PCFD - A New Classification for Fallen Arches: MI Podiatrist
 
According to Conway McLean, DPM, "A new classification system, “progressive collapsing foot deformity” (PCFD) has been developed for flatfoot, one in which the foot gradually collapses. The new classification system has allowed for more precise staging, which serves to better standardize our treatments. Not everyone progresses through all of the stages of PCFD. Unfortunately, we can’t yet determine precisely who will, but when the treatment is non-invasive and easy to utilize, it should be instituted more consistently."

"A strengthening and stretching program is adjunctive to physical methods to prevent the abnormal, pathologic motion. This is best achieved with a custom foot/leg/body support, typically either a prescription foot orthotic or foot-ankle brace. RE: Invisible Metallic Fragments (Morgellons Disease?)

My experience with Morgellons disease was with some heavy recreational drug users. Get an extensive history.

I had a similar case involving a massage therapist who presented with lateral heel pain. He reported stepping on a fishing hook three months prior while fishing but couldn’t recall whether the hook had broken off. Since then, he had been experiencing persistent pain. Interestingly, when he placed a magnet on the heel, it stuck. Initial imaging, including x-ray and ultrasound, revealed nothing abnormal. Two months later, he returned—this time with pain in the opposite foot. He admitted he might have been mistaken about which foot had stepped on the hook. Once again, the magnet adhered to his heel, but imaging was inconclusive.
 
During this second visit, I conducted a more thorough physical examination and was able to reproduce the pain. I diagnosed him with bursitis, and he was satisfied with the explanation. In hindsight, I regret relying too heavily on his knowledge of soft tissue anatomy due to his profession. Yet, I still find myself puzzled—how did the magnet stick to his heels?

I’m not sure if this advice is proper, but if you are dealing with inflated practice numbers obtained by fraudulent billing, and the seller is not willing to renegotiate, I would look into a potential whistleblower strategy. That being said, the following advice is a good strategy for purchasing a practice: 
 
When I sold my primary office, the person who eventually purchased the practice shadowed me for a couple of hours prior to closing the deal. This entailed watching patient treatment and interaction followed by a discussion of procedures and billing. Before the sale was completed, the buyer had a very accurate assessment of the type of practice he was purchasing. I strongly recommend this protocol when purchasing a practice. One can look at all the numbers they want. Without really getting a pulse on the practice, you potentially can end up in an unfortunate situation like the one “Name Withheld” is in.

The last few days, I’ve been thinking about exactly what this profession we call podiatry is. It’s important because it’s so much more than a foot doctor but may not be a foot and ankle surgeon for everyone who still refers to themselves as podiatrists. This idea of a definition becomes important as we explore the concept of a single board for certification and as we try to salvage the profession from obscurity as admissions continue to dwindle under the degree DPM.
 
I recalled a Latin term popular in philosophy and law. The term is sui generis which best translates for our purposes as “unique unto itself.” According to Aristotelean logic, it is difficult to define podiatry with a statement that contains the essence of the term and an accidental condition that is descriptive but does not alter the essence as this accidental term is changed. The observation of Dr. Myers is not at all unusual, as malpractice litigation is the paragon model of damned if you do, damned if you don’t. Some years ago, I received two cases to review in the same week. In the first case, a patient underwent an Akin osteotomy and second toe arthroplasty for a main concern of a painful hammertoe. The podiatrist concluded the hallux required correction to allow for reduction of the second toe, the Akin resulted in pain, swelling, and non-union, and the podiatrist was sued for performing an unnecessary Akin. 
 
In the second case, a patient underwent a second toe arthroplasty for a painful hammertoe. The hammertoe recurred rather quickly, and the podiatrist was sued for not performing an Akin procedure to reduce the concurrent hallux valgus. You might argue that each case might be meritorious in its own circumstance. My thought? Get them both in the same courtroom and let a jury determine whether a hallux valgus deformity in a patient with a symptomatic second toe hammertoe but asymptomatic hallux valgus should or should not be reduced. Like most discretionary surgeries, it comes down to “it all depends”. 
 
This is one more reason that documentation and rationale for treatment decision-making is so important. There are 85,000 malpractice cases filed every year. You cannot watch television or listen to the radio without hearing ad after ad from personal injury lawyers. When you document in the medical record, remember that the mythical “standard of care” is often a standard of “it all depends” on the individual circumstances. This is where your documentation can determine the outcome of a medical negligence accusation against you, or whether a suit is even pursued.

More than 35 years ago, I bought a policy. I was one fortunate person, never collecting. As with all insurance, it gives you peace of mind. However, you must examine the definition of disability. My original policy said that if you could do anything in podiatry, you were excluded. I dropped that one and bought one with the correct definition, mainly any disability partial or fully paid out.
