CA Podiatrist Discusses the Epidemic of Ankle Sprains
"Ankle sprains are an epidemic problem, and the numbers of sprains are astounding. It is estimated that there are over 3 million ankle sprains reported to doctors annually in the United States," says Bob Baravarian, DPM. "The problem is, for every ankle sprain that is reported, at least 10 are treated at home without proper care. Sprains that aren’t properly cared for create a great deal of short-term and long-term complications, many of which are only correctable with surgery. And that you want to avoid that at all costs."

FL Podiatrist Elected to FAPM BOD
Richard Polisner, DPM, MJ has been elected to the Board of the Florida Academy of Professional Mediators (FAPM). The Florida Supreme Court certified Dr. Polisner as both a Family and Circuit Civil Mediator in 2013. From 1971 until 2010, he was an ABFAS certified foot surgeon practicing in the Chicago suburbs and in Jacksonville, FL. He earned a Masters degree in health law from Loyola Law School in 1991 and participated in the inception of three podiatric medical malpractice insurance companies and lectured extensively on risk management from 1979 until 1999.

Concurrently, he had the opportunity to review hundreds of podiatric malpractice cases for both plaintiff and defense attorneys. Most recently, he served as COO of Acacia Designs, AB, developing a unique anesthesia device to objectively monitor the reversal of paralytic agents during major surgery. The merged company recently obtained FDA-approval for the monitor and sensors.

Health Conversations: Six Ways to Engage Patients - Part 6
A growing body of research suggests that people who are actively involved in their health and healthcare tend to have better outcomes. Here are six practical suggestions for how to enhance your patients’ engagement in your conversations about their healthcare.
6. Offer Options
When it comes to encouraging health changes, especially for complex behaviors such as eating or physical activity, avoid the “Righting Reflex.” This is the common impulse to want to “make things right” when we see a problem. But instead of telling the patient what to do, try to engage them in the decision by asking for their ideas. If the patient does not have any ideas, offer two or three options and let him or her decide which seems like the best fit. Once a decision has been made, summarize the plan and verify that the patient feels confident in their ability to follow through with it.

Query: Hospice Status
Is there any way to find out when a patient is on hospice care except by asking them directly? We are constantly running into this problem when doing housecalls. In many instances, patients are being switched to hospice care, but we have no way of knowing until later when services are denied and must be rebilled with a GW modifier. We are checking eligibility before each visit with software provided by Trizetto, but can not find if there are any indications of hospice care.

Response: If you sign up for e-services with you local Medicare carrier, you can find this information. I’m not sure why Trizetto doesn’t include this in the information that you get from them. If you look up each patient yourself, it might take more time, but then you will have a clean claim on the first try. Each MAC is different, but you should be able to find help either online or by calling them.

Query: Sanexas Neuropathy Treatment
I just heard from a patient about a clinic in my area that is using a new neuropathy treatment called Sanexas. This treatment uses "electric cell signaling-therapeutic pulsed energy waves that are comfortably and non-invasively delivered directly into the body's desired anatomical region". According to the clinic brochure and the company website, patients require 3 treatments a week for 3-4 months and that "Medicare and most major PPO insurance plans typically reimburse up to 24-36 treatments". They also claim it will increase circulation.
Is anyone familiar with Sanexas? If so, what results have you seen? If it is in fact covered by Medicare and most insurance plans, then what code is being used for this and what is typical reimbursement?

RE: Are There Mobile Applications Related to Nail Disorders?

A study of iOS (Apple’s iPhone operating system) and Android apps was done to determine the types of nail health apps available for patient education. A standard app analytics and market data tool was used to search and analyze iOS and Android nail apps for several nail health keywords. The availability of apps related to nail disorders has not been explored. This study investigated iOS and Android apps, using psoriasis and hair loss apps as comparator groups.
A standard app analytics and market data tool (App Annie; appannie.com/en/) was utilized to search for iOS and Android nail mobile apps. The analysis was performed on a single day (March 23, 2020), given that app searches can change on a daily basis. The search included the following keywords: nail, nail health, toenail fungus, nail tumor, brittle nails, onychomycosis, onycholysis, subungual melanoma, nail melanoma, paronychia, and nail squamous cell carcinoma. The only search terms that returned specific nail apps were onychomycosis and toenail fungus. Neither of the 2 medical apps for onychomycosis and toenail fungus referenced a physician involved in the app development. (Ishack S and Lipner SR, Cutis 2021 March;107(03):153-156.

RE: Neuroma Symptoms In the 2nd Innerspace

Throughout the years, I have noticed that almost every patient who exhibits neuroma symptoms in the second innerspace has 2nd and 3rd toes of the same length, which usually means the 2nd and 3rd metatarsals are of the same length. MRIs are usually negative for an actual neuroma, and I explain this to my patients as a juxtaposition of the two metatarsal heads causing “rubbing” on the nerve. If conservative therapy fails, I perform an osteotomy of the 3rd metatarsal using minimally invasive techniques. The results are immediate and dramatic as far as relief of symptoms.
In my mind, this not only decompresses the nerve but restores the normal metatarsal parabola with the head of the 3rd now behind the head of the 2nd, leaving room for the nerve to pass. Using an MIS metatarsal osteotomy, the patient is off his/her feet for only one day and back in shoes within two weeks with additional home taping for three weeks.

I, too use alcohol injections and find them highly successful. I use 4 percent and, like Dr. Meisler, inject into the region of the neuroma. I have similar results, however, with 4 percent not 10, and without any ultrasound. If we suspect the neuroma is being squeezed between the met heads and under the DTML, then that is a pretty easy location to find without ultrasound. (I have no issue with those who use it and those who bill for using it- good for you). When I was taught the technique by Dr. Dockery - he taught to inject proximal to the neuroma. I recall him giving more injections than the three I tend to, and the difference may be injecting into the nerve vs proximal to it as opposed to the difference between the percentage 4 vs 10.
Finally, the question of what code to use to bill for it comes up often. Some argue it isn’t truly sclerosis, etc. I really wish when issues like this come up, the profession would give a clear cut answer. Due to the success of the procedure, if it isn’t a covered code as one doctor likes to bring up on this forum... then good; we should all charge cash. The lack of consensus on issues like this is unnecessary and the profession should be more definitive with consensus.
