IL Podiatrist Discusses Causes of Pickelball Injuries
The surging popularity of pickleball has resulted in more patients suffering from injuries such as tendonitis, especially involving the Achilles tendon, says Ashley Lee, DPM. Also known as Achilles tendonitis, symptoms include stiffness on the back of the heel and lower leg after sitting. The feeling generally subsides during rest, but returns when activity resumes. "It's caused by excessive stress on the Achilles tendon," Lee said, and is often caused by the short, rapid movements required of not only pickleball, but of tennis and paddle ball, as well.

Stretching before and after activity can help to prevent the injury, Lee said. She also recommends wearing the right shoes. Shoes designated for use on the court offer the stability needed for sudden movements and offer better protection from injury, she said. Pain in the ball of the foot also can result from time spent on the pickleball or tennis court. The frequent pivoting required of the sports can result in metatarsalgia, or inflammation in the long bones in the front of the feet.

Podiatry Screening to be Extended to All Malta Government Schools
Podiatry screening of schoolchildren is being extended to all government schools following a successful pilot project earlier this year, Health Minister Jo Etienne Abela said on Monday. Speaking during a visit to a government school in Sliema, the minister underlined the importance of screening children's feet for early detection and treatment of any problems.

The pilot project screened the feet of 235 pupils aged six and seven. 24 were subsequently referred to the Podopaediatrics Clinic in Birkirkara for further investigation. Abela said he looked forward to the screening service being extended to church and independent schools. The minister was accompanied on his visit by Education Minister Clifton Grima.

| PRACTICE MANAGEMENT TIP OF THE DAY | |
9 Tips for Collecting from Patients
Experts provide nine tips to help practices successfully collect patient balances.
3. Make it easy for patients to pay
“If a small practice wants to survive, it can’t just send a statement and hope patients pay,” says Denny Flint, chief commercial officer at Millennia, a tech-enabled patient engagement company specializing in the recovery of end-to-end patient financial obligation, adding that practices must tailor patient financial engagement strategies to fit the age and tech-savviness of the demographics they serve.
“Portal-driven online bill-pay and mobile payment methods may attract the millennials and Gen-Xers who are comfortable with DIY technology,” says Flint. “However, baby boomers generally prefer the human touch delivered by a U.S.-based call center that gently informs and educates them about why they owe what they owe. COVID-19 has taught us our goal is a seamless, personalized billing experience where one size does not fit all.”

| RESPONSES/COMMMENTS (PRACTICE MANAGEMENT TIP OF THE DAY) | |
From: Joel Lang, DPM
These thoughts occurred to me when reviewing "9 tips".
1) Collect for orthotics "in advance" at the time of casting. I found that the orthotics that generated the most complaints were the ones not paid for. Also, I personally guaranteed my orthotics, not in writing or verbally, but when an orthotic was uncomfortable after many adjustments, I ceremoniously dropped them in the trash can in front of the patient and provided a full credit/refund for their cost. In over 30 years of practice, I don't think I threw away more that 6 orthotics, more or less.
2) Provide all new patients with a "financial policy" form on the first visit, outlining an agreement which they would actually sign. It was "gently" worded but clear in its intent. I probably had about 6 or so patients who objected and left the office in the many years during which I used such a document. It really clears up what is expected of patients financially and reduces collections drastically.
Patients need to understand that it is expensive to maintain and conduct a practice and while finances are not the first priority, it is also not to be ignored. Now that I am retired, I make sure all my doctors are paid in full and on time. Lessons learned.
Joel Lang, DPM, Cheverly, MD

| RESPONSES/COMMMENTS (MEDICARE ADVANTAGE) | |
From: Richard Rettig, DPM
Medicare Advantage (Part C) plans were initiated in order to save money for the Medicare (MC) system. Many people believe that the "Government" is inherently wasteful, and so private industry can accomplish the same tasks more efficiently for less money and still make a profit. The plan was to start them off with a capitation rate above the cost of traditional Medicare, in order to get them on their feet. They could offer more services than MC, such as gym memberships and rides to the doctor as an incentive to join. Then they would eventually lower the capitation to a small amount below traditional MC costs and capture the cost savings. But thanks to lobbying, that never happened.
The Advantage plans proposed to save money by keeping people healthier, utilizing preventive care. But in reality, they capture the healthier patients, who then switch to traditional MC when they really get sick. I see patients in long-term care who are admitted with a managed care plan. Most of them eventually get switched over to traditional MC. It is rare to find a retired doctor who has not chosen traditional Medicare as his/her coverage.
In addition, corporate greed kicked in. They get paid a higher capitation rate for managing sicker patients. It is estimated there are 8.5 million people in the U.S. with "PVD". 62 million seniors in the U.S. (13%). If you analyzed charts of podiatrists, you would probably come up with a statistic well over 50%. So they pay us large sums to "mine" our charts to establish documentation of that diagnosis. This is just one of many examples of theoretically good ideas that go bad once money comes into play.
Richard Rettig, DPM, Philadelphia, PA

I have been using Freshbooks for nearly 5 years and have been very pleased with it. They have an extension for check writing that integrates seamlessly, as well as payroll options. There are a variety of plan levels depending on needs and budget. It is web-based and there is an app for your phone as well, and all updates are done without extra fees.
