NYCPM Multicultural Cook-Off Benefits SNPMA...and Eaters  
 
On March 11, the New York College of Podiatric Medicine (NYCPM) Student National Podiatric Medical Association (SNPMA) hosted its 2nd Annual Multicultural Cook-Off, inviting culinary contestants from among NYCPM students, faculty, and staff to cook their best.

A large panel of judges considered the entries: Dr. Joy Reidenberg (Faculty), Jaskern Dhami (Class of 2020), Stephanie Solanki (Class of 2020), Thanh Thao T Nguyen (Class of 2021) Felecia Foster (Clinical Dept.), Walquidia Pimentel (Clinical Dept.), Belinda Nicholas (Pre-Clinical Dept.), Keeshagaye Whitter (Business Office). The winner in each category received a $50 gift card courtesy of the Administration. An additional $25 gift card was offered by SNPMA to a very close runner-up.

NY Podiatrist Recommends Training Barefoot
 
Feet have thousands of nerve endings that help you not only feel the floor but also send signals up the body to help you understand your movements better, says Emily Splichal, DPM, a podiatrist and founder of Naboso Technology, a company that makes products to promote barefoot movement. "When you stimulate the nerves of the foot, you get a better understanding of what you're standing on and how you're stepping, and it starts to shape your overall movement," says Splichal. Cushioned shoes block this floor-to-foot connection (especially those with extra support and stability like those you'd lace up for a run).

Biomechanically, working out with your bare feet on the ground also means you can better activate through your glutes and core. "Someone working out might not feel their glutes in a squat, and that's not necessarily because of weakness, but because they haven't established their foundation," says Dr. Splichal.

When was the last time you reviewed your partnership agreement? Even if you are not currently contemplating new shareholders or partners, if you haven’t looked at your agreement in the last three or more years, now may be the time. From the perspective of business operations, let’s break out some of the key questions that you should consider in every partnership agreement:
 
6. Individual risk: Is each partner responsible for their coding and documentation? In the event of an insurance audit that results in ‘take backs, is the group liable for an individual’s errors?
 
7. Different classes of shares: Do the senior partners retain voting rights while more junior owners remain non-voting shareholders?

Query: Telemedicine Visit
 
Has anyone billed for a telemed visit yet? We had a patient call after her global period for her ingrown toenail she had removed and the doctor counseled her in what to do and also looked at pictures she sent him. I’m still a little confused about the different types of codes for the CPT 994XX series. The phone call was around 5 minutes long.

Response: FIrst, you must find out the following:
1) Does your state allow DPMs to participate in telemedicine? Texas does not.
2) Does the insurance company have coverage for telemedicine or do you have to directly bill the patient?
3) Assuming 1 and 2 are yes, then the following codes could be used:
G0406 Follow-up inpatient consultation, limited, physicians typically spend 15 minutes communicating with the patient via telehealth
G0407 Follow-up inpatient consultation, intermediate, physicians typically spend 25 minutes communicating with the patient via telehealth
G0408 Follow-up inpatient consultation, complex, physicians typically spend 35 minutes communicating with the patient via telehealth

RE: The Passing of Brenda M. Salocker-Paullus, DPM
 
Dr. Brenda M. Salocker-Paullus, 69, of Mason City, went home to the Lord on Saturday, March 30, 2019, at MercyOne North Iowa Hospice in Mason City, She earned her doctorate from Illinois College of Podiatric Medicine in 1977, and served her surgical residency at Franklin Boulevard Hospital-Chicago in 1978.

Brenda began her medical practice in Fort Dodge in 1978, and in 1980, she began her position as the director of Trinity Regional Hospital's two-year surgical residency program. Terry Branstad elected her to a nine-year term as chairwoman of the Iowa Board of Podiatry Examiners.

RE: Medicare Advantage Plan Audits
 
It is that time of year again that a lot of podiatrists are receiving requests for charts from Medicare Advantage and other plans. As if we are not burdened enough with paperwork and jumping through hoops for insurance payments, these insurances are asking for patient charts for audits as required by Medicare. Companies such as Arro health, Inovalon, Ciox, are contracted by the insurance companies. They request doctors’ charts and gather information that can actually increase their fees from the contractors. They contact our offices via telephone or fax with their requests. The number of charts can range from a few to many. They are relentless in their efforts that borderlines on harassment.  
 
My solution to this big inconvenience is to fax them an invoice for $25 per chart. My invoice itemizes the charge as chart recovery, copying, and preparation. I will not send a chart without payment. I then put these requests in a file and send them if payment is received. I will not copy any patient charts until payment is sent. I have taken this inconvenience and turned it into a win/win situation. Most of the time, they do not pay (70%-80%), but when they do, I get paid for my time. You are entitled to get paid for this service, and you should!

Dr. Levy provides us with his experience and insight regarding medical education. Thank you for doing so. If all that is required to equate MD/DO and DPM is the addition of 3 courses and rotations (as suggested by Dr. Levy), then why has that not been done? Temple and Rosalind Franklin have medical schools; Des Moines an osteopathic school. 
 
What barriers exist for a trial program to test Dr. Levy’s and Dr. Secord’s suggestion that the education of a DPM is almost equivalent to that of an MD/DO. Add the necessary education and have the students take the USMLE or COMPLEX examinations. What are the reasons this has not...

RE: Outrageous Foot Pathology of the Day

A 10-year-old girl who had developed itchy, painful brown-colored papules on the bottom of her feet was actually suffering from a parasitic sand flea infestation. The girl, who had traveled to Brazil with her family two-weeks prior to visiting a health clinic, reported running through a pigsty with no shoes on.
 
What happens when you walk barefoot through a pigsty!
 
Her case, which was published in the New England Journal of Medicine, included a 10-day history of pain and itchiness before she sought care. Doctors then removed the sand fleas from the bottom of her foot and diagnosed her with tungiasis. In the girl’s case, doctors treated her wounds after the fleas were removed, and her feet healed with no complications.
