Calcaneal Apophysitis is Typically a Self-limiting Condition: MA Podiatrist
 
Footwear and foot orthoses for calcaneal apophysitis offer no relative treatment advantages at 12 months, although they do at two months, according to new research. Dr. Alicia M. James from Peninsula Health Service and Monash University, Frankston, Victoria, Australia, and colleagues conducted a factorial 2x2 randomized comparative effectiveness trial in 124 children aged 8-14 years with clinically diagnosed calcaneal apophysitis.

"Calcaneal apophysitis is typically a self-limiting condition and usually improves over time," Dr. John M. Giurini, of Beth Israel Deaconess Medical Center in Boston, told Reuters Health by email. Dr. Giurini, who was not involved in the study, said that managing the acute problem and reducing inflammation may be adequate to return patients to full and normal activity.

SC Podiatrist Running for State House Seat 
 
South Carolina House District 95 Rep. Jerry Govan and challenger Dr. Kevin Ray are both are running as Democrats. Ray, an Orangeburg podiatrist, says elected officials’ time has expired. “We have to make certain that we’re able to keep our flame burning. Otherwise, we’re going to continue to be taken for granted in Columbia. If we’re not being heard, we’re not being helped,” Ray said.

Ray stressed the importance of making bipartisan relationships in both the House and Senate. “We must start a collaboration between all levels of government here in Orangeburg County so that we can better Orangeburg. This seat is not about me or Rep. Govan. It’s about the people of Orangeburg,” he said. Ray says he wants to make sure that the people of Orangeburg get exactly what they deserve, and get their fair share in Columbia.

CA Podiatrist Writes a Book About Legendary Basketball Coach

Michael Levi, DPM has written a book titled Wednesdays With Wooden. It's about his relationship with the legendary UCLA Basketball coach John Wooden. As Wooden's podiatrist in his later years, Dr. Levi made house calls to Wooden's home in Encino, California.

Wednesdays With Wooden

A special bond developed, similar to one Levi had with his own late father. The book is a tribute to both. There are new insights and anecdotes in this book, plus it brings back memories of the glory days of UCLA basketball. A portion of the proceeds from the sale of the book will go to the Hirschberg Foundation For Pancreatic Cancer Research.

Lack of Autonomy and Respect Threatens "Doctorness," Physicians Say
 
Doctors say that “doctorness”—the traditional way that physicians practice medicine—is threatened, a new study reported. These threats include the increasing complexity of the health care landscape, combined with today’s technology-enabled consumer, according to the “Truth About Doctors” study conducted by marketing services firm McCann, in New York, NY.
 
The study found that the pressures of today’s world have not only stolen time and autonomy from doctors, but have simultaneously demanded they do more on someone else’s agenda. “The autonomous, entrepreneurial role the doctor has played in the past has changed dramatically. In the last five years, doctors have gone from being the lynchpin in the healthcare system to a devalued cog in a larger wheel,” said co-author of the study Hilary Gentile, Chief Strategy Officer, McCann Health North America.

"While the law allows for an extension of time to file, it does not provide for an extension of time to pay. You will be hit with significant penalties and interest if you pay your taxes late. To avoid these penalties and interest, self-employed physicians should make an estimated tax payment each quarter. If you are employed, you should double check that enough pay is being withheld. Your CPA can estimate your tax bill so you can cover any deficiency when you file your extension, even if you do not have everything needed to file the return itself."

RE: MIS Can Reduce the Need for Bone Surgery in Diabetics

There is a tremendous cost in treating diabetic ulcers which can involve expensive wound care centers, hyperbaric treatments, hospital stays, IV antibiotics, and sometimes amputations. It is not uncommon for these cases to cost over $100,000, which does not count the loss of productivity or expensive aftercare after amputations.
 
J. Monroe Laborde, MD has developed minimally-invasive tendon balancing techniques to reduce the need for bone surgery when treating diabetic ulcers. He has been able to prevent countless amputations using his techniques, which also minimizes the need for bone procedures. A good reference for this is his article. Some other interesting bone and tendon procedures for treating osteomyelitis without amputation can be found on Don Peacock's site with youtube videos. Dr. Laborde will also be on this week's Meet the Masters. Both J. Monroe LaBorde and Don Peacock will be lecturing at the June 16-18 AAFAS meeting in New Orleans and demonstrating these techniques at the LSU Medical School cadaver lab during this New Orleans meeting.

RESPONSES/COMMENTS (NON-CLINICAL) - PART 2
RE: Podiatric Surgery and Value-Based Reimbursements

Much of the present podiatry service mix is elective surgery. Many podiatrists are financially dependent on its reimbursement. In the new Medicare value-based payment system that is replacing most fee-for-service reimbursement, elective surgery has low priority and a low payment outlook. Already, new payment criteria being developed by narrow network insurance payers are considering bunions and hammertoes cosmetic problems – less expensively treated by prescribing a larger shoe rather than incurring the expenses of a surgeon, facility, and post-operative disability. It seems pretty clear that, unless a podiatrist has an employed position as a surgeon, not being dependent upon elective surgery income is a good thing for the future.
