"Foot Pain is Never Normal" - FL Podiatrist
 
While the great majority of American adults across generations experience issues with their feet of some kind, younger generations are less likely to understand the connection between foot health and overall health—and are also less likely to understand the benefits of consulting an expert podiatrist when necessary, according to new research conducted by the American Podiatric Medical Association (APMA).

WI Governor Signs Bill Allowing Podiatrists to Delegate Certain Services to a PA 
 
Governor Walker has signed Assembly Bill 582 allowing a podiatrist to delegate certain services to a physician assistant. Authored by Senator Jerry Petrowski (R—Marathon) and Representative David Murphy (R—Greenville), the bill passed the Assembly on a voice vote and was concurred by the Senate on a voice vote. It is Act 227.
 
The new law allows a physician assistant who is under the supervision of a podiatrist to engage in the practice of podiatry, except that, like an advanced practice nurse prescriber, the practice of the physician assistant is limited to providing non-surgical patient services. The law also allows the Podiatry Affiliated Credentialing Board to promulgate rules regarding the practice of podiatry by physician assistants.

Aussie Podiatrist Enjoys Treating Complicated Foot Problems
 
Podiatrist Sally Belcher recently treated a patient who had sustained a fracture from spraining her ankle. The patient’s fracture had been healed for five months, but the inadequate rehabilitation she had received meant that she was still experiencing pain. After Belcher assessed the patient’s walk and her shoes, she fitted her with an orthotic solution to support her foot. Within a week of doing so, the pain dissipated.

“When there's another practitioner involved, whether it’s a physiotherapist or chiropractor or GP, when they can't quite work out why the person is still in pain, they’ll refer to a podiatrist,” says Belcher. “All we deal with is feet all day, so we know them quite well.” One of the most satisfying aspects of her work, she says, is treating patients with complicated problems. “Someone comes in with a pain in a certain area of the foot and you have to understand the function of the foot to understand why that area is under strain or pressure and hurting for the person,” says Belcher.

Aussie Podiatry Clinic Receives $250,000 Grant
 
Echuca Podiatry will step into a new era thanks to a $250,000 grant from the Federal Government. Federal Member, Damian Drum, dropped into the clinic on Friday to announce the grant, which is part of the $20 million Regional Jobs and Investment Package for the Goulburn Valley region.

The grant will be a vital kickstarter for Echuca Podiatry’s planned $775,000 expansion, which will see the clinic double in both size and staff. "Without this grant, these expansions would take us another 10 years," Echuca Podiatry owner Emma Hall said. "By setting up this clinic in Echuca, I’m hoping to provide a service so people don’t have to travel any more."

Painful rashes are not common. This child needs to be evaluated for cardiovascular problems/vasculitis. Once ruled out, then a skin biopsy would be appropriate. Hand, foot, and mouth would be the first consideration, but is usually self-limiting. I would get on the phone with the pediatrician and seek referral to a pediatric cardiologist and/or rheumatologist.

The painful rash on the hands and feet of a two year old likely represents a Cocksackie A 15 viral infection, otherwise known as hand, foot, and mouth disease. He should be seen by the pediatrician and have his throat and mouth examined. This infection is generally self-limiting and extremely common in that pediatric age group.

My son is a pediatric dermatologist at the University of Virginia. I sent him this case and photo from PM News. His differential diagnoses include: neutrophilic eccrine hidradenitis, erythromelagia, dyshidrotic eczema, and mercury poisoning. He recommends consulting a pediatric dermatologist of course!

RE: Costco Selling Custom Orthotics

I was browsing the publication, published by Costco, and to my surprise there was an advertisement for custom insoles. The company is listed as RESA. They have a scanner that you step on and then a 3D printer fabricates an insole. They state that a medical grade orthotic cost $300.00 or more and theirs is $139.99 for adults and $99.99 for children under 16.  I am curious how my colleagues react to this and question if this is a custom orthotic and needs to be prescribed by a medical practitioner.

I found in my Costco members' newsletter that they're apparently getting into 3-D printed orthotics (yes, they use the term orthotic, as well as insole). Your foot is scanned by an "operator", who then asks questions about your activity, shoe type, etc., and the insoles are printed while you shop. There is no mention at all of any type of podiatric or medical personnel involved in the process.

I respectfully disagree with Dr. Udell's post, "How would you feel if that person leaves your practice and gets a job working for an MD across the street and the doctor starts advertising that he or she now provides foot care by a well-trained PA or NP." These "podiatric insecurities" are still unfortunately part of the paranoia that was ingrained into our training and psyche. When podiatrists whom I have trained moved down the street, it never negatively affected my patient load; it only increased my bottom line. 
 
Competition is good and healthy as it raises awareness of what a podiatrist's scope of practice is. With the obesity crisis in this country, there will be more work for all of us (think wound care,diabeties, plantar fasciitis, etc.). 
 
Having NP/PAs you have trained who work in a PCP's office will only enhance your practice as they will know what services that they and their new boss do not want or know how to do. "The more the merrier".
