PODIATRISTS IN THE NEWS
OH Podiatrist Now Does Wound Care Exclusively

Saving a limb often means saving a life, said Dr. Windy Cole, associate medical director of the Cleveland Clinic Akron General Wound Center. More than half of patients who have a leg amputated die within five years, she said, because it often limits their ability to function. Lack of mobility can worsen pre-existing conditions like diabetes or increase the risk of pneumonia.

Dr. Windy Cole

Cole was trained as a podiatrist, but started moving into wound care about five or six years ago. Now it’s all she does. She has pulled safety pins, thumb tacks, toothpicks, and all sorts of other sharp items out of patients’ feet. She has treated people with third-degree burns on their soles after walking barefoot across hot pavement in the summer. And one particularly unlucky man lost his fourth and fifth toes to his dog when he fell asleep. The dog wasn’t being malicious, Cole said. It was doing what dogs do — licking a wound.

RELEVANT RESEARCH
Less Load Measured Through Foot When Subjects Wore Walking Cast Frame

A walking cast frame used for lower limb immobilization helped substantially reduce transmission of force through the foot during weight-bearing compared to a Sarmiento cast above-the-knee and a standard below-the-knee cast, based on study results presented here. A walking cast frame significantly reduced the mean peak force through the first metatarsal head through the fifth metatarsal head and through the calcaneum when compared to a standard below-knee cast, James T. Berwin, MBBS, and colleagues noted in their study.

The load on the foot from casts has not previously been evaluated, Berwin said. He and his colleagues devised a measurement system that used force sensors applied to the first and fifth metatarsal head and the plantar surface of the calcaneus to obtain baseline force readings. They used the system in seven men and seven women who were healthy volunteers. Force measurements were taken first without a cast applied, according to the abstract, and then with three separate casts. “Compared to the standard below-knee cast, the Böhler Walker reduced the mean peak force through the first metatarsal head by 58.9%,” Berwin said.

PRACTICE MANAGEMENT TIP OF THE DAY

Online Reputation Protection for Physicians - Part 6

6. Aim to delight

It's amazing to see what a truly service-oriented staff can do. When patients are greeted uniformly by staff who are personally committed to caring for the comfort of each patient, your practice can stand apart from the typical practice where staff seem disinterested and/or too busy to bother. A side benefit of the high-attention-to-patients practice — it's much more pleasant to work at too!

RESPONSES/COMMENTS (CLINICAL)
From: Dennis Shavelson, DPM

I have a few questions to ask Dr. Shea about “the cuboid syndrome.”

1. How can the peroneal and posterior tibial muscles and the plantar fascia be both overworked and weak?
2. How does cuboid pathology affect the posterior tibial muscle engine?

Dennis Shavelson, DPM, NY, NY

RESPONSES/COMMENTS (NON-CLINICAL) - PART 1
From: Joseph Borreggine, DPM

Reimbursement for the 4th year comes from the budget of CMS dollars for the first 3 years. The hospitals do not contribute. The 4th year pays less than the first 3 years.

ACGME approved fellowships do get paid in the medical world. Hospitals do contribute to the fellowship because the fellow is an employee of the hospital, or the funds come from the research arm of the teaching university or from the certifying board of that specialty.

Joseph Borreggine, DPM, Charleston, IL

RESPONSES/COMMENTS (PM NEWS QUICK POLLS)
From: David Secord, DPM

I understand that Dr. Kittay was attempting to point out the futility of increasing the number of schools and graduates without making sure that these people have the possibility of post-graduate training. However, his use of the quote: “…some people are encouraging even more student enrollment as the solution - the same people who say lower taxes will yield increased revenues,” doesn’t make sense.

Every, single administration which has lowered taxes has seen the economy expand, the number of taxpaying employers and employees increase, and, yes, the federal coffers expanded. It worked for Bush, worked for Reagan, and it worked for the King of Camelot: John Kennedy. The slashing of the tax rate by JFK remains the largest in the nation’s history, a factoid that some seem to hate.
