OH Podiatrist Warns Against Self-Treatment of Ingrown Toenails
“Almost everyone will suffer from an ingrown toenail during their life, whether it be the result of an injury, poor fitting shoes, or genetics,” explains Ohio-based podiatrist Amanda Meszaros, DPM. Many people will attempt to resolve the issue with “bathroom surgery,” i.e. an at-home, self-directed procedure, often performed with non-sterile, ineffective instruments.

A common result: a splinter that will fester, cause an infection, or trigger a fleshy growth called pyogenic granuloma that results from minor trauma. What to do instead: “Seek professional help early,” advises Dr. Meszaros, noting that in-office treatment is easy, simple, and offers quick relief of symptoms.

| PRACTICE MANAGEMENT TIP OF THE DAY | |
7 Essential Elements for Your Practice Website - Part 1
1. Services: When describing your medical services, it is best to understand the difference between benefits and features. Features represent the factual descriptions of the service, while benefits describe the things a patient would gain upon purchasing the service. Let's take the example of a car. A feature of a car would be anti-lock brakes or power steering, whereas the benefit is that you are keeping your family safe on the road. If you are a bariatric surgeon, offering lap band surgery to reduce a patient's BMI would be considered a feature. The benefit to the patient would be to transform their life both physically and emotionally.

| CODINGLINE CORNER | |
Query: In-Hospital Patient Consultations
I am unclear as to what code to use for an inpatient hospital initial consultation. I have been denied by my Medicare Part B contractor on CPT 99222 (initial hospital care, per day, for the evaluation and management of a patient, which requires these 3 key components: a comprehensive history; a comprehensive examination; and medical decision-making of moderate complexity), stating only an admitting physician can use that code. So, then I tried CPT 99252 (initial inpatient consultation for a new or established patient), and it was denied stating that code was invalid.
Vanessa Sloan, Effingham, IL
Response: Medicare eliminated “consultation” codes so, yes, those are invalid. You are allowed to use the initial hospital care codes, if, as you say, you meet the code’s requirements. These codes for Medicare are not exclusive to the admitting doctor. Your Medicare contractor denied you incorrectly.
You may want to call and ask for a supervisor. If you get the same information, I would then notify your state association and bring this to their attention. Get the reference number of the call you made as that can be used to track down what happened.

RE: High Cost of Drugs (George Jacobson, DPM)
From: Elliot Udell,DPM
Let’s open another can of worms with regard to the cost of drugs. In New York State, doctors are required by law to e-prescribe. My allergist e-prescribed some medication for me. I paid for it at the local pharmacy, only to find out that I could have saved a significant amount of money had I gone to another drug store. The way e-prescribing is set up with insurance companies is that the pharmacy will not give you the final price of the medication until they process the prescription. In order to price compare, I would have had to ask my doctor to send off the prescription to other pharmacies, which he cannot do using his program. This set-up is another example of how our government has set up prescription benefits in a way that does not help the consumer.
Elliot Udell, DPM, Hicksville, NY
