TX Podiatrist Discusses Walking Shoes for Seniors
For people who walk frequently, choosing the right walking shoe is super important to prevent your feet from aching and developing or worsening painful foot conditions. But wearing the proper supportive footwear is especially essential for older adults, said Chanel J. Perkins, DPM, a board-certified podiatrist based in Texas. “Supportive shoes help maintain balance and alignment, which are necessities in this population since they are at a greater fall risk,” she told HuffPost.

“They also help decrease pedal muscle fatigue and overall soreness since biomechanics forces are evenly distributed throughout the shoe.” Dr. Perkins described the best shoes for older people as ones that are lightweight and breathable to prevent sweaty feet. They should also be well-cushioned, have good traction on the soles to prevent slips and falls and be easy to get on and off.

Survey Reveals The Current Cost of Foot Pain in America
The survey data presented in this report is based on a sample of 6,030 American adults, aged 18 years and older, who reported experiencing foot pain within the last 12 months (January–December 2022). The survey has been conducted with a margin of error of +/- 1.66% and a confidence level of 99%. These results provide a statistically significant representation of the population surveyed.
The study revealed that 8 in 10 adults (83%) with foot pain spend an average of $228 each month to treat their pain. These expenses include podiatrist visits, treatments, medications, orthotics, and more. 51% of adults spend $100 per month ($1,200 per year), while 1 in 10 adults spend $250 or more per month ($3k per year) on foot pain-related expenses.

Student Loan Concepts to Know During Tax Season
With tax season approaching, many people are focused on gathering all the tax forms to make sure they don’t miss the April deadline; however, few will pause to inquire how their tax filing could impact their student loan payment. Fortunately, there are many ways that borrowers can get creative with income reporting come tax season that could impact their student loan payments substantially.
2. The Payment Calculation - The monthly student loan payments are a percentage of what’s called your discretionary income. For most of these plans, your discretionary income is classified as the amount by which your income exceeds 150% of the poverty guideline for your family size. Your annual payment will be 10% of your discretionary income amount in most repayment plans. This calculation does go off the Federal poverty lines and so yes, leave it to the Department of Education to make the amount of discretionary income for someone in New York City the same as someone living in Albuquerque. Regardless, this is how they derive your payment amount and a key component to understanding the reporting strategies.

Query: Toe Tumor Surgery
I have a patient who presented with clinical and x-rays findings of a subungual osteochondroma of the distal phalanx of the great toe. I am scheduling a patient for surgery. Could I bill a CPT 28124 for excision of the growth and a CPT 20220 as I will be sending it for biopsy?

Response: If you are removing a bone tumor, usually you are coding it as CPT 28108-Excision or curettage of bone cyst or benign tumor, phalanges of foot. CPT 28124 is used when you are removing bone only and part of the phalanx for example an exostosis, enlarged bone or osteomyelitic bone. If this tumor is more than you expect and could be cancerous or more involved, think of CPT 28175 Radical resection of tumor; phalanx of toe.
As for CPT 20220-Biopsy, bone, trocar, or needle; superficial (e.g., ilium, sternum, spinous process, ribs), this is reserved usually for a fine needle bone biopsy when you are treating a potential cancerous problem and from what you described, it does not seem that you would be performing CPT 20220. This answers your question.

In response to Dr. Hultman’s statement “…maybe we should form a new board that covers podiatric medicine and surgery…” I submit the following, as I have done over the past years on this forum. There has been a board that does just that and more, and it is called the American Board of Multiple Specialties in Podiatry (ABMSP). This board, unlike all of our current self-certifying boards, is accredited by both the International Organization for Standardization (ISO) and the American National Standards Institute (ANSI).
This board is superior in all metrics compared to what we have. Because of the politics they are up against, they have been sidelined. This perfectly illustrates what this profession is up against. Our profession suffers at the hands of the few in power who do not wish to relinquish it.
