Recession Hurting Children's Feet: UK Podiatrist

The UK's children are literally being crippled by recession, as many are being forced to wear second-hand shoes that do not fit their feet properly. That is the key finding from a recent survey by shoe retailer Brantano, which found that almost one in three parents (28 per cent) have started buying fewer pairs of shoes for their children since the start of the economic downturn.

Emma Supple, a podiatrist who advises Brantano, said: 'If children aren't wearing correctly fitted shoes, it not only has short-term effects such as redness, soreness, and blisters, but can also impede foot growth leading to longer-term health problems." Children with ill-fitting shoes may face a future of bunions, foot deformities, posture problems, and even back pain, the expert added.

Aussie Podiatrist Discusses Plantar Fasciitis

Dean McGregor, of Leisure Coast Podiatry in Kiama, says plantar fasciitis seems to be most common in people in the 30- to 40-year-old age group. "It's common with athletes, but it also happens in people who stand a lot, such as hairdressers," McGregor says.  Around July, just before the City to Surf race, is when McGregor says he starts seeing a lot of people struck with the foot pain.

Women seem to suffer from the pain more, he says, adding that walking in thongs can be as bad as high heels, as the action of squeezing the toes can also inflame the area. McGregor says it is rare to resort to cortisone injections or surgery to treat the condition. "Rest, ice the area after you've been on your feet, and do lots of stretches and massage."

The 35th Charing Cross Vascular Symposium again attracted several thousand physicians, surgeons and researchers to Olympia in London this week. The section on Diabetic Foot Care, now termed "ILEGx Collaboration Day" featured key members of the team from around the world included Prof. David G. Armstrong, Director of the University of Arizona's Southern Arizona Limb Salvage Alliance (SALSA).

Doctor-Owned Device Firms Under Federal Scrutiny for Fraud

Physician-owned companies that distribute medical devices at hospitals and ambulatory surgery centers where their investors practice medicine have the potential to violate federal anti-kickback laws, the federal government has cautioned. On March 26, the Dept. of Health and Human Services Office of Inspector General issued a special fraud alert stating that the way these “physician-owned distributorships,” or PODs, do business could pose a fraud and abuse risk as well as jeopardize patient safety.

The concern is that the financial incentives offered to physician owners in these arrangements might encourage these doctors to perform medically unnecessary procedures, or choose to prescribe devices the distributorships sell instead of those that may be more clinically appropriate, the alert stated. According to the OIG’s findings, some PODs may have characteristics that put them at risk for violating the federal anti-kickback statute, which is designed to safeguard patients against any physician referrals that may be influenced by financial incentives.

Supervision and billing for the service of full body hyperbaric oxygen therapy by podiatrists is an interesting question. I practice podiatry in Georgia. I bill for full body HBO therapy when I am treating a lower limb pathology. It would be outside my scope of practice to supervise and treat pathology that is not in the " foot and legs".
 
When I owned my own HBO chambers, I billed and was paid by Medicare for the service, but it was a battle; a battle that every year or two, is thrown back in my face. Luckily, I saved the letters signed by my local Medicare Medical Director acknowledging that I am legally allowed to bill and be paid for supervising HBO when treating lower limb pathology in the State of Georgia.

RE: APWH Holds First Annual Conference

I attended this conference as both a speaker and an attendee, not exactly knowing what to expect. The quality of the speakers was beyond my expectations and the attendees of all medical specialties (more than 150), stayed from early morning until the last speaker's lecture was completed. The interaction of the exhibitors and the attendees was also superb.

I want to personally congratulate both Dr. Kravitz and the APWH board for putting on a conference, which provided me with a significant amount of information that I can use first thing Monday AM. For a young organization in its first year to put on such a conference is astounding. I eagerly await attending next year's symposium in Philadelphia.

With all due respect to my fellow colleague, it sounds like justice has been served. Billing a nail avulsion when performing a nail debridement is wrong whether done once, a hundred, or a million times, even if everyone else is doing it.

I am sure Dr. Kirk is not a bad man, but unless I misunderstand the facts, he made a huge mistake and will pay for it. We should all be mindful as to not approach this slippery slope.

RE: MI Podiatrist Sentenced to Prison for Healthcare Fraud

I offer my condolences to yet another podiatrist caught in the 11730, 11732 minefield. This is the code that puts most podiatrists in jail, beyond phantom patient billing. This all stems from the fact that nursing home/office nail care is not paid by insurers or Medicaid, except under very specific conditions we are all aware of (or should be). In order to perform this legally, it must be a covered RFC service, with all of the attendant clinical factors as part of the service. Or, alternatively, a local anesthetic must be injected. I have written this before, but yet I still read about it. 

If you see nursing home patients, you must search for other codes to use, and often there are OTHERS that are appropriate. But you must actually examine your patients, their history and physical, and their legs and ankles. About 30 to 65% of NH patient care is given free. Only you can decide if that fits your practice model.

Auditors first look for 11730. There are ways to document this service (including photographs of your injection with a chalk board and the patient’s name and date). Unless you are able and willing to use this standard of documentation, my advice is to eschew NHs entirely. The next time your patient asks: “How come Dr. Downtheblock does this for free?”, do yourself and your family a favor and send them there.

In the movie Office Space, there is a classic scene wherein Peter (Rod Livingston) is explaining to Joanna (Jennifer Aniston) that he and his friends are merely diverging fractions of a penny as a rounding off measure to his bank account. "So, you're stealing" says Joanna. "No, we are taking the rounding off of the fractions of a penny," responds Peter. "So you're stealing," responds Joanna again.

Dr. Eckles' letter, in response to the residency shortage concern, was certainly exculpatory for the APMA, CPME, and COTH. Assurances were made by Western that, if approved as a school, residencies were already in place to compensate for the increased demand, for everybody. Nobody is to blame. The system is great. It is a well-oiled machine, just like Initech in the movie.

But we are still short 104 residency positions for our graduates. It happened under the watch of those responsible to assure us that no such problem would occur. Thank you for the explanation. Everybody has done everything correctly. That should certainly be reassuring to any unmatched graduate. No one is at fault. It was a rounding error. But Dr. Eckles, here's the reality: "So....you're still short 104 residencies." Like Jennifer Aniston in the movies, the answer is " yes."

104 podiatrists could learn pedorthics and gain valuable experience in conservative foot care they will use throughout their podiatric career. They can get internships at pedorthic and other healthcare facilities and sit in and fit retail shoe stores, earning income while learning how to use pedorthic products and techniques to help people. Pedorthists help manage the most extreme mechanical foot deformities and disorders, a great training for every podiatrist. Perhaps it’s time to review the curriculum and make room for pedorthics.

Having completed three years of surgical residency almost 30 years ago gives me some perspective on this matter. The more quality surgical training, the better. We ARE the experts of the foot and ankle, and all of us must be prepared to handle any problem with great skill and dispatch. Three years of training is never wasted, BUT the new graduates have to be realistic that if they enter private practice, it could be slow for quite a while.

Emphasis still needs to be given to providing and possessing great office skills, and this is where office rotations, working in a podiatry clinic, and working up ALL patients is still needed. During a residency, the diagnostic skills for surgical work-up or conservative care are very much required. There are many fabulous opportunities in our profession. All providers need to do is open up their eyes and make it happen. If we sit back and complain, nothing will happen. Meanwhile, the powers that be must be held accountable to provide the programs that are necessary for this training. Where there is a will, there is a way; now make it happen.
