Examine Your Feet Daily: IN Podiatrist

Most people believe "out of sight, out of mind" when it comes to their feet. But a local doctor says it's important to pay attention to an important part of our body that keeps us mobile. "Because our feet are tucked away in boots and shoes, we actually need to be more thorough about taking care of them," says Michael Nirenberg, a doctor of podiatric medicine in Crown Point.

"Examine your feet daily. If you cannot see the bottom of your feet, use a mirror or have your spouse check them," he says. "Checking your feet daily is especially important for those people who are diabetic, have poor circulation or are elderly, as these people are at higher risk for foot infections."

AT THE COLLEGES

Barry Students Care for Needy at Annual Outreach

With the help of more than 400 volunteers, the Miami Rescue Mission/Broward Outreach Centers recently served more than 2,000 meals to the homeless and hungry of South Florida at their three campuses in Miami, Hollywood, and Pompano Beach. In addition to the meal, homeless and needy men, women and children in attendance were able to have their feet washed by volunteers and cared for by students from Barry University’s podiatry program.

“I haven’t had someone be concerned with my well-being for months, much less care for me like this,” said Mary Thomas while receiving foot care from a volunteer. “I didn’t think something this simple could make me cry, but it is just such a beautiful thing they’ve done here.”

PODIATRY ENDOWMENTS

Ornstein-Guiliana-Bakotic Podiatry Trust Formed

A trust was recently established by Hal Ornstein, DPM and John Guiliana, DPM, MS with the primary purpose of supporting practice management initiatives for podiatry students and residents as well as other areas within the profession. This evolved out of an investment with a podiatry company that was recently sold, and the execution of a promise from both doctors that their efforts on behalf of the trust would be 100% voluntary and with no personal financial gain.

The first use of this trust will be to offer CDs to the library of each podiatry school to help with student self-development, and provide a strong foundation to be the best practitioner possible.  When Dr. Brad Bakotic heard of the trust, he offered to be a partner in the endeavor and match the amount in the trust, contributing $21,000. Additionally, he will provide significant financial support to the trust in the future.

QUERIES (CLINICAL)

Query: Dense Macules/Nevi
 
A 79 year old Caucasian male presented with a chief complaint  of several hundred B/L pigmented spots from his knees distally to his toes which started showing up one year ago. He has no pain or pruritus, but is embarrassed about the appearance. He saw a dermatologist who biopsied one of the lesions, but no conclusive diagnosis was obtained. PMH: asthma, GERD, DJD, HTN, hyperlipidemia, basal cell carcinoma x2 removed from scalp. Meds: atenolol, Vytorin, minocycline. FH: one son deceased of melanoma age 53.

Dense Macules/Nevi

An exam reveals hundreds of circular brownish black macules and patches which are very dense on the shins and medial malleoli which are non-raised but some have scaling. Any diagnosis/treatment ideas would be appreciated.

What does the MRI show about the inferior calcaneal bursa?

I applaud Dr. Miller's approach to the stubborn heel pain patient. At this point, I recommend an ankle MRI to rule out a partial rupture of the plantar fascia near the insertion, most likely involving the central band. If so, then it's 4-5 weeks of immobilization in a cast, WB or NWB, depending on severity. Use a cast as opposed to Cam-Walker, because" removable cast" means the patient will do exactly that - remove it. This is a good treatment, even for fasciitis with no rupture. Then, for fasciitis or fascia rupture, try TOPAZ (Arthrocare) coblation. It's an 8 minute percutaneous procedure with excellent outcomes and minimal complications.

RE: Recalcitrant Heel Pain (Loren J Miller, DPM)

The patient appears to have recalcitrant heel pain despite the numerous conservative and surgical treatments employed. In light of the posting of treatments thus far offered, I suggest two follow up approaches in conjunction with post-operative orthotic therapy which I assume is continuing.
 
First, a full arthritic profile should be performed to see if there is a systemic arthritic condition that is contributory to his heel pain. The profile should consist of a sedimentation rate, HLA-B27, uric acid, rheumatoid factor and ANA. If these factors test positive, appropriate referral to a rheumatologist may be required.
 
Second, does the patient exhibit lower back symptomatology. Lower spinal segment impingements can also contribute to recalcitrant heel pain. Physical therapy and/or chiropractic care is essential after MRI assessment of such pathology.

I heard about this treatment when I was in podiatry school (1978). Is this another anecdotal treatment? Are ABIs/PVRs being done pre-treatment? If it is an ischemic wound and blood is restricted to the foot, how is vasodilating the capillaries helping with blood flow? What about PT blocks? Shouldn't they be more beneficial. Will these topicals work if blood flow is restricted to the ankle? Is there any system absorption of the nitroglycerine paste which could impact the cardiovascular system?

The outside of the box answer is right in front of us. There is a shortage of doctors! This shortage is forecasted to grow. This is a great opportunity for podiatry schools to help the nation! Just offer a joint degree option, DPM/DO or DPM/MD! Beef up the academic course of study to qualify the graduates to go down either path, or both paths, as they choose This program will eliminate the need for marginal podiatric residencies and strengthen the remainder. How is that for PARITY!! Remember, there are only people who manage to get things done and those who don't!
