well-trained foot and ankle surgeon is just that, without regard to professional title.
 
Thank you for the opportunity to clear up any confusion about our ongoing support of the podiatric community.

We got the same denial reason. We called Noridian about it. There was a system glitch causing the denials. They are aware of it and will be reprocessing the claims automatically when the system glitch is corrected. We flagged our chart to follow up on it in 30 days if it isn't paid yet. If you are certain your coding and modifiers are correct, you don't need to do anything at this point.

With all due respect to Dr. Lipkin, the decision to drop out of the insurance networks is not a "knee jerk" reaction as he implied. And as he said, "thinking with your head, and not your heart", is exactly why he and others should drop out. If anyone in solo practice thinks they are going to negotiate better fees with any of these insurance companies, they suffer from delusional personality disorder. It's not going to happen. If you think its all right to devalue your services on one hand to get thrown some crumbs for another service, then continue to practice that way. 
 
But if you're looking for real solutions, put your big boy pants on and stop putting up with it. I remember some time ago the late Neal Frankel, DPM met with our division in NJ and told us something that stuck with me. He said the CEO of one of the larger insurance companies told him and I quote, "why should we pay podiatrists more when we know they'll work for less."  That statement continues to resonate, and its one of the reasons I'm out-of-network today. I only wish I had done it sooner.

UHC’s reimbursement schedule methodology, at least regarding podiatry, is simple. Reimburse the least amount of money for a service that a critical mass of podiatrists will accept for that service. Critical mass of providers is the least amount of providers based on: a. the number of beneficiaries the insurance company covers to provide podiatric services and/or b. the number of providers required by law based on the number of beneficiaries. The value of podiatric services is solely based on cost, with minimal value placed on quality. Again, price is based on supply and demand without regard to the value of quality to the consumer of podiatric services. All healthcare based on this model will result in continuous deterioration of our healthcare system.
 
Personally, I am not an advocate of government interference in private industry; however, if the industry is defined as a right, then oversight regulation may be a method to control the cost of healthcare and keep it in the private sector.

Many years ago, Dr. Allan Levy and I were the physicians for the New Jersey Americans (ABA), the Westchester Bulls, and the New York Football Giants. Along with his excellent knowledge of sports medicine and his kindness, he taught me about what sports medicine is all about. We traveled from the Teaneck Arena to the Island Garden to treat and deal with the players. I mourn his passing.

This pro golfer could have been a podiatrist!
