CA Podiatrist Delivers Oration on Diabetic Foot Remission

Noting that diabetic foot complications are common and complex, Dr. David Armstrong, Professor of Surgery and Director, Southwestern Academic Limb Salvage Alliance, Keck School of Medicine, said that every 1.2 seconds, someone develops a diabetic foot ulcer, and every 20 seconds, someone with diabetes undergoes an amputation.

PODIATRISTS AND THE VA
Defective Diabetic Shoes Leads to Amputation of Veteran's Foot

An elderly veteran lost his foot due to deficient podiatry care. The whole sorry mess involved defective specialty diabetic shoes, blocked blood flow to the leg, a patient fall due to the shoes, lack of patient education, lack of communication among staff … and ended with amputation of the patient’s foot.

There was enough blame to go around, but nobody to throw in jail. The podiatrist didn’t adequately instruct on the proper fitting of the shoes. The fall wasn’t correctly reported because the safety officer was new. It could not be determined who did/didn’t warn the patient about possible amputation. And so on.

PRACTICE MANAGEMENT TIP OF THE DAY
6 Steps to Starting an RPM Program

Here are six steps to get you started with a remote patient monitoring (RPM) program:

3. Devices, a team and a leader

Go online to study devices and codes before working with a treatment partner, said Marc Gruner, also a co-founder and chief medical officer of Limber Health, which offers online physical therapy.

Get your compliance and billing staff talking about how to implement the program. “Introducing new technology into a busy primary care doctor’s office, a specialist’s office, is not an easy feat,” Gruner said. Identify an internal client success leader to champion RM because they believe in it and the benefits to patients.

QUERIES (MEDICAL-LEGAL)
Query: How Long to Retain Medicare Charts

Being retired, I want to try to condense as many of my charts as possible. As far as Medicare is concerned, how many years do I have to retain my charts before I can dispose of them?

RESPONSES/COMMENTS (NON-CLINICAL)
From: Kathleen Neuhoff, DPM

I accepted Dr. Tomczak’s challenge and answered all of the questions in Part Two correctly. I think I answered all in Part One correctly also but did not see the answer for one so I am not sure…all others were correct. It does not seem that these questions would be too difficult, especially if students received a board passing course which is pretty standard for most professions requiring passage of an exam for licensure.

I am one who is not in favor of an MD degree as I was not in favor of lengthening the podiatry curriculum or requiring everyone to do a surgical residency. I believe these things are eliminating the skills which made us unique and channeling our potential students into PA schools. However, I think it is a disservice to our students and schools to say that the inability to pass the test is the reason not to proceed.
