IL Podiatrist Discusses Injury Prevention With Soccer Coaches
 
Last week, Chicago Fire Junior soccer coach Ty Bishiri and league administrator Steve Morris hosted a meeting for their coaches stressing safety, parent participation, and coaching young players from 10-17. Lower extremity foot, ankle and foot-related concerns were emphasized.

Dr. Bob Weil discussed the concerns such as cleats and heel problems in young players, as well as shoe criteria. Treatment and prevention of acute and chronic injuries were key topics. Attention to foot mechanics and the importance of foot and ankle strengthening were also highlighted. Other important topics included indications and use of orthotics in soccer, and the value of routine podiatric participation in the evaluation and care of these young athletes.

Wrong Type of Shoes Is Primary Cause of Most Foot Problems: Canadian Podiatrist  
 
According to Dr. Tracy Andrews, a podiatrist for 26 years, one of the worst things we can do for our feet is to wear the wrong type of shoes. "Wearing completely inadequate footwear is the primary cause of most foot problems. If shoes don't fit correctly, they cause corns, calluses, and ingrown toenails. If they don't have proper support, they cause strain of immediate muscle groups. For the most part, we just wear the wrong footwear," Andrews said.

With sandal season just around the corner, Andrews highly recommends avoiding flip-flops, really high-heeled shoes, and slip-ons. "If you only wear them once in a while, for the vast majority, that's okay," said Andrews. "For heels, it's best to wear a wedge that is 1.5 inches high or less. If a shoe hangs onto your foot and has ankle straps, it is much better than anything you slip into. It doesn't matter if it's a $2 or $200 flip-flop, if you have to claw your toes to hang on to them, in the long term, that's bad for your feet."

Paul Kesselman: Today, TENS units are affected. While you can dispense them, the price will be the lowest price that was “competitively bidded down in your area.” Going forward, this will hit podiatry regarding a lot of the new HCPCS codes, for example L4360X code. Medicare gave the new codes the old definition and the old codes were given a new definition, and this is set up, I think, to allow some of these products to be included in the bidding process.

THE FUTURE OF HEALTHCARE
The Economics of the New Healthcare Era: Part 1 - What Has Been Happening?
 
The following events have been taking place the past few years. It was decided in 2012 that there was a need for strong healthcare analytics. Along with the need for analytics in healthcare was the need for analytic solutions which would help the healthcare system in regulatory compliance, reducing hospital admissions and hospital stays. Also, solutions were needed to address the prevention of chronic disease, improve the quality of care, increase fraud detectionand most importantly, reduce the cost of care. During the past five years, initiatives were set in place to identify and reduce the cost of practices that were associated with a high dollar amount and to reduce escalating CPT billings. Prescriptive analytics were set in place to better forecast costs and the quality of care through data collection, data sharing, and improved actuarials. 
 
The results we have seen to date (and there are more to come), are systematic quality improvements along with cost reduction, enhancing the physician-patient relationship and clinical risk intervention with population management. This has also led to new technological advances in healthcare monitoring of the patient, a steady upward increase in the number of the physicians extenders, changes in licensing laws and scope of practice laws of healthcare professionals (telemedicine), patient direct access to nurse practitioners, and the incorporation of new alternative payment models. By the year 2020, there will be little resemblance to the healthcare landscape we see today.

"If you own your practice, a simplified employee pension (SEP) IRA plan gives business owners an opportunity to establish and make tax deductible contributions for their employees’ retirement as well as their own retirement plan. For the 2015 tax year, employers can contribute up to 25 percent of compensation or $53,000, whichever is lower. The primary advantage of a SEP versus a traditional IRA is the higher contribution limit ($53,000 versus $5,500). The primary drawback for this type of plan is that it requires employers to make a contribution on behalf of their employees as well as themselves."

Query: L2275 Billed with Orthotic Forefoot Posting
 
Does anyone have experience billing L2275 (addition to lower extremity, valgus or varus correction) when the orthotic needs forefoot posting at the time of billing L3000? Our local pedorthic lab does this and suggested that we do as well.

Response: L2275 (addition to lower extremity, valgus or varus correction; plastic modification, padded/lined) can be found in the "Additions to Lower Extremity Orthosis - Shoe-Ankle-Shin-Knee" HCPCS section. 
 
This is separate, preceding the foot orthosis coding which resides in the section headed by "Foot (Orthopedic Shoes) - Insert, Removable, Molded to Patient Model" beginning with L3000. It would be inappropriate to choose a code for an AFO addition item and apply it as an addition to a foot orthosis device code.

RE: Cloning of Notes

Since the advent of EMR, much has been said about the "cloning of notes" in your patients' medical records. I have read that some of our colleagues recommend changing one or two words in the entry to give the impression of entering a new note. I think we are better than that. Often, when I see a patient every two or three months, the subjective, objective, assessment, and plan might be exactly the same. I then tell my EMR to print the previous note. IT IS WHAT IT IS.
 
If that's what you heard, saw, and did, why in heaven's name would you have a need to change. If a change is made just to satisfy an insurance examiner, I find that to be a poor reason. I, for one, would feel less of a professional. Obviously, if you heard, saw, or did something different, that should be memorialized in your patient's record.

Let’s go one better. Why pay quarterly? Sign up online for EFTPS (Electronic Federal Tax Payment System). Then, pay your tax every WEEK or even every DAY if you choose. That puts YOU in the driver's seat, and not the Feds! The Feds LOVE you to be late. All that wonderful penalty and interest! Stay ahead. Don’t get behind. You do need an accountant to cross the t’s and dot the i’s, and file the returns, but you do not need one to tell you that you owe taxes. You KNOW that! Pay them and pay early or on time.
 
Who knows? You might be like millions of other Americans who get money back! Don’t wait until April 15th and get hit with some huge sum you can’t pay. Too late for this year? Get ready for 2017. You are already one quarter behind for 2016.
