NY Podiatrist Discusses Treatment of Cracked Heels

A regular daily body moisturizer is not going to do enough to benefit cracked heels. Podiatrist Jacqueline Sutera, DPM, recommends using creams that are specially formulated for tougher foot skin, including Amlactin and Kerasal. “These have gentle but effective acids that help break down the callus a little at a time,” she says.

Dr. Jacqueline Sutera

You can help the smoothing process along with a foot file, says Dr. Sutera, who recommends using it one to two times per week when you’re in the first stages of treating cracks. “Remember to go in one direction and not back and forth, which can rip up skin more,” she says. You can use a manual file or battery-operated one. The tools you use on your feet should remain yours and yours alone—don’t share them with family," advises Sutera. She also recommends cleaning and drying them thoroughly after using and replacing them every two to three months.

OH Podiatrist Charged with $1.1 Million Healthcare Fraud

A Mentor, OH podiatrist convicted in a mortgage-fraud scheme now faces charges that he defrauded Medicare and Medicaid by submitting claims for more than $1.1 million in services, even though he was prohibited from doing so. Dr. Robert Rosenstein executed a scheme through Community Foot and Ankle of Mentor, the company at which he practiced, to receive money for claims submitted to both programs, according to charging documents filed Wednesday.

PRACTICE MANAGEMENT TIP OF THE DAY
Partnership Agreements: Why it May be Time for an Overhaul - Part 1

When was the last time you reviewed your partnership agreement? Even if you are not currently contemplating new shareholders or partners, if you haven’t looked at your agreement in the last three or more years, now may be the time. From the perspective of business operations, let’s break out some of the key questions that you should consider in every partnership agreement:

1. Addition of new partners: Does the current agreement outline how you will add new partners or shareholders? Is there a buy-in? If so, will that be in the form of financial or sweat equity? Will the buy-in amount or term change (e.g., if it is practice value)? Has a method been determined for assessing that value and has it been incorporated into the agreement?

QUERIES (MEDICAL-LEGAL)
Query: Termination Letters from UnitedHealthcare

Recently, some of my patients that have UnitedHealthcare are presenting us with letters they have received from UnitedHealthcare stating "RE: Notice of Termination-Physician will no longer be in UnitedHealthcare network beginning 3/31/19." The letter further states that I "will no longer participate in the UnitedHealthcare network" and "you should choose another physician within the UnitedHealthcare network."

Our office checked with UnitedHealthcare and we have NOT been terminated and can offer no explanation why these letters were generated. Before I move forward with resolving this disturbing and damaging situation to my practice, I would like to know if anyone else has experienced a similar problem with UnitedHealthcare.

RESPONSES/COMMENTS (CLINICAL)
From: Tip Sullivan, DPM, Ed Cohen, DPM

Stage the procedure.
Stage 1- structural
Stage 2 - cosmetic

If you do not know how to remove excess skin, then send the patient to plastics or another pod who is trained to do it.

Tip Sullivan, DPM, Jackson, MS

My answer would depend on the x-ray and photo. If I could see these two things, I could give a surgical opinion. This is an extremely common problem, but the surgery depends on the position of the toes, the length of the toes, the metatarsal length, and the pathology that the condition creates.

RESPONSES/COMMENTS (NON-CLINICAL)
From: Allen Jacobs, DPM
.
Dr. Sokoloff opines that, with reference to the education of an MD or DO compared to a DPM, such training “should be” no different. "Should be" is key. Presently, it is not. And therein remains the vive la difference. Pass the USMLE? Sorry, not with the current education available. Radical change would be required. Until such changes do occur, and the DPM demonstrates USMLE competency, the DPM degree shall remain a “less than” medical degree. That is reality. The APMA can declare “equality” all they wish; you are still a DPM with a limited license and scope of practice. You may be excellent at what you do, but your degree is not equal to that of an MD or DO. Laws and regulations say so.

I for one am proud of my DPM degree. I find it offensive when I hear DPMs state they “graduated from medical school” or are “foot and ankle surgeons.” Excel at what you do. Demonstrate academic and clinical excellence. Bring credibility to the DPM degree. Compel others to acknowledge the credibility of the DPM for what we do. But stop trying to legislate to equality; or radically change the DPM educational process. The real question: do you really need an MD degree to function as a DPM? I say no.
