PA Podiatrist Discusses Ankle Sprains and Edema
 
Regardless of your age, ankle sprains are common. About 25,000 ankle sprains occur daily, and more than 70 percent are recurring ankle sprains that never completely healed. A common demoninator in ankle sprains is swelling. According to podiatrist Dr. Christine Nolan, "You can control the inflammation, reduce pain and swelling by using the R.I.C.E treatment: 1) Rest the injured ankle (using crutches or walking boot if needed) 2) Ice for 10 to 20 minutes with a half-hour break in between treatments 3) Compress the injury by wrapping the ankle in a bandage 4) Elevate the foot at least 30 degrees above heart level when possible. It is ideal to elevate while icing."

"Gradually improving your mobility will aid in recovery, decrease swelling, and increase strength. A good way to achieve better mobility is working the foot up and down, side to side, and in circular motions. It is important to work on not limping, as this changes the gait pattern and prolongs recovery. Rehabilitation involves strength, balance, and functional exercises to transition you back into your regular activity," says Dr. Nolan.

A short walk from Lake Erie’s edge, nestled on four acres of gently wooded land, is Dr. John Smothers’ home and woodworking shop. For the past 40 years, it has been his oasis where he created wood furniture for sale and juried shows; completed projects for the Ottawa County Museum; and, most recently, worked on the restoration of the Port Clinton Lighthouse.

Smothers, a retired podiatrist, is primarily self-taught, although he has occasionally gathered hints and advice from local woodworkers. Smothers creates a variety of pieces, including small tables he often entered in juried shows in Cleveland and Maryland.

Lace-up Shoes are Still the Best for Feet: UK Podiatrist
 
It's hard to better the men's lace-up Oxford when it comes to shoes, says Emma Ashdown, a podiatrist based in Ashtead, Surrey. "They hold the foot in place," she says. "With slip-ons the foot has to keep working to keep the shoe on - the toes dig in all the time which can lead to hammertoes and calluses."

"Completely flat shoes, like women wear, put too much strain on the Achilles tendon and wearing too high a heel can pull on muscles and lead to problems with the back and hips. The other plus of men's brogues (Oxfords) is they're made of leather, which helps the feet breathe. This means their feet are less likely to smell or develop fungal infections," she says.

Skin Infection Patients Adhere to Just Over Half of Their Antibiotic Regimen: Study 
 
Patients with Staphylococcus aureus skin and soft tissue infections took an average of 57% of their antibiotic doses after being discharged from the hospital, a new study published in Antimicrobial Agents and Chemotherapy reported. Researchers added that close to half of these patients acquired a new infection or needed additional treatment for the existing infection. 
 
Study authors from the Los Angeles Biomedical Research Institute measured antibiotic dosing by using medication containers fitted with electronic caps that documented when patients opened the pill container. This method allowed researchers to detect a large discrepancy in patient reports and electronic measurement. Patients had reported taking, on average, 96% of their prescribed medication, nearly double the 57% that was reported by the electronic caps.

I can't tell you why so many payer directories are out-of-date, but inaccurate ones send many patients to physicians who are out-of-network; which results in unhappy patients hit with additional expenses they weren't expecting. This task might only take an hour or two every few months, but it might just save you a nasty review.

RE: Dealing with Podiatry Vendors Who Sell Their Products Online for Less

Very often, patients will apprise me of the fact that a good product I might dispense from the office is available to them for less money on Amazon.com and other online sources. These products range from well-known OTC orthotics to various creams and antifungals that we dispense from our offices.
 
I have confronted reps of these companies at conventions with this information, and they always appear dumbfounded. In some cases, they assure me that they are doing something about it, yet their products are still being sold online at prices that are sometimes less than we can buy them for. It's about time that we as individuals let these vendors know that their business practices are unfair to us. If they want to keep our business, they need to make the tough choice of either distributing their products through us or via the Internet.

Starting March 27th, all doctors in the state of NY must send prescriptions via the computer or face penalties. I have mixed feelings about this. On one hand, I have been e-prescribing for a long time and it is a service to our patients who no longer have to wait for hours for the pharmacy to read a handwritten prescription and then have the patients wait on line for them to fill it. On the other hand, some patients want handwritten prescriptions so that they can comparatively shop different pharmacies and get the best possible price. New York's law is yet another example of government encroachment into the rights of patients.

Samuel’s and Ross’ comments regarding students, schools, and residencies, I first must congratulate the actions that took place last week in Washington, DC by the APMA Board and the 2016 House of Delegates. A conscious effort was made to address those very issues. Those issues are much more complex than just creating new residencies or lowering the number of students allowed in our schools. They are issues that greatly affect our profession and must be dealt with on many different levels. Our profession will fail and we will cease to exist if these issues are not addressed and changes are not made soon. 
 
We, as a profession, need to address three things to grow and remain relevant. First, we need to market and create additional...

Like Dr. Smith, I also graduated in yesteryear (PCPM '75). I was very fortunate to be selected for one of the then top surgical programs in the country. MANY podiatric grads did not receive ANY residency, much less one recognized  by the ABPS. My training was very heavy in forefoot surgery, but was state-of-the-art, which also included hospital charting, coding, office design. risk management, as well as personal finance advice (life insurance, disability, etc.) I was very successful and practiced office and hospital-based procedures and "routine" care for nearly forty years. There is a VERY strong need for a general podiatrist who is trained to treat ALL foot complaints and NOT just very limited and sophisticated rearfoot procedures.

Mr. Posa, the sky is not falling. I agree with most of what Dr. Brody just stated. I have lectured on the differences of CR vs DR in 2010 and after. I am familiar with the technologies. One thing that you might be misleading the readers on is that the current selection of DR machines produce pretty bad images at the podiatry price point. If you go with a very good DR x-ray machine, it would be above what most podiatrists are willing to pay for the quality.
 
I will take another look at the Midwest Podiatry Conference this weekend, but I would rather have a CR machine that produces great quality than a subpar DR machine. Also, to make a blank statement that those with a CR machine should donate it is unreasonable without knowing their practice. Not all practices see a lot of Medicare patients. It will be interesting to note if the manufacturers in DR x-rays will be raising their prices to compensate for the made-up demand. I suggest they don't, as it will be noted. My 7 year old CR works just fine and takes great x-rays.
