PODIATRISTS IN THE NEWS
KY Podiatrist Discusses Morton's Neuroma

"If you have foot pain, especially in the ball of your foot between the third and fourth toe, you may be suffering from Morton’s neuroma. Wearing high heels or tight shoes increases the risk for developing this condition. Likewise, partaking in high impact activities, such as running, may also cause Morton’s neuroma–by subjecting your feet to frequent trauma. Lastly, if you suffer from bunions, hammertoes, or flatfeet, you have a higher chance of developing a neuroma," says podiatrist Dr. Matthew Ellsworth.

Dr. Matthew Ellsworth

"The first defense to treat Morton’s neuroma is to reduce the inflammation and the nerve injury. The best way to accomplish this is to choose a pair of shoes with stiffer soles and combine with orthotics to take the pressure off the ball of the foot. The orthotics can be adjusted with felt or foam padding as needed to decrease the pressure. Icing and resting the foot can also be effective treatments to relieve the pressure. Careful and routine use of Ibuprofen or other NSAIDs can be a helpful treatment. Steroid and/or alcohol sclerosing injections may be helpful in treating the disease. These injections decrease the inflammation around the nerve tissues to allow softening of other surrounding tissues," Says Dr. Ellsworth.

PROFESSIONAL DISCIPLINE
NH Podiatrist Voluntarily Surenders License

A podiatrist with offices in Concord and Peterborough voluntarily surrendered his license last week, just days before the Board of Podiatry was expected to evaluate his reported “cognitive difficulties” and behavior that was reported as odd and bizarre. Dr. Edward P. Newcott surrendered his license March 21, telling the podiatry board he was retiring from his practice, according to a final order the board issued last week.

PODIATRIC PRODUCTS IN THE NEWS
Compulink Showcases Podiatry-Specific Products at the Midwest Conference

Compulink Business Systems, Inc., a recognized leader in EHR and Practice Management, will unveil the latest enhancements to Podiatry Advantage™ at Midwest Podiatry 2016. New features and functionality being showcased include: OneTab EHR Solution: Compulink's OneTab layout allows providers to easily view and document the entire exam from one screen. This new single screen approach improves provider productivity by eliminating the clicks and the time spent searching for patient information.

PracticeWatch™: Fully integrated with Podiatry Advantage, PracticeWatch automates repetitive tasks, freeing staff to focus on patient care. A built-in scheduler allows tasks to be scheduled to automatically run monthly, hourly, weekly, or daily-day or night. This includes tasks such as running reports and sending of appointment reminders to patients. PracticeWatch also allows users to define business rules to monitor conditions within the practice and execute tasks when specific conditions are met. For example, using PracticeWatch, providers can create a task to monitor patient flow and automatically send them an Instant Message.

IN THE COURTS
Providers Urge Court to Forbid Statistical Sampling in False Claims Cases

Several major industry groups have jumped into a case over whether whistle-blowers may use statistical sampling to prove liability in fraud cases, saying allowing such sampling could be “disastrous” to hospitals and other providers. The whistle-blowers argue that prohibiting the use of statistical sampling could allow providers and others to more easily get away with large-scale fraud. A ruling allowing statistical sampling in this case could lead to more whistle-blowers using the tactic on healthcare organizations, which already face the lion's share of False Claims Act lawsuits.

The American Hospital Association, the Catholic Health Association and nursing home industry group, the American Health Care Association have all filed briefs in recent weeks siding with the provider, Agape Senior Center of South Carolina. The whistle-blowers allege Agape inappropriately certified patients for hospice and other services that were not medically necessary. The whistle-blowers worked at only one Agape facility but want to use statistical sampling to allege fraud throughout the organization. A lower court ruled that the whistle-blowers could not use statistical sampling. The 4th U.S. Circuit Court of Appeals is now considering the issue.

PRACTICE MANAGEMENT TIP OF THE DAY

Online Reputation Protection for Physicians - Part 3

3. Resolve to communicate well with patients

Starting at check-in, simply keeping patients abreast of what to expect next and when to expect it can make all the difference — a patient told that the doctor is running 15 minutes late will be a lot happier than a patient who waits 15 minutes wondering if they'll be waiting an hour. While a doctor shouldn't be kept waiting for a patient to be roomed, it's important that a patient not be roomed long before they are expected to be seen by the physician — time can pass frustratingly slowly when stuck in a white room with a poor selection of magazines. Your whole team should be vigilant about keeping patients informed.

QUERIES (NON-CLINICAL)
RE: Where are Graduating Podiatry Residents Getting Jobs?
From: Name Withheld

I recently posted an ad for an associate position in a large metropolitan area with a direct path to partnership. The salary for the position and the benefits were very competitive with similar positions. I posted the position on PM News and several other websites. I had very little response to the ad. Of the resumes I did receive, it seemed as though most of the applicants were underqualified for the position. It would seem that with over 500 residents graduating from residency programs each year, there would be much more demand for a competitive associate position. Where are all the graduating podiatry residents getting jobs once they finish residency?

RESPONSES/COMMENTS (PM NEWS QUICK POLLS) - PART 1A
From: Joel Lang, DPM

Along with the advantages Dr. Udell already identified, this system is also designed to identify doctors who over-prescribe narcotic medications and patients who “shop” doctors for their narcotics. It will also prevent dispensing incorrect meds because of handwriting issues and the sale and exchange of written prescriptions on the streets as well as modifying written prescriptions to change the quantities or dose.

Patients can still shop online for the best prices for their medications for long-term medications and refills. We really need to relinquish the paranoid attitude that everything the government does is an “encroachment” on everyone’s rights.

Sammy software allows you to print prescription on paper so patients can shop around. You still get the credit for e-prescribing.

RESPONSES/COMMENTS (PM NEWS QUICK POLLS) - PART 2
From: Jeff Kittay, DPM

I am reading Dr. Smith's letter for the third time and still can't believe it. More self-congratulations for the APMA Board and HOD, who have just noticed that there is a problem with the number of graduates and the smaller number of residency slots. My graduating class (1979) had slots for about 50% of the graduates, and residency genesis was the only thing being talked about. Now, nearly 37 years later, it's still on the table. We have double the number of schools, many more graduates, and there are STILL insufficient slots; and some people are encouraging even more student enrollment as the solution - the same people who say lower taxes will yield increased revenues.

RNs and NPs are taking up the "routine foot care" slack BECAUSE of Vision 2015, with its focus on turning out 3-year rear-foot surgeons to the exclusion of nearly everything else. This is a foolish and terribly misguided plan to gain "parity" with orthopedists. Dr. Smith correctly points out that a greater focus on forefoot surgery and general podiatry is the key to saving our profession from itself.

Get rid of several of the tuition-driven schools, change the curricula to an MD/DO by affiliating with allopathic institutions to achieve that elusive "parity," create and maintain residencies that reflect what is really needed by our aging population, and the profession will prosper. Continue to tweak the old paradigm and become obsolete.

RESPONSES/COMMENTS (NEWS STORIES)
From: Robert Kornfeld, DPM

I must say that although this publication rates DPM income as #2 on the list of non-MD healthcare professionals, the salary listed is quite disappointing for anyone who has to go to college for 4 years, podiatry school for 4 years, and a residency for 3 years. My daughter was making more than this 3 years out of college. Honestly, I don't think this salary is something to celebrate. It is woefully low for the investment in tuition, time, and commitment required to be a practicing podiatrist. I can only celebrate the fact that I am at the tail end of my career.

It looks to me like being a podiatrist is going to lose its appeal in the near future. And let me pre-empt some of the responses I expect to see. I am NOT practicing podiatric medicine because I am altruistic. I am practicing it because I love it AND it has afforded me a very nice living. Without the very nice living, I would never have stayed in practice. By the way, does anyone think it's embarrassing that pharmacists make more money than podiatrists?
