SC Podiatrist Discusses Bunion Surgery
 
"Bunions form when the normal balance of forces that is exerted on the joints and tendons of the foot becomes disrupted. This disruption can lead to instability in the joint and cause the deformity. Bunions are brought about by years of abnormal motion and pressure over the MTP joint," says Lori DeBlasi, DPM. a board-certified podiatrist at MUSC Health-Podiatry at the MUSC Health-Florence Medical Center.

According to Dr. DiBlasi, "A simple bunionectomy, in which only the bony prominence is removed, might be used for the less severe deformity. Severe bunions might require a more involved procedure, which includes cutting the bone and re-aligning the joint. Recuperation takes time, and swelling and some discomfort are common for several weeks following surgery. Pain, however, is easily managed with medications prescribed by your podiatrist."

MN Podiatrist Provides Tips for Injured Runners
 
The temperatures are warming up and local runners are ready to hit the trails. Don’t let common foot and ankle injuries slow you down. Spencer Mortensen, DPM, a podiatrist with Baptist Health says "a lot of injuries I see are from overuse. These include tendonitis, but we also see stress fractures and ligament injuries. A majority of the time these can be treated conservatively, but if this is not helpful, MRIs or x-rays are necessary."

"If you are an injured runner, there are some steps you can take including 1) take a break while running,  2) take time off from running, 3) Make sure your shoes are in good shape, and 4) don't push through the pain. This could aggravate or lead to a more severe injury," says Dr. Mortensen.

NM Podiatrist Caters to Elderly with In-Home Visits
 
As he approaches his 90th birthday, Roger Bergstrom said the simple act of bending over to trim his toenails is becoming a challenge. Thankfully, he said, Adam Cornejo, DPM -- known as "the Toe Doc" -- is there to help. "He takes care of it -- the feet in general," said Bergstrom, who, like many elderly people, has a mild foot fungus that causes yellowing in his toes. Such work is not for the squeamish. But the services are needed, Cornejo said, because many elderly people neglect foot health at a time in their lives when they're more susceptible to toe fungus, dry skin, and ulcers. His mission, he said, is to offer affordable, in-home foot care for older people and other patients who are largely confined to their homes.

Dr. Cornejo graduated from Western University of Health Sciences in California with a doctorate in podiatric medicine and then completed a year-long clinical residency at a Veterans Affairs hospital in New Mexico. After realizing he didn't want to become a surgical podiatrist and making a decision to pursue a private practice doing in-home preventive work, he said he initially "had a fear I wouldn't be accepted" by colleagues and other professionals in the area. However, since he made the leap and launched his solo-run practice in July, "people have been really supportive," he said.

Miskin is a member of the American Podiatric Medical Association and the American College of Foot and Ankle Surgeons.

ON THE INTERNATIONAL LECTURE CIRCUIT
U.S. Podiatrists Lecture in Spain
 
Russell Volpe, DPM (New York), Thomas McPoil, (Colorado), Kevin Kirby, DPM (California) ,and Madelin Ramil, MD (Florida) lectured together at the 3rd Seminario Internacional de Patologia del Pie in Zaragoza, Spain, on April 5th and 6th. Also included within the lecturers were numerous Spanish PhDs, podiatrists, and orthopedic surgeons.

Lecture and workshop topics included evaluation and treatment of pediatric flatfoot deformity, tissue stress theory, minimal incision foot surgery, plantar plate tear biomechanics and treatment, transverse plane foot and leg deformities in children, running biomechanics, running shoes, and injuries. Over 400 podiatrists and orthopedic surgeons from Spain, Portugal, Russia, UK, Canary Islands, and other countries attended this two-day meeting which is one of the largest podiatric medical conferences in all of Spain.

Partnership Agreements: Why it May be Time for an Overhaul - Part 9
 
When was the last time you reviewed your partnership agreement? Even if you are not currently contemplating new shareholders or partners, if you haven’t looked at your agreement in the last three or more years, now may be the time. From the perspective of business operations, let’s break out some of the key questions that you should consider in every partnership agreement:
 
9. Withdrawal from partnership: Does the agreement designate how far in advance a partner wishing to leave the group must notify the other members? For example, retirement notice should given be at least 18-36 months ahead of time, and it may be prudent to consider that shorter notification periods could result in forfeiture of part of the buy-out.

Query: E/M During a Post-Op Visit?
 
I recently had a patient return for a second post-operative visit. The patient’s surgery involved the repair of dislocating peroneal tendons. The sutures were removed and we were about to apply a cast when the patient mentioned his calf pain. The visit switched gears as I started to work the patient up for a deep vein thrombosis. We re-splinted the patient and sent him to the vascular lab for a venous duplex Doppler. This ultrasound ruled out the DVT. My partner believes that I should be billing an E/M code for the evaluation of the leg pain.

Response: Your partner is wrong. The only E/M modifier option available during a global period is the “-24” modifier (unrelated evaluation and management service by the same physician during a post-operative period). The pathology for which the E/M is being performed must be “unrelated” to the procedure that was performed. Some interpret this to mean a different diagnosis, like wound dehiscence or post-op infection or DVT, but that is an incorrect interpretation. It must be unrelated. In the scenario you describe, the calf pain is related to the surgery. Therefore, it would be inappropriate to submit an E/M for this encounter.

Dr. Solokoff made a reasonable assessment when he suggested reducing the hours spent on podiatric surgery and biomechanics and including hours in pediatrics, Ob-Gyn, and psychiatry that would enable a podiatrist to sit for the USMLE and COMPLEX examinations. That would enable those podiatrists seeking an MD/DO degree the ability to take those exams and receive those degrees. Then the adverse forces outside podiatry would be mitigated.

As we all know, preparation for podiatric medical practice today includes undergraduate podiatric medical education and, as part of a continuum, 3 years of residency training. Not too many years ago, the DPM completed podiatric medical school and perhaps a year of residency. As a result, education and training took just four years, and perhaps for a relatively small core of graduates, another year of training. Today, much more general medicine and surgery is a standard part of podiatric medical school which is significantly increased in the three years of residency that follows. 
 
But in spite of our greatly expanded education and training and the fact that there is virtually no difference in how we practice when compared to specialists in medicine such as ENTs, ophthalmologists, dermatologists, the recognition we receive is still considerably less. This lack of recognition often...

Several years ago, podiatric students from California took the USMLE examination. In point of fact, the podiatry students performed horribly in the examination. The results of this testing was to test the current status of general medical education in the podiatry schools. I suspect that medical students would fail the podiatry national boards.
 
If the objective of podiatry school is to prepare students to pass the USMLE examination, a restructuring of the current state of education within the podiatric colleges would be required. This would involve more than simply adding coursework in three areas. Clerkships and rotations would also have to change. Students and residents would have to...
