DMU-CPMS Students Exceed National Average on Licensing Exam
Two classes from Des Moines University College of Podiatric Medicine and Surgery have exceeded national averages on the American Podiatric Medical Licensing Examination. Compared to the national passing average of 83%, 95% of DMU's CPMS students pass the exam. The APMLE consists of four parts to assess whether a candidate has the knowledge required to practice as a minimally-competent entry-level podiatric physician and determine if the candidate's knowledge and clinical skills are adequate for safe, unsupervised practice.

The CPMS Class of 2025 had a pass rate of 96% on Part II of the exam when the national average was 83%. Part II is a written exam that looks at the candidate's knowledge in the clinical areas of medicine, radiology, orthopedics, biomechanics, and sports medicine, anesthesia, and surgery, community health, jurisprudence, and research. Toenails Often Reflect Systemic Health Issues: Aussie Podiatrist
Sydney podiatrist Kate Edwards said healthy-looking feet should have pink, clear nails with white tips, typically a half moon shape at the base. If you're unsure of what to look for, she advises paying closer attention to the appearance, skin, texture and color of your feet, as these can often reveal an underlying condition before any other part of your body. "Nails and skin can show signs of common inflammatory skin conditions such as dryness, redness, and skin flaking, which may indicate possible underlying issues like tinea, eczema, psoriasis, or the overuse of medications," Kate told FEMAIL.
|
|
Kate Edwards |
Simple foot signs such as redness, swelling, and pain are easy to dismiss but can indicate that your "body is trying to fight against an infection. Dark colored spots can be possible melanomas. These can appear in the skin - top and bottom of feet and nails. Please speak to a medical professional or better yet get a skin check as soon as possible.

| PRACTICE MANAGEMENT TIP OF THE DAY | |
13 Tips for Firing Practice Employees
Firing an employee is rarely pleasant, but it can be especially sensitive in a medical practice, where patient care and privacy are top priorities. When handled properly, a termination can protect the practice’s reputation and maintain a professional environment. Here are 13 tips for managing the process:
6. Choose the Right Time and Place
Timing can influence how the termination is perceived. Typically, it’s best to conduct such discussions on a day and at a time that minimizes disruptions to the practice and respects the employee’s dignity. A private, quiet space helps maintain confidentiality.

Query: Injection For Sesamoiditis
While reviewing coding descriptions, the question of an injection for sesamoiditis was stated as extraarticular, defined as a procedure where medication is injected into the tissues surrounding a joint, rather than directly into the joint. There is no indication the injection was into a tendon, ligament, or aponeurosis as described in CPT 20550 or 20551. Would then CPT 96372 for subcutaneous or intramuscular be more appropriate?

| RESPONSES/COMMENTS (CLINICAL) | |
From: Bruce Kaczander, DPM,
It is highly unlikely that it’s a bilateral symmetric foreign body and highly unlikely that a 1 mm macule would cause pain like that. If she is having symptoms at bedtime (HS), consider something neurogenic. Tarsal tunnel will cause HS symptoms due to engorgement of venae comitantes. Do a Tinel’s test.
Radiculopathies can cause HS pain, even in the absence of current back pain. Is there a history of spinal issues? Bottom line, start her on gabapentin or Lyrica and titrate the dose as needed.

RE: April 13 and Proposed Cellular Tissue Product Policy
From: Paul Kesselman, DPM
In just a few days, all 7 Medicare contractors are set to put their proposed policy into effect which will, in my opinion, cause more harm than good. The current policy is antiquated and based on decades old technologies. No doubt a new responsible policy is needed reflective of 2025.
I urge all readers who provide cellular tissue products to reach out to their Congressional delegates and let them know the potential harm and increased costs this proposed policy may cause to both patients and taxpayers, regarding sepsis, amputations, etc. If you are a member of any wound care association, they all have templated letters, and the email addresses of your Congressional delegations who may be able to influence CMS to place a pause on this soon to be enforced irresponsible LCD.
Removing or changing an effective LCD is far more difficult than placing a pause on a proposed final LCD. The time to act is now.

RE: MA Advantage Plans Get Increase
From: Farshid Nejad, DPM
Our leaders are failing us. How do MA plans get a 5% bump and all private practitioners are getting a 4% cut. This is ludicrous. Even more insane is providers accepting the terms of MIPS. If CMS wants information, they should bonus those who participate. Medical companies are being purchased for billions not just in the private equity arena but also by conglomerates like Amazon. We are not getting paid to participate but rather penalized by 9% if we fail to participate. I challenge newly elected Dr. Brooke Bisbee and her board to make this a priority.
These complaints about the egregious acts of CMS have been posted many times with calls to unionize. Quite frankly, we just need to grow a couple and tell lawmakers that enough is enough. This is unsustainable. We need to work with AMA and all the other associations including DOs, physical therapists, etc. to stop the wasteful spending into MA programs and re-invest in us. APMA, don’t brag about reducing Medicare cuts; we want the next media release to be about a positive pay increase equal to or greater to the inflation rate. The floor is yours APMA…show us why we pay our dues!

RE: Podiatry Salaries vs. Nurse Salaries from Becker's Hospital Review
From: Steven D Epstein, DPM
Certified registered nurse anesthetists (CRNAs) make more than most of us do. An RN makes this after only four years of undergraduate education. And this doesn't preclude them from going into advanced practice; in fact, experience as a "mere" RN is usually required to be accepted for advanced practice training. Failing to be accepted for that consigns one to a miserable six figure salary career.

Nurse's Salaries (Source: Becker's Hospital Review) |
This kind of info is now freely available for high school and college students to see. Is it any wonder that smaller numbers of students are choosing podiatry as a career?

| RESPONSES/COMMENTS (NON-CLINICAL) - PART 2 | |
From: David Secord, DPM
Once again, Dr. Tomczak helps focus upon the pivotal point in the progression of our fate. That he does so while being able to throw in a little modus tollens Aristolean syllogistic logic is that much more entertaining. It had been a while since I’d seen the term portmanteau, so kudos!
His description of our struggles to define ourselves before we can define what we do and who we are as a profession to the lay public as a synecdoche or metonymy rings true. Dr. Tomczak does a wonderful job in his musings as a erziehungsroman of sorts. We are truly, as a profession, writing a bildungsroman as we go. Hopefully, the story will end as a victory and not a tragedy. Change is inevitable. Our indecision to initiate and continue the journey is not encouraging.
David Secord, DPM, McAllen, TX
