SMUCPM Honors 2 CA Podiatrists
Samuel Merritt University College of Podiatric Medicine (SMUCPM) honored two distinguished California podiatric doctors with prestigious Health Impact Awards at a gala celebration in Oakland on April 3, 2025. David Armstrong, DPM ’93, Distinguished Professor of Surgery and Neurological Surgery at the University of Southern California, received SMU’s Distinguished Alumni Health Impact Award.

APMA, Members Advocate for Medicare Payment Fix During Legislative Conference
Last month, APMA hosted a successful Legislative Conference and Capitol Hill Day with nearly 100 podiatrists participating and advocating for the profession. Before visiting Capitol Hill, the members dedicated a day to exploring the policy issues impacting podiatrists, such as Medicare reimbursement, Medicaid, and student debt. They also learned how to be effective advocates and craft memorable stories. Congressional staff were present and shared valuable insights and best practices to help members maximize the effectiveness of their visits.

Members then went to Capitol Hill and asked Congress to reverse the 2.83-percent cut to the Medicare Physician Fee Schedule and pass HR 879, a bill that would stop these harmful cuts and provide an additional increase to help offset inflation. Members stressed physicians continue to face year-after-year cuts, while other Medicare payment systems, including Medicare Advantage plans, receive increases. Short-term patches are not enough; members also advocated for a permanent solution. Members shared their frustration and disappointment that Congress did not cancel these Medicare cuts in its full-year funding bill passed in early March. However, Congressional leadership has since committed to addressing these cuts as a part of the forthcoming budget reconciliation process. I also had a case almost identical to this one in question. I originally saw the patient when she was 5 years old, complaining of multiple painful "shiny things" on multiple toes. She had the reddish brown papules in the symptomatic areas. Nothing was found clinically or on x-ray. She presented again as a young adult many years later with the same complaint. She stated that she found black filaments in the areas when she looked for them. Once again, I could find nothing on examination. I had a difficult time doing a neuro exam, since she complained of severe pain with manual muscle testing and with percussion of the superficial nerves looking for a Tinel's sign.
Consultations and workups with a neurologist and rheumatologist yielded no results. Her PCP placed her on gabapentin. About a year later, she developed severe CRPS secondary to an ankle sprain, despite my immediately sending her to physical medicine and aggressive physical therapy. Another year or two after that, she developed severe CRPS of her upper extremity, after a bruising injury to her hand. She had been treated by a psychiatrist for many of those years also, but, unfortunately, none of the treatments by any of the other physicians seemed to have any effect. She passed away at a relatively young age due to heart failure. Neither myself nor any of the other doctors were able to make a definitive diagnosis.

Only took an extra 10 minutes to research, educate and enlighten myself using Dr. Google. Very impressive! Had Latin and French in HS, no help. German may have. Recently retired after 40 phenomenal, fabulous, and VERY satisfying years helping people with their feet and listening to their problems. I could not have wished for a better life or profession.
I was considering discontinuing PM News, but the variety and quality of the letters will keep me reading. Much better than politics! Thanks, fellows. Keep smiling and enjoy something every day.

I was extraordinarily stupid when I purchased my practice. The seller convinced me that a professional assessment was unnecessary and rather than see this as a red flag, I believed him. His representation was that the office equipment was relatively new and everything was above board. Once purchased, I did my own deep dig and found the following:
· All of the office equipment was obsolete, including the copier in the front which, when opened, it died a few weeks after I bought the practice and had over 1 million copies on it.
· A locked door, for which no key appeared to be available, was opened by a locksmith. On the shelves were...

The prophylaxis for DVT, and resultant possible PE, has been a controversial subject for years. The postings here reflect that. The discussion has had statements that it is not our problem as we are podiatrists. Another stated that a doctor has had malpractice claims for prescribing blood thinners and for not.
I would like to share a past experience. I was attending an Ortho meeting years ago. I was interested in a lecture reporting the findings of a multi-center analysis of DVT and PE incidence following lower extremity surgery. Their findings suggested that prophylaxis is not recommended as the vast majority of DVT cases were below the popliteal vein and as such these were less likely to result in PE. An attendee came to the microphone and asked for a show of hands of how many of those attending the lecture had ever lost a patient from PE. The number of hands that went up was a sobering...
