PODIATRISTS IN THE COMMUNITY - PART 1
IL Podiatrist is Guest Speaker at Carmi Kiwanis

David Gunzel, DPM opened the program at Carmi Kiwanis, April 4, 2019, talking about his time as a former Kiwanian, actually helping to establish a club in the past. He is returning to southeastern Illinois after a several year absence. He practiced in Harrisburg from 1993 to 2008 with his cousin, Dr. Roger Herrin. Following the Harrisburg flood of 2008, he joined a practice in Carbondale and remained there until 2018. He has now returned to take a position with Ferrell Hospital, based in Eldorado.

Dr. David Gunzel

Dr. Gunzel said that podiatry has changed in southern Illinois over the last 25 years. When he began his practice, he was one of only a handful of podiatrists in all of southern Illinois. Today there are more than a dozen podiatrists.

Out-Of-Control Bus Nearly Destroys NY Podiatrist's Office

An out-of-control MTA bus crashed into a Queens bagel shop in a chain-reaction crash that injured a driver and left a bus-sized hole in the shop’s entrance. The front of the shop was pulverized, and there was damage to a Little Caesars next door.

Fortunately, the office of podiatrist Neil G. Blatt, DPM in the same building was not damaged and was open for business on Monday.

PODIATRISTS AND FOOTWEAR
Fashion Craze "Floss Heels" are Bad for Your Feet: CA Podiatrist

There are many spring shoe trends, including the floss heel craze of 2019. According to The High Heel Doc, Bobby Pourziaee, DPM, of Rodeo Drive Podiatry, there's actually quite a bit to not love. He says that spring's biggest shoe trend—floss heels—is, in fact, bad for your feet.

Dr. Bobby Pourziaee

"Even though floss heels show off your feet for the world to admire, they don’t provide the best support. There is a general lack of support around the foot and ankle, which can easily lead to twisting of the ankle while walking. The lack of support can also lead to overuse injuries like tendonitis," says Dr. Pourziaee.

RELEVANT RESEARCH
Vitamin D Deficiency Increases Risk for DFUs

Adults with diabetes and severe vitamin D deficiency are three times more likely to develop a diabetic foot ulcer (DFU than similar patients with sufficient vitamin D levels, according to findings from a meta-analysis published in Nutrition & Diabetes. “Vitamin D has been suggested to play an important role in many chronic diseases, such as diabetes,” Yimin Chai, MD, PhD, professor in the department of orthopedic surgery at Shanghai Jiao Tong University, China, and colleagues wrote. “Low serum vitamin D levels are associated with insulin resistance, impaired beta-cell function and the development of [diabetes]. There is also ongoing interest in the association between lower level of vitamin D and diabetic complications.”

Chai and colleagues analyzed data from five retrospective cohort studies and two prospective cohort studies analyzing the relationship between serum vitamin D level and diabetic foot ulcers in 1,115 patients with a mean age ranging from 50 to 70 years. In four studies that reported on severe vitamin D deficiency, defined as a 25-hydroxyvitamin D level of less than 10 ng/mL, researchers observed severe deficiency in 216 patients with diabetic foot ulcers and in 108 patients with diabetes but without foot ulcers (48.98% vs. 22.78%), for an OR of 3.22 (95% CI, 2.42-4.28).

PRACTICE MANAGEMENT TIP OF THE DAY
How to Protect your Reputation Against Defamatory Online Reviews - Part 1

Doctors, dentists, psychologists, clinical laboratories, and other medical providers have a right to defend their reputations and livelihoods against reviewers who spread false and damaging claims. Fortunately, there are a variety of legal options available to combat defamatory online reviews.

1. Know Your Rights - Defamation laws vary from state to state but, generally, a person may be liable for defamation when he or she posts a review that contains demonstrably false assertions of fact (as opposed to opinion), that are damaging to the subject of the review. This means that patients are entitled to post reviews that are statements of opinion, such as “I didn’t care for the doctor’s bedside manner” or “The office was poorly decorated.” While a provider may disagree with a patient’s opinion, those disagreements are not proper bases for legal action because opinion is generally protected by the First Amendment. However, if a review contains a false assertion of fact, the reviewer may be liable for defamation. This may include statements such as “I was billed for procedures that were not performed” or “The doctor botched the procedure.”

QUERIES (NON-CLINICAL)
Query: Quality Metrics

I am part of a medium size multi-specialty group practice. As part of our ongoing commitment to quality improvement, all specialties are tasked with identifying, measuring, and meeting quality metrics. Examples include percentage of patients with BP below 140/90 or glycohemoglobin below 7 for primary care, or cholesterol screening for pediatric patients, etc. I am requesting suggestions for quality metrics for podiatry from my colleagues.

RESPONSES/COMMENTS (MEDICAL-LEGAL)
From: Linda McSmith

Providers may or may not charge for copies and supplies necessary to copy, depending upon the stipulations in the contract with payors. PICA recommends that you check your contract with the payor to determine if you can charge for charts being requested for audits.

RESPONSES/COMMENTS (NON-CLINICAL) - PART 1A
From: Alan Sherman, DPM

Former podiatry school and Nova Southeastern osteopathic school Dean Leonard Levy is one of my heroes in podiatry and our finest futurist in podiatry. Barry Block, Bryan Markinson, Harvey Lemont, Tracey Vlahovic, Brad Bakotic, myself, Leonard Levy, and others have all grown beyond clinical podiatry to establish consultancies or full-fledged businesses outside of clinical podiatry. I’m all for our podiatry students and residents who feel up to it to take the USMLE. I bet a subset of our residents can pass this test if we add Ob/Gyn, Peds, and Psych to our curriculum.  Any prediction on what percentage will pass?

RESPONSES/COMMENTS (NON-CLINICAL) - PART 1B
From:  Paul Kesselman, DPM

Over the last 35 years, I have seen many DPMs who left the profession and did quite well with the DPM degree as part of their resume. Several have gone into teaching at the HS or university level and have excelled, receiving numerous awards, grants for research, etc.

Others may have started in the "cellar" of pharmaceutical sales and go onto become national directors, national physician education liaisons, far surpassing the average incomes of most MD/DO/DPMs. These gifted individuals no doubt would have excelled at anything they chose to do, but the fact was their DPM degree and educational background did get them in that first door which...

RESPONSES/COMMENTS (NON-CLINICAL) - PART 1C
From: Simon Young, DPM

Bottom line... we have a limited license. MD/DO/NP/PA all seem to be able to treat the whole body (give or take). We can't even give a flu vaccine in NYS. My local pharmacist can. Others can be hired in hospitals and by insurance companies. Unless there is a special niche, or someone wants specialized opinions, we don't fit the majority of needs. Unless we are able to increase our scope of medical (not surgical)  practice, we are stuck.
