| ON THE INTERNATIONAL LECTURES CIRCUIT | |
MO Podiatrist Lectures in Sweden
H. John Visser, DPM, Director of the SSM DePaul Residency Program in St. Louis, MO just returned from a week-long stay overseas where he was a distinguished guest lecturer and instructor at the Foot & Ankle techniques course in Gothenburg, Sweden. The course was held at the Sahlgrenska University Hospital, and his topics included new techniques in hallux valgus, Lisfranc dislocation, and Charcot neuroarthropathy reconstruction.

Lecturers for the course represented the countries of Sweden and Germany, with Dr. Visser being the only American distinguished lecturer and presenter. Visser was also recently inducted into the German Society of Foot and Ankle Surgery in 2016 where he also presented in front of an audience of more than four hundred professionals in Munich, Germany.

#DFCon17 Breaks Records, Unveils New Data and Tech in 1st Houston Symposium
In its 17th year, the Global Diabetic Foot Symposium (DFCon) reported standing room only during its first time ever in Houston. "To see the enthusiasm, diversity of thought and specialty, as well as the new tech at this year's meeting is just breathtaking" noted Dr. David G. Armstrong, Professor of Surgery at the University of Arizona and co-Founder of DFCon.

The meeting, which hosted delegates from 50 nations and was streamed live online, revealed several new technologies this year from new data on anti-infectives to next generation smart textiles and wearable robots.

PA Podiatrist Gets Prison for Recording Co-workers in Restroom
A podiatrist will serve up to 16 years in a state prison for secretly recording videos of co-workers in an employee restroom. Dr. David Todoroff was sentenced Tuesday in Dauphin County Court to 64 to 192 months imprisonment. He pleaded guilty in December to eight counts of invasion of privacy. Todoroff, 58, was also ordered to pay a $4,000 fine, and he was ordered to have no contact with the victims.

Query: Coding for Plantarflexed Metatarsal
I have a new Medicare patient that presents with a plantarflexed 5th metatarsal and localized bursitis. I evaluated the patient and gave an injection of lidocaine and dexamethasone, along with an offloading pad. What ICD-10 and CPT codes would you suggest?

Response: CPT 20550 is defined as “injection(s); single tendon sheath, or ligament, aponeurosis (e.g., plantar fascia)”. Your post did not state that you injected either a tendon sheath or ligament.
CPT 20600, on the other hand, is defined as “arthrocentesis, aspiration and/or injection; small joint or bursa (e.g., fingers, toes); without ultrasound guidance” which appears to meet your coding needs.

I believe the better way to go would be to change the name to "The American Association of Podiatric Physicians and Surgeons (AAPPS)." There would be no restrictions on pursuing an expanded scope of practice because of the specificity of the name.

While I can respect the need to eventually make a name change with regard to the American Podiatric Medical Association, I am not sure that the name change to the American Association of Foot & Ankle Specialists is appropriate. My recommendation to the APMA would be to take a page out of big business….hire a PR Firm to re-brand the APMA to something that will hit on social media, the Internet, marketing, and BRAND podiatrists to a degree and level that the AMA, AAOS, SVS, and other organizations have built.
If we all learn one thing from the last presidential election — branding is everything. Messaging follows. If this is the best name we can come up with, fine, then run with it. Otherwise, walk the name back and figure out a better name to rebrand ourselves. 80% of Americans own a smartphone. Do the math. If someone is looking for a DPM, most bets would be it is on their smartphone. Brand the APMA to something that is easily searchable, readable, and palatable.

There is no medical specialty called foot and ankle specialists. There are podiatrists and orthopedic surgeons who specialize in the ankle as well as the foot, but no school gives a degree in the foot and ankle. This change completely eliminates the word that defines who we are. Now, if the desire is to mask who we are and pretend to be something we are not (hint-hint), then this might work, but I am a podiatrist and want to be known as one. Thirty or forty years ago, the general public was not as familiar with the word podiatrist, but now not only they but the medical profession knows us as such.
We get MD referrals who tell their patients that they need to see a podiatrist, not a foot and ankle specialist. If we become foot and ankle specialists, what is to separate us from orthopods? Nothing. We are podiatrists; we treat the foot and ankle in various ways and the constraints of what we can do is often limited by law. All of us are not ankle surgeons, and to imply that we are is not good for the profession. What we do, we do well. No other profession comes close. Let’s not hide who we are.

So often, I read of podiatrists having multiple problems getting their insurance claims paid. They jump through hoops, spend time on phone calls with minimum wage insurance clerks and negotiate fees with patients who are unexpectedly denied. My advice is not to be the “ball” in a ping-pong game between patients and their insurance carriers. It is the weakest position you can be in.
In my practice, I required that orthotics be paid in full at the time of casting. After submitting the claim and the insurance paid for the service, the patient was promptly refunded the amount of the insurance payment. This was all stated in a signed release at the...
