NY Podiatrist Discusses Treatment of Verrucae
Plantar warts are hard growths caused by the HPV virus that appear on the bottoms of the feet — while they can be a nuisance and sometimes painful, they aren't generally serious and may even clear up with over-the-counter remedies. Duct tape, topical acids, and cryotherapy are several common methods that can all be used at home. "If home treatments are not successful after a few applications, seek the advice of a medical professional," says Bruce Pinker, DPM.

Immune therapy and laser treatments are two options for stubborn plantar warts that must be prescribed by a doctor. Surgery to remove plantar warts is usually a last resort.

NJ Podiatrist Discusses Validating Technologies With Studies
Ben Pearl, DPM interviewed David Zuckerman, DPM this week on his Fit Foot U segment. They discussed the importance of validating technologies in your practice with solid studies and pathologic evidence. Dr. Zuckerman recounted how he collaborated with Harvey Lemont, DPM with pathology specimens of the plantar fascia which at the time were speculated to be a "soft tissue malunion" by Zuckerman. After examining the specimens under microscope, Dr. Lemont coined the term plantar fasciosis, which he later expanded upon in a 50-case series paper on heel spurs that was published in JAPMA in 2003.

Also discussed was the importance of establishing reasonable controls for a specified time period that can be followed up. As it is the natural tendency for many painful conditions to get better over the passage of time, this is especially important in examining studies related to pain.

March 22, 2022 was Diabetes Awareness Day. Student Doctor Janice Therese Bautista of NYCPM and Jean Chen-Vitulli, DPM, MS gave diabetic foot care lectures and conducted diabetes foot screenings at Endocrine Associates of West Village along with other healthcare professionals.

Attendees were made aware that COVID-19 can cause diabetes, and its symptoms were discussed. The half-day event also included blood sugar and blood pressure screenings. Samples of healthy food snacks were given to participants.

The standard of care might be amputation of the digit or a transmetatarsal amputation (if more than one digit is involved).
The patient must agree to an off-label procedure or rather a departure from the current standard of care. Before even proceding, there are many potential considerations including:
1. Even if a vascular intervention is attempted, a posterior tibial nerve block might improve the circulation to the digit.
2. Off-loading all pressure off the digit is needed.
3. Using a dermatoscope to better make informed decisions regarding the debridement.
4. Obtaining a bone biopsy, to determine if there is any coagulation necrosis as well as identifying any organisms involved.
5. Treating the osteomyelitis and soft tissue infection with targeted antibiotic powder as well as an antibacterial, anti yeast, antifungal solution.

I have read with interest the previous posts with bewilderment as I have never had a problem or concern achieving total anesthesia on a digit. However, this week, a patient came into the office with a chronically infected hallux that was insufficiently treated with multiple rounds of antibiotics and painful failed surgical intervention from the emergency department. We all get these, and you smile, knowing that this will be a chip shot and you will be the hero.
After two rounds of standard local anesthetic of 8cc total in a standard H block with Lidocaine and Marcaine and 20 minutes to allow it to work, the blocks failed both times; even after a repeat block. I felt that no more could be given as it was risking vascular compromise due to too much volume in a confined anatomical location. I did what had to be done and removed the retained nail pieces left in the borders from the previous failed attempts. The patient was in pain, and I felt terrible, but I had to do what I had to do to get the job done. Never say never.

We used to use 3-WEA on nails and calluses, but as Dr. Ressler notes, at the recommended dilution it wasn’t really softening enough, and at a more potent dilution, it made the skin slippery and also left a residue on treatment chair surfaces as it dripped off the foot. We had to quit using 3-WEA when it was discontinued years back. I switched the usual 10 blades to 313 chisel blades to trim calluses and never resumed use of 3-WEA when it started production again. The heavier blades don’t flex like the 10s going through thicker calluses and makes it much easier to take them down. The round beaver handles are more comfortable in hand than the scalpel handles too.
I replace my nail nippers more frequently as I found that having them sharpened wasn’t worth the time and expense and didn’t fully restore the fresh edge they had as when new. After eliminating the 3-WEA, I found that I was much happier to do routine foot care, and my work area and treatment chairs stayed much neater. I’ve had maybe a half dozen patients actually say that they missed getting the 3-WEA treatment.

While streaming is convenient, state societies must have annual meetings to conduct the business of the association and hold elections. Offering in-person CME is the "hook" necessary to have enough members show up for a quorum.

I too agree with Dr. Chaskin. The real world is living in the digital age but our own profession prefers to keep us in the dark ages. We should be embracing technology. Who wants to sit through a full day of lectures and lose a day of work or with your family? I would much prefer to listen to one lecture on my own time. In New York, MDs do not need CME for re-licensure. Podiatry, still lacking parity, needs them. But, for Pete’s sake, let us get them how we want, when we want.

In my previous post, I cited an important reference from the National Law Review. I think the reference is important and I would advise those who would be so inclined to copy the reference to demonstrate to their patients that the procedure is a non-covered service.
Dr. Abrantes states that the inappropriate billing for the service is the fault of the manufacturing companies that misrepresent the billing regulations. We should all be aware that according to Medicare guidelines, the lack of knowledge by the provider does not negate the requirements for reimbursement or the possibility of potential fraudulent charges. Lack of knowledge with billing/coding is not a viable defense should you be challenged by Medicare or the insurance carriers. The best advice is to contact your insurance carrier and find exactly what they do or do not cover and get it in writing.
