Early Signs of Diabetic Foot Often More Noticeable at Night: OH Podiatrist
 
The condition diabetic foot affects about half of people with diabetes. "This can be a serious condition, because peripheral neuropathy can lead to increased falls, cause pain, and lead to decreased quality of life," says Kevin Springer, DPM, a podiatrist at The Ohio State University Wexner Medical Center.

Recognizing the early signs of diabetic foot is essential, but can be difficult. "The earliest warning signs can be vague and go unnoticed," since they are subtle and not easy to feel due to nerve damage, says Dr. Springer. Early symptoms are often more noticeable at night, Springer says.

Medicaid Expansion Has Led to Dramatically Fewer Amputations: Study
 
Dr. Tze Woei Tan (University of Arizona) and Prof. David Armstrong (USC), et al. published a study in JAMA Network Open finding that Medicaid expansion following the Affordable Care Act was associated with a relative improvement in major amputation rates among racial and ethnic minorities (n=115,071 hospitalizations) in early-expansion states (defined as Medicaid expansion by January 2014) compared to non-expansion states. Medicaid expansion associated with a lower increase in ulcer-related hospitalizations and amputations for racial and ethnic minorities, regardless of insurance type
 
Uninsured adults in early-adopter states from a racial or ethnic minority see a whopping 33% reduction in amputation rates with no change in non-adopter states. Medicaid expansion has great potential to solve a “mega disparity”; adds to the growing body of evidence from the ADA 2021.

Query: Difficulty for New Podiatrists to Get into Provider Networks
 
I am just starting out in practice. The most common private insurance in my area is Fidelis Care. I cannot get into their provider network, and most of the patients do not have the ability to go out-of-network. There are a few other insurance plans I cannot get into. This is going to put a big damper on my ability to make a living. How can this be legal? Any tips for getting into these networks? When I apply, I get the following message:
 
"On behalf of Fidelis Care, I would like to thank you for your interest in our provider network. After a careful review of our present provider network, we have concluded that at this time, we will not be expanding our network in your specialty area."

I recommend that after you have performed the digital block, you massage the medicine in for about 30-45 seconds. This helps push the medicine around the facial layers and speed onset time for anesthesia of the toe. I do this for all my digital blocks and I highly recommend it. If anesthesia is not achieved after 10 to 15 minutes, I would perform another digital block. The massage takes place at the injection sites around the toe, so it is not painful, and is proximal to the infected nail.

Kevan was a pillar of podiatry in Michigan for decades, and I think I can speak for all us and say we are still in a daze. Kevan was kind, caring, and a consummate professional. He was respected and admired and a role model for all us. On top of that, he was always fun to be with. As those who attended the APMA House of Delegates this year can attest that he was outstanding as his kindness, intelligence, and humility shined through in his first year as Speaker of the House. 
 
As someone who loved a microphone, he seemed born to do the job; and we all thought it was the beginning of a long tenure in that position. At his funeral, all of his children spoke and it was beautiful to see his basic goodness reflected through their words.  A gold soul, a truly great man gone too soon. Sadly, I don’t think I really realized how much I appreciated him until now.  
 
We should all be like Kevan. RIP my friend.

Dr. Morelli makes a good point when he says that not only should podiatry seminar lectures be streamed but should be recorded and made available to those who might have missed something. The AMA once did a study on the span of attention of medical doctors at conventions and found that after fifteen minutes, retention was markedly reduced.
 
Since the average lecture at podiatry conferences is an hour, I doubt how many people in the audience would pass a simple test on what they retained. Hence, making lectures available after a seminar serves an important purpose.
