OH Podiatrist Gives Shots to Promote Vaccinations
 
A Republican congressman gave COVID-19 vaccinations Sunday to promote efforts to get reluctant people to get the shots. An early advocate of getting vaccinated, Rep. Brad Wenstrup, DPM is among Republicans concerned about their voters getting inoculated because of unfounded claims about the vaccines.

“A lot of it is distrust in the government,” said Dr. Wenstrup a podiatrist who served as an Iraq War combat surgeon. “Everyone is going to have a different reason, I think.” “I think it’s mixed messages that people have been getting. Get the message from someone you trust, I think is the most important thing,” he said. He has given shots at several clinics in his district, including twice in his role as an Army Reservist.

SD Podiatrist Discusses Ankle Repacement Surgery
 
Yes, ankle-joint replacement technology exists. That’s something Garrett Wobst, DPM, Avera Medical Group foot and ankle surgeon, more often will say to patients who face arthritic ankle pain. “In just the last 10 years, they have become more commonplace as the technique and the technology and materials have matured,” said Wobst. “Modern ones are long-lasting and give many patients a great alternative to ankle fusion surgery, as well as relief.”

Ankle replacement is an intricate approach to surgery. Still, once the 90-minute procedure is completed, patients may spend one or two nights in the hospital before beginning therapy and the journey back to full mobility. In most cases, they should avoid bearing weight on the ankle for about three to four weeks. “Ankle fusion patients are asked to be non-weight-bearing for eight weeks in most cases, so the contrast is striking,” said Dr. Wobst. “Among all the joints, the ankle is the one that gets a tremendous pounding onto a very small surface area. The surgery can be challenging, but for the right patient, it’s a cure for pain.”

TX Podiatrist Comments on Callus-Removing Booties
 
So, how do these callus-removing booties work? They’re essentially a chemical peel for your feet, with acids that strip away dead skin. However, Larissa Rolim, DPM, MS, a board certified podiatrist at Maverick Foot and Ankle Specialists in Irving, Texas, says that the glycolic, salicylic, or lactic acids that are often in these foot peels can potentially burn your skin.

“Many times, the acid percentage in these foot peels is not regulated or reported and can cause a chemical burn requiring medical attention,” Dr. Rolim says. Rolim and other health professionals we contacted say that diabetics should avoid foot peels altogether because high blood glucose levels affect the body’s ability to heal wounds.

Every year, the American College of Foot and Ankle Surgeons (ACFAS) recognizes its student club presidents from each of the podiatric medical school campuses for all the work they do throughout the school year on behalf of the organization. Each ACFAS Scholar receives a scholarship for $1,000 to defray their costs of travel and lodging to ACFAS 2021 in Las Vegas in May. They also receive complimentary registration. A growing body of research suggests that people who are actively involved in their health and healthcare tend to have better outcomes. Here are six practical suggestions for how to enhance your patients’ engagement in your conversations about their healthcare.
 
4. Learn to move slowly
 
If your patients are resisting a discussion, try simply offering additional resources on the topic. Let them know that, whenever they are ready to discuss the topic, the door is open. If they do give permission to discuss a sensitive topic, make sure to use motivational interviewing and ask open-ended questions. For example, you could ask, “How is your weight affecting your quality of life and your health?” or “How do you feel about your alcohol consumption?” Your tone should convey empathy and a genuine desire to help the patient.

Query: Coding for an Intermetatarsal Ligament Release
 
I have been having problems finding the proper code to use for an intermetatarsal ligament release to decompress a Morton’s neuroma. The codes I have used for this, CPT 64722 and CPT 64726, have both been rejected by insurance as invalid codes. What is the best code to use for this procedure?

Response: I have performed this procedure in the past and I have billed CPT code 64722 (Decompression unspecified nerve(s) (specify)) and CPT code 64726 (Decompression; plantar digital nerve) and have been reimbursed successfully using ICD-10-CM code G57.61 (Lesion of plantar nerve, right lower limb) or G57.62 (Lesion of plantar nerve, left lower limb) as indicated. Back in the day, the correct ICD-9 code to use was 355.6 (Lesion of plantar nerve).
 
There is no indication in the post which health insurance carrier or carriers were billed and rejected the claim(s) nor is there any indication what the ICD-10-CM code was that was appended to CPT codes 64722 and 64726. If the coding scenario was billed as I have indicated above and the claim was rejected, it is now time to put Plan B into action. Plan B has two CPT codes to bill. The first one is CPT code 28899 which is defined as: Unlisted procedure, foot or toes. The second one is CPT code 64999 which is defined as: Unlisted procedure, nervous system. When either of these CPT codes is billed, they would be appended by either the RT or the LT modifier as appropriate.
 
The ICD-10-CM code that would be linked to either of the CPT codes would be G57.61 or G57.62, depending upon the laterality of the procedure. The claim would have to be submitted hard copy along with a copy of the operative report. If you pinned me down, I would bill CPT 28899. After all, it is an intermetatarsal ligament that is being released to decompress the Morton’s neuroma.

I am watching this 800-pound gorilla rear its ugly head once again. Being retired (from a solo Florida practice), I am no longer aggravated by the pain of collecting Medicare deductibles. But I can certainly empathize with anyone who is still involved. I am curious as to how much of a problem this presents in other parts of the country.

Sounds like a good idea. My son is in the business world. He is involved in complex litigation. The lawyers have a huge six figure retainer before they do one bit of work. This is standard procedure for them, other than for the personal injury lawyers. Go to the dentist for procedures and try to leave without a healthy payment. We have been saying this same mantra for fifty years. Preparation work is hardly ever considered by the patient.

I could not disagree with you more regarding the size of the neuroma which is why I did not mention it. I have decompressed huge “neuromas” and the only difference is that the enlarged part of the neuroma is scar and fibrosis, which under loupe magnification is relatively easy to dissect off with proper instrumentation and technique. I see very little difference in the success rate. This is also similar to that of my colleagues in the AENS. 
 
I assume you have not decompressed bigger neuromas or at least not enough to make your opinion regarding bigger neuromas being doomed to failure. I always tell my patients that size does not matter when it comes to decompression. I have decompressed hundreds of Morton’s nerve entrapments and success is not dependent on size. Maybe it took a bit longer to resolve, but not much more than usual.

I answered steroid injection as my primary treatment for Morton's neuroma because it is my first treatment. By far, my most successful treatment is a series of 10% alcohol sclerosing injections. We have given many thousands of alcohol sclerosing injections under ultrasound guidance with significant success. The injection is given directly into the nerve, directed distal to proximal through the webspace. Over the years, we have very gradually increased the alcohol percentage from 4% to 10% and for the last the last 5-7 years, we have only used 10% alcohol solution. We have found 10% to be significantly more successful than 4% and have seen no change in adverse reactions as compared to traditional steroid or alcohol sclerosing injections. Usually 3-8 injections are required. 
 
We have a very high success rate with alcohol sclerosing injections with 10% alcohol. Extremely few patients require surgery. If you are comfortable injecting directly into the nerve, you can be comfortable knowing little steroid or alcohol is leaking into surrounding tissues.
