Bunion Pain is Often Complex: MN Podiatrist
"When questioned, most people would assume a bunion is painful only when there is pressure to the bump from shoes," says Conway McLean, DPM. "Predictably, it is much more complex than that, with surprising ramifications. Although there are exceptions, bunions can become a source of numerous, and very different, problems. Certainly, an abnormal bump on some part of the foot is probably going to get pressure from shoes. Yet, this recurrent and excessive force on human tissue has consequences.

"A sack of fluid can develop over a bunion, a structure referred to as a bursa, labeled bursitis after becoming inflamed. These structures develop in a variety of places throughout the body, the hip being an especially frequent one. Typically, a nerve runs right past this area of the foot, so it often gets pressured when a bunion exists. Any nerve getting excessive, repeated pressure will induce pain, so this peculiar type of discomfort is common," says Dr. McLean.

Better Podiatry Services Can Avoid Amputations: New Zealand Podiatrist
Around 600 amputations could be avoided in New Zealand each year if better foot screening and podiatry services were made available for everyone with type 2 diabetes, a recent report shows. The report found that millions of dollars are spent in the public hospital system each year on diabetes-related amputations, which are often preventable.

“New Zealand needs to more than double the current number of podiatrists if it is going to meet current and future demand,” says Jennifer Pelvin, PodiatryNZ CEO. “Without regular podiatric care, people with diabetes can find themselves candidates for amputations when their physical condition deteriorates. Falls, ulcerations, and infections can rapidly become life or limb threatening conditions.”

‘I’M NOT A FOOT,’ SAYS MO PODIATRIST
Many have heard of the civil case hearing where lawyer Rod Ponton was unable to figure out how to turn off the cat filter on his Zoom call during a hearing in Texas District Court. In the newest Zoom video to go viral, Allen Jacobs, DPM showed up in court virtually as a defendant’s expert witness, but was unable to turn off a filter he had enabled that transformed a person’s face into a foot. “I don’t know how to remove it,” he said. “I’ve got my assistant here and she’s trying to.”

To get the hearing moving, Dr. Jacobs offered: “I’m prepared to go forward with it.” Then, crucially, he clarified: “I’m here live. I’m not a foot.” This caused Mr. Phillips to look up and, finally, the exchange drew a smile and a laugh from him as Judge Ferguson responded: “I can see that.”

5. Ask about any barriers that might exist to following your advice
Giving advice without asking patients about possible barriers can make them feel you do not understand or care about their circumstances, needs or concerns, and thus cannot really help. At that point, they are more likely to discount any information or advice you provide. Let them know you sincerely care about any hurdles they may face in following your advice and develop strategies with the patient to overcome his or her barriers.

RE: Impaired Glucose Tolerance and Neuropathy

Diabetes is the most common cause of peripheral neuropathy, accounting for approximately half of cases, but up to 1/3rd of neuropathy patients have no identifiable etiology. Among this population, impaired glucose tolerance (IGT or "pre-diabetes") is observed in approximately 40%. However, an internationally agreed definition as to what constitutes pre-diabetic neuropathy is required. Evidence suggests IGT causes neuropathy which may occur early in diabetes. The neuropathy associated with IGT is clinically similar to early diabetic neuropathy, with preferential injury to small nerve fibers, resulting in pain and autonomic dysfunction. IGT and diabetic neuropathy patients share abnormal microvascular endothelial dysfunction.
IGT subjects treated with diet and exercise have reduced risk of progression to diabetes, and those with neuropathy experience a short-term improvement in small fiber function with sustained benefit for pain. (Smith GA and Singleton JR, Neurologist, 2008 Jan;14(1):23-9. In the United States, 88 million adults—more than 1 in 3—have pre-diabetes, but 84% of them don’t know they have it. With pre-diabetes, blood sugar levels are higher than normal, but not high enough yet to be diagnosed as type 2 diabetes. However, pre-diabetes raises the risk for type 2 diabetes. (CDC June 11, 2020).

RE: OSHA Requirement for Laser Surgery

CO2 laser therapy is becoming more available and affordable for in-office use. I have owned one for many years. It is a great marketing tool and worth the money if you need a marketing boost. As far as billing for actual procedures, you will never make your money back. One thing you should keep in mind when using a laser on warts is that there is a fear that the wart virus could be found in the plume or smoke from the wart destruction. If you have office or hospital staff assist you during the procedure, OSHA requires that you provide them with special masks made for laser surgery. You can't just wear a surgical mask because they are worthless against viruses.

RE: New Foot Implant Might Revolutionize the NFL Kicking Game

An Israeli engineer has developed a foot implant that could revolutionize the college and NFL kicking game. The implant, the HyproKick, is a convex titanium device attached to a screw that is inserted into the dorsum of the navicular. Originally developed for soccer, the CEO, a former soccer player himself, stated that he got the idea while visiting the U.S. and watching college baseball players hit with metal bats.
The procedure involves a dorsal incision just medial to the navicular away from the contact point. A hole is drilled into the navicular from dorsal to plantar. A sizer (1-5) is then used, and the permanent implant, depending on the size and shape of the navicular is then inserted. Preliminary results have been dramatic with increase in kicking distances ranging from 15-20%.
One college kicker who volunteered for the trial, stated that “the HyproKick has made me think about an NFL career”, and he further stated that he has been routinely kicking field goals of 70-75 yards with his previous longest being 51 yards. He has been punting the ball upwards of 80 yards. A spring-loaded version is in the trial stages with possible release date in early 2023. Of course, the legality of the HyproKick might be questioned by the NCAA and NFL, but it can only be detected by x-ray and clinically could be mistaken for a dorsal exostosis. A happy April Fool’s Day to all my podiatric colleagues.
