IL Podiatrist Discusses Bunion Surgery
If pain from a bunion is not impacting your daily life, getting wider shoes or using a splint may do the trick. When the discomfort becomes intolerable, a doctor will discuss surgery. “We make a cut in that bone, bring it over back into alignment and put some screws in,” Marc Leonard, DPM explains. “Then we protect the area during post-operative course.”
|
|
Dr. Marc Leonard |
The surgery is outpatient, and you typically can walk out of the doctor’s office. But beyond daily household activities, you should be off your feet for a month. “We find the more people rest and elevate the foot, the better they feel. There’s less swelling and less potential for complications,” Dr. Leonard says.

Ecletic NY Podiatrist is Founder of Bellantz Face Fitness
“If your body can look fit, so can your face,” says Naeemah Ruffin, DPM, former assistant clinical professor at the Mount Sinai Hospital in New York City and founder of Bellantz. Ruffin’s career began as a CPA at Ernst & Young, followed by 11 years at AMEX. Although she was grateful for her business and management experience, she did not feel this was her life’s purpose. So, after a medical mission trip to Africa, she left her corporate career at age 37 to pursue a career in medicine and public health.
|
|
Dr. Naeemah Ruffin |
She completed a dual DPM/MPH program at the New York College of Podiatric Medicine and the Icahn School of Medicine at Mount Sinai in New York. In addition, she completed her surgical residency at Mount Sinai Hospital and later completed a podiatric mini-fellowship in dermatology and pathology.

UK Podiatrist Discusses Subungual Dark Spots
According to Emma McConnachie, a podiatrist and spokesperson for the Royal College of Podiatry, dark vertical bands on the toenail or dark spots under the nail could be a sign of pigment deposits, trauma, or skin cancer.
|
|
Emma McConnachie |
If a dark brown or black discolored straight vertical stripe appears on the nail, this could indicate melanonychia — which can be caused by injury or illness. "The darker your skin tone, the higher chance you have of developing dark stripes in your nails. These will usually be seen in the 5th and 1st nails but can appear in all," she said. Dark stripes could also be a sign of melanoma — a type of skin cancer that develops from skin cells called melanocytes — McConnachie warned.

| PRACTICE MANAGEMENT TIP OF THE DAY | |
Student Loan Concepts to Know During Tax Season
With tax season approaching, many people are focused on gathering all the tax forms to make sure they don’t miss the April deadline; however, few will pause to inquire how their tax filing could impact their student loan payment. Fortunately, there are many ways that borrowers can get creative with income reporting come tax season that could impact their student loan payments substantially.
4. Married Borrowers
Another rule that we must acknowledge is that if you file your taxes jointly as a married couple, all the IDR plans will take your and your spouse’s joint income into consideration when calculating your payment even if you simply report off your paystub. That said, there are certain payment plans that will allow you to file your taxes separately to isolate your sole income for student loan reporting's sake. This means that they will look at your income alone when determining your discretionary income instead of looking at your and your spouse’s income jointly, which can save thousands of dollars annually in some instances.

| CODINGLINE CORNER |
Query: Aetna Reimbursements
Is Aetna allowed to deny payment for two procedures performed on the same day? I keep getting denials on the following combination of codes: CPT 11721-59 and CPT 11055 saying that “mutually exclusive procedures cannot be performed at the same day/time.” This does not make sense to me. Medicare covers these codes. So why does Aetna deny them? Do I have any recourse?
PM News Subscriber
Response: We have appealed all of the Aetna Medicare denials and have been paid. We’ve sent a cover letter, the MLN Matters discussing the use of the -59 / X-, as well as the office visit documentation. It takes staff time and payment is delayed around 3 months.

RE: America's Opioid Crisis: Doctors are Not to Blame
From: Allen Jacobs, DPM
With regard to prescribing opioids, I suggest reading this article.
It’s time to stop wasting our time and blaming doctors for this crisis.
Allen Jacobs, DPM, St. Louis, MO

| RESPONSES/COMMENTS (MEDICAL-LEGAL) | |
From: David Secord, DPM
I thought I’d comment upon people in our profession referring to an MD as practicing allopathic medicine and a DO as practicing osteopathic medicine and then putting ‘podiatric medicine’ in a separate category, as if podiatric medicine wasn’t allopathic medicine. There are a certain finite number of medical theories out there, including allopathic, osteopathic, homeopathic, chiropractic, native American Indian pan-theistic naturopathy, witch doctors, Eastern Indian Ayurvedic medicine and a few others.
Allopathic medicine has as its basis the idea of pathology from disease state: bacteria, virus, spirochete, genetic dyscrasia, prion, etc. Unless I missed something critical in medical school, that’s the disease model we in Podiatry follow as well. As such, podiatric medicine IS allopathic medicine. It is likely that the DEA doesn’t mention podiatry separately because it mentions the requirement for allopaths and that includes our profession.
David Secord, DPM, McAllen, TX

| RESPONSES/COMMENTS (APRIL FOOLS NEWS) | |
From: Robert Creighton, DPM
It seems like this year's April Fools post is another example -- among many in contemporary American culture -- of an attempt at parody turning out to mimic reality. As a longtime gym rat aware of the benefits of the "reverse sled pull" along with being an ACSM certified personal trainer, I am familiar with the literature investigating the effects of walking backward. A quick search of pub med confirms that walking backward, while not something done during activities of daily living (ADLs), appears to have a positive biomechanical role to play.

St. Louis Catholics Have Obtained St. Louis' Holy Toe
There's a new relic in town — and it may be the holiest digit to grace this city of sin. The relic, now in the possession of the Archdiocese of St. Louis, is purportedly a piece of toe once belonging to St. Louis IX, the French king for whom St. Louis, Missouri, is named.
|
|
Toe of King Louis IX |
The toe bone was recently gifted to the Archdiocese of St. Louis by the Cathedrale Monreale in Sicily. After all, King Louis IX was on the shores of Tunisia in an ill-fated crusade when he died of dysentery in 1270. After that, Louis' body was dismembered.
