NY Podiatrist Discusses Self-Care of Bunions
 
You’ve likely seen an array of over-the-counter bunion products. “Bunion correctors and splints may reduce shoe irritation, but these products are unlikely to reduce the actual deformity,” explains Michael Trepal, DPM. “There’s no good research these will routinely correct a bunion. You can get some pain relief with a variety of over-the-counter pain reducing creams or perhaps even one with cannabis compounds,” says Dr. Trepal. Topical cannabis oil or creams may have anti-inflammatory and analgesic abilities, he adds.

“Custom-made orthotics, following a comprehensive biomechanical evaluation by a podiatrist, is generally the best route,” says Dr. Trepal. But if you’re not ready to splurge on a custom pair of orthotics, OTC varieties can still help. If self-care doesn’t provide enough relief, or you want to get rid of a bunion for cosmetic reasons, surgery is a possibility. And while surgery is the only way to completely get rid of a bunion, it’s not foolproof. “A bunion can come back, even after it’s removed,” warns Dr. Trepal.

MultiPlan: Health Insurers’ Lucrative, Little-Known Alliance
 
Large health insurers are working with a little-known data company to boost their profits, often at the expense of patients and doctors, a New York Times investigation found. A private-equity-backed firm called MultiPlan has helped drive down payments to medical providers and drive up patients’ bills, while earning billions of dollars in fees for itself and insurers.
 
The most common way Americans get health coverage is through an employer that pays for workers’ medical care itself and uses an insurance company to administer the plan. Providers in the plan’s network have agreed-upon rates, but out-of-network providers often must negotiate payments. By using MultiPlan’s frugal recommendations, insurers say they are saving employers money. But insurers and MultiPlan also benefit because their fees are typically based on the size of the declared “savings” or “discount” — the difference between the original bill and the amount actually paid. In some instances, insurers and MultiPlan have collected more for processing a claim than the provider received for treating the patient.

Experts provide nine tips to help practices successfully collect patient balances.
 
2. Decide whether the practice will require upfront collections.
 
Many practices became lax during COVID-19 because they simply wanted to maintain or rebuild patient volumes. Now it’s time to revisit their strategy, says Reed Tinsley, CPA, healthcare consultant in Houston, Texas. “A medical practice is a business,” says Tinsley. “You need to have cash coming in to pay expenses.”
 
One challenge with upfront collections is that the clinical circumstances of the visit are frequently unknown before the encounter, says Denny Flint, chief commercial officer at Millennia, a tech-enabled patient engagement company specializing in the recovery of end-to-end patient financial obligation. This makes it difficult to know exactly how much patients will owe, he adds.

RE: Medicare Advantage in the Headlines

A recent Becker's Payer Issues has many stories on Medicare Advantage. One such story states the following:
 
"The Medicare Payment and Advisory Commission called for a "major overhaul" of Medicare Advantage policies in its annual report to Congress. The commission estimated that in 2024, the government will spend $83 billion dollars more on Medicare Advantage beneficiaries than if they were enrolled in fee-for-service Medicare." In addition, Becker's cited 7 other problematic issues with these replacement plans.
 
As both a taxpayer and provider, I have no words to reflect my exact outrage! These companies have bilked the system, given their CEO 8 figure annual salaries, and paid providers often less than 50% of what Traditional Fee for Service Medicare would pay...all at increased costs to the government. It's time to shut this system down! It's now not only non-advantageous to providers and patients alike, it's also disadvantageous to the government and the taxpayers!

We as podiatrists can moan and complain about the guidelines Medicare has adopted for procedure code 11730 but the bottom line is those guidelines are not going to be changed. The procedure that podiatrists must perform to substantiate billing a 11730 code to Medicare truly sets our profession apart in skill level, training, and expertise from other healthcare workers, and more importantly nail technicians working in salons and non-medical environments.  
 
One cannot argue that simply cutting back a small slant section of the nail border as compared to infiltration of anesthesia followed by removing a section of the nail border to the level of the nail matrix is the same procedure. There is no doubt that there are circumstances that performing the procedure required to bill a 11730 is considered overkill. If that is the case, how could one justify the fee generated from that procedure code and doing a much less complex procedure? 
 
Proper surgical technique by the physician and post-surgical care by the patient should not significantly increase the risk of infection or other complications. For those who present the argument of putting your diabetic and other “at risk” patients in danger if you adhere to Medicare guidelines of performing a 11730, you should select a different procedure or explain to your patients that the treatment needed to remedy their problem is not covered by Medicare.

One metric that has not been critically evaluated is $/minute/sq ft or $/sq ft/minute, both gross and net. This can help the provider see the cost of bringing in new equipment or an associate, particularly if there is empty office space.
