CA Podiatrist Stresses the Health Benefits of Walking

Walk to Work Day (W2WD) celebrated its 14th anniversary on Friday, April 6th. Held the first Friday in April, W2WD was begun in 2004 by the U.S. Department of Health and Human Services as a means of helping increasingly sedentary Americans incorporate walking into their daily routines. “We are in the middle of an obesity epidemic,” said Holly Spohn-Gross, DPM, a podiatric physician and surgeon in private practice in Lake Isabella, CA.

“The good news is that study-after-study indicate that moderate exercise for just 30 minutes a day can reduce not only one’s waistline, but also the risk of heart disease, cancer, stroke, diabetes, stress, and depression. And, walking is just about the best exercise and health bargain around. It's FREE, low-impact, easy to do, environmentally friendly, and research supports the long-standing belief that taking just a few extra steps a day can improve overall health. NY Podiatrist Discusses Bunions and Footwear
 
If you have bunions, you know they can be painful and near impossible to ignore. Wear a shoe that's made from a constricting material or is just a touch too small, and the protruding bumps at the base of your big toes will cry out in pain. "Feet, by design, are not padded with a lot of extra fat," says Howard Friedman, DPM of Suffern Podiatry in New York.

"So, if someone wears a shoe that is creating a pressure point over a bony area, the skin will become red and inflamed, and in some cases cause a blister or wound that can become infected," says Dr. Friedman.

Blues Plans Suffer Setback in Major Antitrust Case
 
A federal district judge handed Blue Cross and Blue Shield insurers a setback Thursday in a major lawsuit over their practice of creating exclusive territories, ruling the deals could directly violate antitrust law. U.S. District Judge R. David Proctor of the Northern District of Alabama found that healthcare providers and consumers showed that some of the practices of the Blue Cross and Blue Shield Association and its 36 member plans should be reviewed as a "per se" violation of federal antitrust law if the case goes to trial. 
 
The judge said the combination of the Blues plans' exclusive territories, in which they agreed not to compete, and the agreement to limit competition on non-Blues branded products, is a per se violation of the Sherman Antitrust Act. The decision is a significant legal blow for the Blues organizations and makes it easier for providers and consumers to prove that the plans impede competition by offering insurance coverage in exclusive markets. BCBSA said it would appeal the judge's decision.

2. Misuse of Modifiers
 
Another common billing mistake is the misuse of modifiers while coding, according to Michael Fossum, CEO of Nobility RCM, a Chandler, AZ-based company helping practices with financials. "If the proper separation modifier is not put in place, or a discriminate procedure modifier is not put in place, the insurance company will recognize that multiple procedures were produced on one claim," says Fossum.
 
Modifiers, and coding in general, can be particularly tricky for small practices lacking the resources to hire a third-party billing company. Hiring someone to perform billing and coding services is something Fossum says is strongly due to the complicated nature of the process. "It is such a time-intensive and knowledge-intensive process; you have to spend a lot of time on the billing and coding aspects of a practice," says Fossum.

QUERIES (CLINICAL)
Query: Chronic Sesamoiditis with Plantar Plate Lesion
 
A 40-year-old active female who used to do a lot of walking and cycling has had chronic sesamoiditis for three years. During that time, she has had ongoing treatment with all sorts of physical therapy for tibial sesamoiditis, including home use of a bone stimulator daily for 3 months -- all to no avail. In the past 3 years, there have been moderate remissions with exacerbations but never has it been pain-free. An MRI showed an intermediate grade plantar plate lesion/tear medial to the 1st proximal phalangeal base.
 
Her rigid pes cavus foot type has been properly addressed with well cushioned orthotics that closely approximate her high arch with forefoot valgus posting, a first met head cut-out and lots of cushioning under the sesamoid with thicker accommodation around it. She wears them daily in good running shoes. She has seen a number of orthopedists in various places, including the Cleveland Clinic, and is now contemplating a sesamoidectomy by an orthopedist who apparently "does a lot of them.Given that there is no sesamoid fracture, would a sesamoidectomy be a good idea?

The South Carolina Podiatry Scope of Practice bill (H.3622) passed through the SC House of Representatives on April 5, 2018. The bill will allow podiatrists in SC to perform rearfoot and ankle surgeries distal to the myotendinous junction. The bill will now be sent to the SC Senate and assigned to the Senate Medical Affairs Committee. We would like to thank all the SCPMA and APMA members who have put their time and effort into this endeavor.

I reviewed the RESA website. They have a homogeneous proprietary plan using software, an algorithm, a technician, and a scanning method developed by a cyclist + engineers to create what they value as a $199 product. Remember the Soles 3-D printed orthotics that DPMs were dispensing that is now out of business having lost $30 million. 
 
I welcome the competition from Costco that will help educate the foot and postural suffering public towards the need for customized orthotic props. My insult comes from Costco stating that the DPM product is worth $300 when mine are...
