When Your Feet Hurt, Your Face Shows it: PA Podiatrist
 
When your feet hurt, everything hurts and your face shows it. According to Kenneth Kilgore, DPM, foot and ankle specialist at Lititz Podiatry at the Warwick Center, heel pain can be caused by ill-fitting or worn out footwear, overexertion, outright trauma, and biomechanical problems that involve the alignment of the foot with the leg.

In the early stages, conservative non-invasive methods often suffice. However, many patients go untreated for many months or even years before seeking professional help and may require more involved means to help resolve the condition. "The longer one waits, the more difficult it is to successfully treat. The best treatment protocol for any ailment should be tailored to the patient’s specific needs, taking into consideration the causes, the daily activities of the patient, and potential downtime required for certain treatments," says Dr. Kilgore.

Aussie Podiatrists Create World's First 3D-Printed Feet
 
It’s gory, sticky, and undoubtedly on the nose, but a blend of icing sugar, chicken stock, and flexible resin is proving to be the just the right recipe for creating realistic foot ulcers as part of a world-first podiatric training initiative at the University of South Australia (UniSA)Concocted by Drs. Helen Banwell and Ryan Causby, the gooey mixture is being added to newly printed 3-dimensional feet and is designed to mimic infected and non-infected diabetic foot wounds.

The 3D printed feet are created with wound-like cavities in place. Made from thermoplastic polyurethane (a plastic with many properties, including elasticity, transparency, and resistance to oil, grease, and abrasion), each takes a week to print, and costs less than $4 to produce. The addition of life-like ulcers and effects is added by the creative hands of UniSA’s podiatry team and can encompass anything from dry gangrene to oozing pus.

Aussie Podiatrist Creates Orthotic for Alan the Wombat
 
A Queensland podiatrist and a prosthetist have branched out from their usual profession of treating humans to help create an orthotic for an injured wombat. Rockhampton Zoo staff approached a local podiatrist about an ulcer on wombat 'Alan's' back leg. Fate, it would seem, led them to a podiatrist of the same name, Allan Harwood, who said he was instantly keen to see what he could do for the furry marsupial. "Alan was a wombat that the zoo picked up quite some years ago," Harwood said. "He was hit by a car, which left him with an injury to his right rear foot, which left it misshapen."

"Standing on that foot, there was a pressure point that kept breaking down. While his health was good, in his latter years, he needed some extra help and the question was asked whether the podiatrist could do anything to help this spot and help heal the ulcer." Harwood said they attempted to take a plaster cast of Alan's foot but he didn't enjoy sitting still for too long. Instead, they managed to take a print of his paw using foam, which they were then able to derive measurements from.

FL Podiatrist to Host Musical Cable TV Show About Feet

Foot doctors are making it on TV. The popular A&E Series Dr. Toe Bro features Canadian chiropodist Jonathan Tomines. TLC recently premiered a new series. My Feet are Killing Me, featuring Brad Schaeffer, DPM and Ebonie Vincent, DPM. Now Alan Sherman, DPM, CEO of PRESENT e-Learning Systems which focuses on podiatric post-graduate education, has added another notch to his belt…this time hosting a new series intended to inform the public called Musical Feets.

Bravo has announced that Sherman, a podiatrist who gained fame before becoming a podiatrist with his campy song Hello Muddah, Hello Fadduh!  will have podiatric residents present songs about podiatric issues with selections such as Wart’s Up?My Feet Stink, and There’s a Fungus Among Us.

When was the last time you reviewed your partnership agreement? Even if you are not currently contemplating new shareholders or partners, if you haven’t looked at your agreement in the last three or more years, now may be the time. From the perspective of business operations, let’s break out some of the key questions that you should consider in every partnership agreement:
 
2. Division of duties: Will all partners carry equal responsibility and be expected to dedicate equal time to administrative and clinical duties? Or is there one 'managing partner,' or even a Board of Directors? If so, how are those partners compensated differently?

Query: Midfoot Exostectomies
 
A patient with previous triple arthrodesis had a bony prominence on the dorsal medial side of the foot with a history of ulcerations and another prominence on the plantar medial aspect of the same foot. I did a 3:1 excision of the scar tissue on the dorsal medial side of the foot and an excision of the underlying bony prominence. I made a separate incision plantar-medially to remove the other prominence. Would you code CPT 28122, Partial excision tarsal or metatarsal bone except talus or calcaneus twice? Or would they just reject the second procedure? Would you code this as a CPT 28122 and CPT 28122-51?

Response: I agree that CPT 28122 is appropriate for both of the procedures you described. I suggest coding this on one line with CPT 28122 X 2 units.

In the fall of 1980, while attending orientation at PCPM, this same discussion arose under the guise of a degree "name change". I recall several incoming first year students pushing for PMD. One showed his character by going so far as to suggest we could put a pen in our breast pockets that hid the monogrammed P so it would look like MD.  
 
While our training has evolved dramatically it is not, no matter what anyone says, global or detailed enough to be equivalent to that of an MD or DO. Are there not podiatrists who rail against orthopedists who treat the foot? They are licensed to do so yet some DPMs argue they are not competent. This is not about intellect, but about training and competence.

Allen Jacobs, DPM makes reasonable points regarding podiatric medical school education. I’m sure the education in medical school is more comprehensive, as is their training, compared to podiatric medical school. Where I take issue with his position is the pride, excellence, and credibility podiatrists should exercise, not legislate our way to another degree. I don’t think that’s the core problem. The core problem is the disparity in reimbursements for the same Dx and Tx, and how institutions deal with podiatrists. Yes, there are prominent extraverted podiatrists that are treated as equals by MDs and DOs, but that’s not the primary concern. I’m certain that even the “prominent” podiatrists have confronted professional discrimination by doctors’ insurance companies, hospitals, and other institutions.
