NY Podiatrist Warns of Risks of Barefoot and Minimalist Running

The debate about running shoes has been particularly heated, focusing on the merits of running barefoot or in “minimalist” shoes that mimic running barefoot. “For thirty years, there has been very little structural change in running shoes,” says Dr. Alan Berman, a podiatrist with Somers Orthopaedic Surgery & Sports Medicine Group. “Now runners and shoe manufacturers are jumping on the barefoot bandwagon with dramatically different designs that have serious implications for runners' form, and the potential for injury.”

“The bottom line,” Dr. Berman concludes, “is that the body clings to what it knows. Changing shoes doesn't mean you will automatically run in proper barefoot form. Many new barefoot runners continue to stride as they always have, landing heavily on their heels but without the cushioning of their usual shoes, which increases their risk of injury. Further, we are all different in our biomechanics, our daily activity, and our level of training or running experience. So a ‘one-size-fits-all’ approach is not appropriate and minimalist shoes certainly isn't right for everybody.”

Magnolia podiatrist Dr. Carlotta Wells-Thomas told about 100 youths they can achieve anything in life if they are determined to do so. "I stumbled upon college by my association with other kids who were on that track. However, my desire to be like them or better kept me focused and interested," said Thomas.

QUERIES (CLINICAL)

Query: S/P Fracture Left Ankle

A 52 year old female presented after being treated on 3/24/12 at a local hospital for a fractured left ankle (distal fibula shaft). The patient has been using crutches and complains of pain w/attempted weight-bearing. Past medical history includes depression, seizures, HTN. Social history: 4 cigarettes/day for 30 years, drinks alcohol 1-2 per day. Clinically, vascular status within normal limits w/forefoot swelling for a splint noted. 

S/P Left Ankle Fracture

X-rays show a mildly displaced (about 2mm) fracture of the distal fibula shaft. I know what the literature says about the 2mm rule, but I'm not 100% comfortable taking this patient to surgery based on her social history. Currently, she's in a BK cast and will be using a knee roller for ambulation for 6-8 weeks. Perspectives are sought for this matter.

QUERIES (EMR)

Query: E-Prescribing Program
 
I initiated EHR and E-Rx on June 1, 2011. Although 12 E-Rxs (10 are required) were sent in June, only 8 were Medicare patients. I received notice last month, without explanation, that my Medicare payments for 2012 are reduced by 1%. Do I have a legitimate challenge to this reduction of payment? I have received no explanation from Medicare to date.

Query: Digital X-Ray Sizing

I have a digital x-ray system. How does one know if one has an accurate 1:1 sized image? The tech dept of the system I purchased was unable to answer this simple question. I was planning a surgery and wanted to use the x-ray as my template. I realized I have the capability of zooming in and out, but then wasn't sure when I actually was looking at the true-sized image.

RE: Titanium Screws and Metal Detectors (Alan Mauser, DPM)

I don’t know how many screws, plates, etc. that I have implanted in 23 years. I can tell you of only two incidences that have occurred regarding metal detectors. One was following a major rear foot reconstruction using two titanium plates and several screws. The patient was a lawyer, and the detector going into the courtroom went off. I was called from the courtroom to confirm the implants. The problem was that the sensitivity of the machine was set too high.

The other case was another major reconstruction in which lots of hardware was used. They used a wand at the airport and could see the scars. There was no problem, just a hassle for the patient. There are small cards (like credit cards) which you can give to patients that they can put in their wallet/purse that gives the implant name, location, your name, and the date of application. Ask your Wright Medical rep for them. I have never had any issue with a single metatarsal osteotomy screw, but certainly I would suspect the metal detector and have it looked at for adjustment before I took someone to surgery to take out a screw.

This looks like an acute dermatitis. Use a class-1 steroid, e.g., Temovate in conjunction with a keratolytic agent such as urea.

It seems that you have thrown everything at it for tinea and hyperkeratosis that you could without significant results. It might be time to change the diagnosis or dig a bit deeper. My first impression is that this is a form of dermatitis, likely psoriasiform spongiotic dermatitis. I have had several patients with a similiar presentation and had excellent results with a combination of topical keratolytics and oral as well as topical steroid preparations. I would certainly recommend a full thickness biopsy be sent to a dermatopathology lab for diagnosis and  documentation.

I have used both Dragon 10 Medical Professional and am currently using Dragon 11 Medical Professional. The professional version allows you to use bookmarks so you can navigate through your templates with your voice or keyboard. I have not found a significant difference between the 10 and 11 versions. Most EMRs require you to use at least version 10.  Get a good microphone since the one that comes with Dragon is not very good. I use the VXI TalkPro USB 1. Dragon works well with AllScripts and allows for full text control. Not all EMR's allow full text control with Dragon, which I find unacceptable. I just wish the billing part of AllScripts was better.

RE: Pyrocarbon and Foot Surgery (Lowell Weil, Sr., DPM)

We agree with Dr. Lowell Weil, Sr. that pyrocarbon is the hottest thing in foot surgery about to come down the pike. We have been interested in pyrocarbon for the replacement of the 1st MPJ for several decades – Dr. Hetherington, since the late 1970s, and Dr. Kawalec-Carroll since the early 1990s. Pyrocarbon is an ideal material for foot implants because the mechanical properties are similar to that of bone. Our research has shown that pyrocarbon implants show more potential for success as a hemi-implant, compared to cobalt-chromium alloy implants. (1)

Pyrocarbon has a very smooth surface, which does not allow for adequate fixation in bone. We have been studying various methods to increase fixation of pyrocarbon implants. Our studies found that hydroxylapatite (HA)-coated pyrocarbon implants have...
