Podiatrists Can Lower the Rate of Amputation in Diabetics: TX Podiatrist 
 
Dr. Joseph Caporusso is a podiatrist in McAllen. He started practicing medicine in 1989. During that time, he said he saw a lot of patients with diabetes. “Probably 75 percent of our patients here have diabetes and have some type of foot problem,” he said. Dr. Caporusso said diabetes affects the circulation and nerves. “These people’s feet are so numb, they could step on a nail, go right through their shoe and have no realization that they have something going on in their shoe until they see the blood on the floor,” the doctor said.

Dr. Caporusso said not all patients have to undergo an amputation. Statistics showed 75 to 80 percent of the ulcers that don’t heal are at risk of being amputated. He wants to slow down the complication rate. “We’re trying to slow down the amount of people that get an amputation because of diabetes and because of non-healing ulceration,” he said.

NY Podiatrist Discusses Treatment for Flatfeet in Children
 
All healthy babies are born with flat feet, says Dr. Michael Pliskin, chief of podiatry at North Shore University Hospital in Manhasset. So they all initially have the appearance of a flat foot. That's why new baby footprints are so full and cute, he says. If, as the baby becomes a preschooler, no arch appears, parents should either point it out to their pediatrician, who may recommend a consultation with a podiatrist, or make an appointment directly with a podiatrist, he says.

A podiatrist might suggest a custom-made orthotic device be placed in the child's shoes, depending on whether the child is experiencing symptoms such as pain or the beginning of other foot deformities. Or he may just watch the child periodically or suggest physical therapy, Pliskin says. "I don't usually recommend orthotic devices until they are about 4 years old," he says. "That's when they start doing that heel/toe gait, putting the heel down, passing through the arch, and propulsing on the toes."

39% Say They Would Exercise More if Their Feet Didn't Hurt: APMA Study 
 
According to a recent survey conducted by the American Podiatric Medical Association (APMA), 39 percent of American adults said they would exercise more if their feet didn't hurt. With this in mind, APMA has announced a new campaign for Foot Health Awareness Month called "Today's Podiatrist Keeps America Walking." This campaign will reach all adults who are active or wish to be active to educate them about the importance of foot and ankle health to a healthy lifestyle, and a podiatrist's critical role in helping to treat and prevent foot and ankle injuries.

"Many Americans cannot wait to start walking, jogging, running, or hitting the gym when the warm weather returns," said APMA President R. Daniel Davis, DPM. "With this excitement, many adults find themselves pushing their limits and suffering from foot and ankle overuse injuries, which can prevent them from keeping active. That is why it is so important to see a podiatrist as soon as you get injured."

Premier Medical Group, Poughkeepsie, recently announced that the office of Dr. Leonard Gerber and Dr. Christopher Bromley, who specialize in podiatric medicine and foot surgery, has joined its group of providers. The addition of the practice will bring five new podiatrists to the group’s existing podiatry division, which includes Drs. F. Gabrielle Summers, Audra Siegel, and  Dany Jabbour.

Canadian Dentist Banned for Life is Now a Podiatrist
 
A former dentist whose license was revoked after a patient stopped breathing in the dental chair and later died is working again in the healthcare field despite attempts to block him from practicing. A Go Public/Radio-Canada investigation found Dr. Pierre Dupont went from disgraced dentist to working foot doctor when he moved from Quebec to Ontario. That's despite attempts by a major Quebec university and the organization that oversees podiatrists in Quebec to block him from practicing. After Dupont enrolled at the University of Quebec at Trois-Rivieres in 2006 to study podiatry, the school tried to kick him out of the program after finding out about his problems as a dentist. Dupont took the university to court and won, arguing he had disclosed information about his past to the head of the program.  
 
After graduation, Quebec's College of Podiatrists also tried to block Dupont by refusing to issue him a permit needed to practice. Dupont appealed the decision. A tribunal eventually ordered the college to issue the permit, ruling there were not enough grounds to bar him. But rather than face another battle with the college for working before the permit issue was resolved, Dupont agreed not to practice podiatry in Quebec. That's when he moved to Ontario where the College of Chiropodists did accept him. He set up shop in Ottawa as a chiropodist, a limited form of podiatry.

Final 2017 Medicare Advantage Payments Lower Than Proposed
 
The federal government has lowered average payments for 2017 Medicare Advantage plans, and it also modified several of the program's policies after weeks of unremitting lobbying from the health insurance industry. But overall, the Obama administration showed a commitment to its proposals which the powerful industry vehemently opposed. One of the most notable policy changes within the CMS' 250-page policy document was the decision to phase in cuts to employer-sponsored Medicare Advantage plans over two years instead of instituting the cuts in 2017. That policy was not nixed altogether, much to the chagrin of insurers.
 
Payment rates for insurers that sell Medicare Advantage plans will rise by 0.85% on average for 2017, the CMS said in its final rate notice Monday, which came out after the stock markets closed. That compares to a 1.35% proposed rate from February, although many insurers argued the average proposed rate was misleading and likely closer to breakeven after factoring in other payment policies. The average Medicare Advantage payment rate will increase by 3.05% after taking into account the way health plans code their members' diagnoses, the CMS said. That's down from February's estimate of 3.55%.

Query: Diabetic Shoes in Assisted Living Facility
 
I was asked to order shoes for a diabetic patient (Medicare and ward of the state) who presently lives in an assisted living facility (POS = 13). Besides the necessary paperwork to be filled out by the primary MD/DO, are there any different requirements for reimbursement as there are when a patient is in a nursing home?  Do you need to have an agreement with the facility or an ABN from the family prior to fitting the shoes because the patient does not have the capacity to make decisions?

Response: Before proceeding any further, be perfectly sure that this patient is eligible for Part B fee-for-service Medicare. Because she is a "ward of the state", you want to be certain that she is not enrolled in a mandated managed dual eligible plan. This would be considered a Part C Medicare plan and could carve out DMEPOS to only large commercial suppliers. If she is confirmed as Part B Medicare, then the same rules apply to a patient living at home. Assisted living (POS 13) is not the same as Part A Medicare (which would cover the patient in a skilled nursing facility - POS 31) where consolidated billing regulations should apply. 
 
In my opinion, no separate agreement with the assisted living facility should be required. It would be the same scenario as a patient living in a nursing home (POS=33) but not in a skilled nursing bed.
