More than two-thirds of the population has one foot that’s larger than the other. This rarely exceeds one-half size. However, this should be considered when buying new shoes. Always choose your size based on the fit of the larger foot, since the fit of the smaller foot can be easily adjusted by adding an insert or arch support. That’s why it’s important to try on both shoes. If a shoe fits properly, there should be a half-inch of space between your longest toe and the front of the shoe, leaving plenty of room to wiggle your toes.

Drunk-Driving UK Podiatrist Has License Suspended
An NHS podiatrist has been suspended after she drunkenly left a wedding at a posh country estate and crashed her car on its grounds. Abigail St. John was taken to the station where a breathalyzer found she was four times over the blood alcohol limit and she was handed a €500 fine (£438.68) and banned from driving for three years. A tribunal heard that when London-based podiatrist St. John returned to the city, she didn’t tell the NHS about her conviction – claiming to be "confused’"about Brexit protocol. Her podiatry license has been suspensed for six months.

Student Loan Concepts to Know During Tax Season
With tax season approaching, many people are focused on gathering all the tax forms to make sure they don’t miss the April deadline; however, few will pause to inquire how their tax filing could impact their student loan payment. Fortunately, there are many ways that borrowers can get creative with income reporting come tax season that could impact their student loan payments substantially.
9. Pretax deductions - Now that we understand the rules and the reason for wanting to reduce our reportable income for student loan's sake, let’s talk strategy. Since the Department of Education uses your AGI, this means that we can reduce that number through pretax deductions like retirement plan (401(k), 403(b), TSP, 457b, SEP, SIMPLE IRA, etc.) as well as HSAs, individual retirement accounts, or alimony. Most of our clients have access to an employer retirement plan that may allow you to contribute up to $22,500 each year on a pretax basis. This would therefore reduce your adjusted gross income commensurately.
By reducing your AGI by $22,500/yr, this could reduce your payment by about $187.50/yr on some plans and even up to $281.25/mo on other repayment plans! Keep in mind that this is simply a way to reduce your student loan payment and there are strong arguments suggesting that making contributions on a Roth basis to those retirement plans could end up saving you more in taxes than you did on the student loan front. Tough to run the exact numbers without having that crystal ball of future income/tax rates and so be sure to discuss the pros and cons with your financial advisor.

Query: Medicare Advantage Plans And CPT 11750
Recently, we have been getting denials for nail avulsions, partial or complete, with matrixectomy. ( l60.0 coded with CPT 11750 and toe modifier). Further digging revealed that this is happening, to our knowledge, only with Aetna STRS Medicare Advantage plans. When calling Aetna, if we are lucky enough to reach a human representative, we are being told it is not a covered benefit.
Are they not required to follow Medicare guidelines when processing claims? The EOBs state that the determination was based on an LCD. We obtained a copy of the LCD and there is nothing indicating this was performed or billed inappropriately. Has anyone else run into this issue?

Response: We have been having the same issue. You are correct; they are supposed to follow Medicare guidelines. I have had to do an appeal for each one and they do pay, but it is just the work you have to go through is annoying. I feel they do it on purpose so that we either do not follow up or we just write it off. It is really frustrating!

If you use 1/16th inch adhesive-backed felt instead of cotton roll, it will be much more durable and easy to customize for your patient. Cut the felt to width and then roll it up like a cigarette. Just make sure that the patient removes it nightly and for bathing.

Doctor Ricciardi, please re-read the posts on this topic as you have misunderstood it. There is no 3.3% increase for you or any of us. That increased payment is being paid by our government to the insurance companies that administer the Medicare Advantage plans, as if they need to profit some more! Physicians are getting an annual pay cut with decreased reimbursement for Medicare patients that we see. In 2023, I believe it was somewhere in the 1-2% range, so if you factor in the 6-8% inflation rate of your expenses increasing, that's a 7-10% paycut this year alone. This doesn't even take into account the 2% "sequester" pay reduction that has been in place for years now. Just compare your Medicare EOBs from 2022 to 2023 and the sad truth becomes evident. The 2024 pay cut is currently set at 1.25%.
There was some good discussion about this topic 6 months ago on this forum and people seemed up in arms, but once again nothing seems to be happening as we have no effective voice to the politicians. The insurance companies are in the driver's seat and have the voice, lobbyists, and financial backing to get what they want. Without unionizing, we seem to have no voice.
