Canadian Podiatrist Appointed as Medical Director of Centric Health

APMA Waives Past National Dues Owed for Former Members 
 
The APMA Board of Trustees has approved the waiving of any past national dues that may be owed by a former member. This will be available to any former member, even those who have taken advantage of a dues waiver in the past. If a former member wishes to re-join during the 2012-2013 year, he or she will only need to pay the first quarter of national dues (along with a completed member application, component dues, and any other documentation required).

Joint Commission Creates HAI Info Portal

The Joint Commission has unveiled an online portal containing information and resources to help providers curb rates of healthcare-associated infections. Known as the HAI Portal, it was created jointly by the Oakbrook Terrace, IL-based accreditation organization and Joint Commission International, Joint Commission Resources, and the Joint Commission Center for Transforming Healthcare.

"Healthcare-associated infections are harmful for patients, costly for healthcare organizations, and largely preventable," Jerod Loeb, the Joint Commission's executive vice president of healthcare quality evaluation, said in a news release. "We hope that the easily accessible information on the HAI Portal will assist healthcare organizations, practitioners, and caregivers to prevent HAIs in their organizations, practices, and homes."

PRACTICE MANAGEMENT TIP OF THE DAY

Record Performance

Leave no doubt when you decide to terminate an employee. Long before that day, you should have created a concrete record of problems and how you notified the employee. If you don’t, you may be struggling to explain yourself in court. Guarantee that your files can stand up to scrutiny:

  •  Start early. As soon as you notice a performance problem, make note.
  •  Be specific. Record the details, including any policies the employee violates. Avoid generalities such as “Always” and “Never.” Example:“Mary violated the attendance policy by arriving 20 minutes late on March 13 without calling in. She said that she overslept.” State the facts, without any commentary or speculation such as “She’s lazy.”
  •  Show you followed up. Include documentation of any conversations with and written notices delivered to the employee. Ask the employee to sign and date any documents you place in the personnel file. If the employee refuses to sign, note that.

Query: eRx Hardship Exception Status
 
We applied for hardship exemption status on the eRx in 2011, and because we used the group NPI and not the individual NPI, it was denied. Nothing can be done until CMS decides to re-look at the decision to accept the usage of the group NPI rather than just the individual NPI. Has anyone else had this experience? Will the same thing happen with the incentive for "meaningful use"?

Responses: I experienced the same issue. I actually received a letter stating the hardship exemption was received and accepted. I was later told this was a form letter response to all physicians who filed using the Group NPI vs. the Individual NPI (since the Group was technically excluded). I've been assured that CMS is taking a serious look into this issue. 
 
I did receive an email from CMS last week requesting additional information (i.e., Physician's Individual NPI number and Tax ID number). I'm hopeful this is a positive step toward Medicare review.

Try Whitfield's ointment. Formulations of varying strength are available. This is old treatment, but it can help in situations like this.

Is it really tinea? Step one is to do a biopsy of the skin to find out exactly what this is. The specimen should be sent to a dermatopathologist well-versed in skin disorders. It is too difficult to work through treatment regimens for the various possible conditions this may represent. However, if you are hesitant to perform a punch biopsy, consider the possibility that some conditions, like psoriasis, are made worse by instituting oral steroids.
 
There seems to be far too many posts asking about dermatologic conditions that have not had a simple biopsy performed. Not only are there little to no complications with small biopsies, but one gets paid to do them, and the patient and doctor are on the immediate path to effective treatment.

CMS has made the new list of approved L1906 braces (Ankle foot orthosis, multi-ligamentus ankle support, pre-fabricated, includes fitting and adjustment). This list as of 4/2/12 is: CFPRO, Deroyal Functional Ankle, Flex-Lite Sport Articulating Hinged Ankle, Judah Armor, Revolve Hinged Ankle, Swede-O Arch Lok, Swede-O Atom, Swede-O Dorsi Assist, Trailblazer Hinged Ankle, and Vertaloc. To limit liability when billing Medicare, I recommend that physicians check their current braces to ensure they are compliant.

I use the newest Dragon Medical Small Practice Edition (Dragon 11) with Traknet. As a Dragon user since 2002, I have seen major improvements in its accuracy. Dragon Medical also has podiatry and orthopedic surgery vocabularies built in. However, ONLY Dragon Medical 10 or Medical 11 will integrate with EMR, to my knowledge; and not all EMRs are compatible with even the medical version of Dragon. Consult your EMR software vendor for a serious voice recognition web demo AND refer to Dragon gurus such as Knowbrainer.com or Dr. Larry Kosova.

I have been with Practice Partners since 1988, and until McKesson bought them, they were great. However, since McKesson bought them, they are worse than terrible. Their program is full of bugs, and when they try to fix one, five more pop up. We have been down for extended times and frequently our insurance claims get kicked out. Their support, once excellent, is non-existent. It is usual to be on hold for 2-3 hours just to get in touch with someone. The last we heard, their entire Atlanta offce quit, and the new people have no clue as to what to do about the problems. We are in the process of changing EMR programs, and trust me when I tell you, it takes a lot to make me go through that, but I have no choice. Stay away from them.

I can only mention the method I use without comparing tablets. I use an iPad with the GoToMyPC app. That connects to my office computer desktop. Once there, I can access EMR or my radiograph program which is OnyxRad. The process can be slow, but overall, it works pretty well.

1. I was treated professionally and with courtesy.
2. I was given complete instructions on the proper use, care, and maintenance of my device(s), including any wearing schedules and pamphlets that may be appropriate.
3. My devices, shoes, and/or modifications meet my expectations.
4. I was told to re-contact Eneslow if there is a problem with the fit or function of my device.
