HCA Florida Northwest Hospital has announced the appointment of Alan MacGill, DPM, to its Board of Trustees (BOT). Dr. MacGill is a board-certified podiatric surgeon specializing in foot surgery as well as reconstructive rearfoot and ankle surgery.

MN Podiatrist Named American Legion's 2021 Physician of the Year
 
A St. Cloud VA Medical Center podiatrist was named the American Legion's 2021 Physician of the Year. Joel Syvrud, DPM has been a podiatrist at the St. Cloud VA for 22 years. Some members of the hospital nursing team wrote up the award nomination, which Director of Surgery and Special Care Dr. Alfonso Alarcon said he endorsed as a supervisor.

NY Podiatry Practice Beats Whistleblower’s Fraud Claims
 
A Batavia, NY-based podiatry practice beat False Claims Act allegations that it sought payments from Medicare and Medicaid despite noncompliance with medication safety standards, a New York district court said. George R. Vito, a podiatrist who formerly worked at Advanced Podiatry Associates, didn’t identify any particular false claims that were submitted to the government, and didn’t allege facts suggesting that alleged schemes resulted in the submission of false claims, Judge Lawrence J. Vilardo of the U.S. District Court for the Western District of New York said in a Tuesday opinion.
 
The court also said Vito failed to adequately raise an FCA retaliation claim because he didn’t allege he was terminated because of complaints about allegedly improper medication use. Instead, Vito claimed he was fired for objecting to podiatrist owner Joseph Canzoneri’s alleged request that Vito pay $250 per surgical procedure, the court said. Vito has 30 days to correct deficiencies in an amended complaint, the court said.

RE: The Passing of Kevan Kreitman, DPM
 
It is with profound sadness that APMA shares the news that Kevan Kreitman, DPM, speaker of the APMA House of Delegates, passed away suddenly this week. Dr. Kreitman earned his DPM from the then Pennsylvania College of Podiatric Medicine. He served as chief resident during his surgical residency at North Detroit General Hospital. Dr. Kreitman was a diplomate of the American Board of Foot and Ankle Surgery.

He was a partner with Shores Podiatry Associates in Roseville, MI, for 20 years and was a leader of the Michigan Podiatric Medical Association, serving as president from 2002–2003. He was active with APMA and the APMAPAC and served as speaker of the House of Delegates for the first time in 2022.

I was saddened to hear of the passing of my dear friend David Cavallaro of Oklahoma City. David was a residency director who mentored a number of young colleagues. He often served on mission trips to Central America. He followed me as President of the Southwest Region of the ACFAS.

The original query was "Has anyone treated osteo of a toe with an antibiotic bead with a K-wire to help as spacer?" I have treated osteo without amputation with antibiotic beads numerous times. I never used a K-wire in any of these cases. I always use the resorbable beads. If it’s a distal phalanx, I’m not sure I understand the necessity of the wire. I certainly would not want that wire going through an uninfected phalanx. 
 
Also, I think the advantage of beads vs. powder is the beads will give a sustained release over time vs. powder that will work and dissolve much more rapidly. I agree that osteo does not always need amputation. I believe amputations become more of a necessity when the surrounding soft tissue is grossly infected, i.e. “wet”, or there is gas present which mandates excision so as not to cause more proximal spread or destruction.

When I was in practice, I used the standard toe block as previously described. After the wait time if the toe was still sensitive, I would have the patient hang their foot over the side of the chair for five minutes. I would re-enter the room and 95% of the time, the toe was numb. Maybe it was the additional time or gravity playing a part. For the other 5%, I re-injected and used ethyl chloride anesthesia spray.

With reference to bunion surgery, my old professor Dr. James Ganley used to query, "how many bad results can you afford to be limping about your town?" Frontal plane correction can be obtained with any planar osteotomy (e.g. Homann, Wilson). Predictors of recurrence have been shown to include post-operative sessions positioning, residual hallux valgus, residual IM angle. A multitude of other factors such as degree of pronation, metatarsus adduced, ligamentous laxity are obviously significant. However, there is a clear difference between recurrent or residual hallux valgus and patient satisfaction. Distal metaphyseal osteotomies and diaphyseal osteotomies have very high patient satisfaction rates even with some degree of residual or recurrent deformity. 
 
There is a difference between the theoretical and the practical. If procedures such as the Austin-type bunionectomy are "doomed to failure", why is this the most commonly performed bunionectomy? Are we all stupid, non-observant, or uncaring? Or is this yet one more example of corporations and their spokespeople attempting to define the standard of care. Furthermore, we see trending MIS surgical bunion correction. How do we reconcile this reversal of the Lapidus philosophically? Again, as Dr. Ganley told us, “beware of the technological imperative!”

To really wet the area with 3-WEY, use soft roll - it saturates very well and clings to the callused area well, thus providing almost instant softening. It’s much better than gauze.

I, too have found 3-WEA to be insufficient in softening keratoses. So my way of dealing with this is two-fold:
1) I use the Personna Plus/Southmedic blades exclusively on these nails/lesions. They are about 80 cents each, but I've tried every other brand. I simply use up three or four of the cheaper blades for every one of the premium type and only the first few swipes of the cheaper blades are useful. 
2) I use a cotton ball soaked in 92% alcohol and I am constantly daubing the callus or nail with the soaked cotton ball with my other hand. It takes a while to get this sequence down, but it becomes second nature in a short time
 
This strength of alcohol softens the keratosis enough to allow the blade/burr/clipper/disc utilized to work easier, as well as cooling the friction developed in reducing the lesion/nail. Thinking about it now, I might try the same technique with a cotton ball soaked in 3-WEA. Let's see what happens.

Dr. Malusky mentioned wonders where he got the idea to use 1% solutions of phenol to soften nails and calluses. I don't know where the original idea came from, but I can report that this solution was being used in the clinics of the then Illinois College of Chiropody and Foot Surgery (later changed to Illinois College of Podiatric Medicine, and then to the Scholl College of Podiatric Medicine) when I matriculated there in 1954. 
 
In fact, at that time, that phenol solution was used for the "sterilizing" of instruments used for routine foot care in between patients. Readers may also be interested in knowing that before a sophomore student could advance to junior status, he/she would have to have treated at least 400 routine foot care patients. At that time, though, routine foot care was called, "palliative foot care" in the clinic.

I had a patient present with symptoms of a stress fracture. Radiographs were negative, but I explained that stress fractures can take up to 2 weeks to show on x-rays. She then asked if I would call her if a fracture shows up on the x-ray in the next couple of weeks!
