MA Podiatrist Provides 4 Steps to Prevent Diabetic Amputations
 
Diabetic foot is progressive, meaning it gets worse over time, and is a leading cause of foot amputations. However, most amputations can be avoided with early intervention. "People tend to wait too long," says David Alper, DPM, a retired podiatrist and member of the American Diabetes Association's (ADA) Northeast Leadership Board. If wounds occur in the feet, they're slow to heal because adequate blood flow is needed for tissue to heal, says Dr. Alper. These wounds can become infected, leading to serious complications like diabetic ulcers, a leading cause for amputations.

Foot care doesn't have to be complicated, but does require daily attention, says Alper. "1) Look. 2) React. 3) Prevent, and 4) Control your blood sugars," says Alper. "Most issues, including amputations, are preventable by following those four steps."

Nurses Should Invest in Compression Stockings: CA Podiatrist
 
Ebonie Vincent, DPM recommends that nurses consider wearing compression stockings, especially if they spend the majority of their shifts standing. She explained that compression stockings can be helpful because veins have valves that push blood back up where it needs to go, but when you’re standing, gravity takes a huge toll on those valves and sometimes, blood can pool in the legs. The blood pooling can cause issues such as spider veins and varicose veins, so compression stockings can help push the blood back up where it needs to go.

“I personally wear compression socks sometimes in surgery when I have long cases,” said Dr. Vincent. “I'm just like, ‘Put these on up because I'm trying to try to have sexy legs when I'm 70, you know what I mean?” she added with a laugh.

Query: Denials Using L84
 
We used to bill Medicare for mycotic toenail debridement and callus paring for our diabetics with neuropathy, using E11.49, L84, and B35.1 with XS modifier. We have had no problems getting paid until recently. The recent claims have denied the CPT 11056, stating that the diagnosis of E11.49 and L84 do not meet CMS guidelines of medical necessity for payment on a CPT 11056. In fact, when looking in the Medicare Online Only Manual, L84 is no longer present. Is this a recent change and if yes, what are practitioners using as a callus code? Help please!

Response: There were changes in the Noridian RFC policy. This impacted both JE and JF regions (you are JF region). There is a list of allowed codes for nail and callus codes as well as allowed pain and systemic codes on the Noridian website. Some codes were expanded and some restricted. Particular emphasis was placed on unlisted and unspecified ICD-10 codes. Update your computer system to reflect these changes.
 
That said, I would call Noridian to make sure the use of this code is what triggered the denial. There could be other reasons for the “not medically necessary” denial remark code.

QUERIES (CLINICAL)
Query: Non-Healing DFU of the 1st MPJ
 
The patient is a 53 year old IDDM with a non-healing ulcer of the plantar left 1st MTPJ. He had his left great toe amputated in August 2021 due to infection. His ABI was 1.1. He did heal while off work by early December. I sent him back to work with an AFO, a patellar weight-bearing device to reduce loading on the plantar hallux. He has returned to work as of 12/22/21, but after 4 weeks at his reduced schedule, the wound re-opened.
 
The use of a kneeler bike has improved the situation in the last 2 weeks. Any thoughts on the long-term management of this individual? The AFO is off-loading, and he removes it once he gets home. He maintains that he keeps the pressure off of it.

Review the layers of the dermis and where the nerves lie so that you're not going deeper than the level of the nerve. A 1cc wheal of local (0.5% Marcaine preferred) in the correct layer at the 4 "corners" of the mid-shaft of the proximal hallux phalanx almost always works. Less is needed for smaller toes.

I was in your situation back in the mid ‘90s, so I can feel the frustration you're going through.
 
See if the hospital you have privileges for has a PHO (physician health organization) and try to obtain membership (that is what worked for me, but it took time). Also, get a list of other medical specialists on the insurance panel and reach out to them for their help, especially the ones you have patients in common with.
 
Ask patients to write a letter or have a form letter that can be forwarded to your area provider representative. I even went as far as mailing a certified letter directly to an insurance company's president/CFO, pointing out all the reasons I should be included on the physician panel - and it WORKED! You have to NETWORK and be PERSISTENT.

My sympathies go out to the author of the post. Any type of managed care health plan has two primary objectives. The first is managing the network financially and to increase their profit margins. The second is geoaccess. That means having enough providers to service the patient population. Without geoaccess they can’t sell their product. They will be hesitant to add providers above what they consider adequate geoaccess.
 
It is well known that overloading a network with providers will increase the medical costs to the health plan. Quality of medical care, although obviously their marketing byline, usually only becomes a discussion when a negative outcome occurs. So what can a young doctor do when contemplating opening an office? The first thing is to do your due diligence. Find out who the big players are and find out if their panel is open or closed. If it’s closed you may want to consider joining a group that is contracted or finding a different location where you are needed.  
 
Of course that doesn’t help the poster who has already committed to a location only to find out the panel is closed. My suggestion to him is to ask to meet with the medical director and explain your unique qualities/training that could bring added value to their patients. And then be persistent….weekly calls, emails, etc. That actually does work on occasion. The final option would be to forget the health plan and market yourself as a concierge practice. If you are successful, you will be much happier in the long run.
