Regular Foot Exams for Diabetics Decrease Risk of Amputation: NY Podiatrist
According to Sneha Patel, DPM, "In 2022, the Centers for Disease Control & Prevention reported that approximately 37.3 million Americans are afflicted with diabetes. Of those, nearly 38 million, approximately 15% of them will experience a diabetic foot ulcer in their respective lifetime. That is over 5 million people with high-risk diabetic foot wounds! Even more worrying, 85% of lower extremity amputations occur due to a foot ulcer."

"I find these statistics deeply troubling. Leg amputations have detrimental effects, especially on someone’s heart. This often shortens the lifespan of the person who undergoes a leg amputation. Podiatrists have a special role in helping diabetic patients prevent foot ulcerations and amputations. Having regular evaluations with a podiatrist can greatly decrease your risk of developing a foot ulcer and if you have a foot ulcer, getting regular care will decrease your risk of amputation," says Dr. Patel.

CA Podiatrist Co-Authors Large Study Showing Impact of Insurance on Amputation
A significant study recently published in the Annals of Vascular Surgery has uncovered a troubling correlation between loss of health insurance coverage and increased risk of amputation. Led by Dr. Tze Woei Tan, a vascular surgeon and associate professor, the research team from the University of Arizona and Keck School of Medicine of USC, which includes co-senior author Dr. David G. Armstrong, a podiatric surgeon and professor of surgery, brings attention to this important issue. Titled "The Impact of Health Insurance Loss on Amputation Rates in the United States," the study highlights the consequences of losing insurance coverage.
Researchers examined a large cohort of patients at risk of peripheral artery disease (PAD) and diabetic foot complications, noting that those without insurance were more likely to experience amputation. The study found that individuals who lost their insurance coverage were 2.5 times more likely to undergo a major amputation compared to those with continuous coverage. This striking difference emphasizes the importance of consistent access to healthcare and the potential consequences of gaps in insurance.

Query: Type of Padding for Diabetic Toe Ulcers?
I have a 74 year old male diabetic patient with recurring toe ulcers at the tip of the left 3rd hammertoe and medially on the left 4th toe. I use a toe crest made of cotton roll to lift the 3rd toe. This helps, but it bunches within few days. The patient needs pad(s) that lift the 3rd toe and separate it from the 4th toe. The patient perspires, but can’t use antiperspirant due to dry skin, and uses powder instead, which usually alleviates the interspace maceration. Any suggestions?

Azole derivatives such as oral ketoconazole and oral itraconazole are excellent choices in treating this saprophytic fungal infection. I have encountered cases of Penicillium niger toenail infection 1-10 in practice, which were treated and responded well to oral itraconazole.

According to my father, a PhD chemist, your jar of phenol is 89%. If it were 100%, it would be a solid. Keep it in a dark place in a brown bottle and it will be fine. If it crystallizes, put it in warm water until it liquefies.

1- Have a pharmacist make up a 2-4oz bottle every 2 months, depending on your practice use.
2- Give the old phenol back to the pharmacy to dispose of.
3- Keep at room temp, keep it capped as much as possible. This will ensure no crystallization.
This procedure will almost guarantee your results will be consistent. Bottom line: Nails and no phenol burns pay the rent.

The best advice I can give is to lose the phenol and switch to sodium hydroxide.
The nail wound heals faster, with less drainage and almost no pain, and in my opinion, with less damage to surrounding tissue. I haven't used phenol since I left podiatry school in 1979.

I am shocked and disappointed when I read of yet another DPM who billed for procedures that require a local anesthesia without demonstrating it. All DPMs should KNOW that the authorities constantly research and analyze billings that “fall” out of the mainstream. Such computer billing evaluations are very, very easy to execute. It is absurd for any DPM to consider that they will go unnoticed.
Yet, I read of yet another DPM who was yet again convicted. A number of years ago, Podiatry Management was kind enough to publish an article I wrote on how to defend yourself using cell phones/inexpensive digital cameras to “create records" to defend yourself. This catastrophe was unnecessary. It is terribly sad.

1) In response to Dr. Eddie Davis, who wrote, "My lease stipulated that I could not have an x-ray unit" and as a guide to our many doctors who might be negotiating office space leases, that clause was likely a boiler plate clause put in your lease and anyone else in the building to protect the radiologic group from competition at the same property. When I signed my first office lease in 1980s, there was, and still is, a radiology group on the ground floor of my medical building. There was a similar clause in the 22-page lease offered to me that excluded me from taking x-rays.
We had a meeting and the property owners agreed, because it was a restraint of trade and a limitation of my scope of practice under my podiatry license in the state of California. The clause was justly omitted, and instead we added a clause that...
