WesternU CPM Celebrates 100% Residency Placement on Match Day
Student doctors from Western University of Health Sciences College of Podiatric Medicine (CPM) held a virtual Match Day celebration on March 19, 2021, learning where they will complete their post-graduate medical training. This year’s CPM class had all 35 eligible student doctors match for a 100% residency placement for the eighth year in a row.

A growing body of research suggests that people who are actively involved in their health and healthcare tend to have better outcomes. Here are six practical suggestions for how to enhance your patients’ engagement in your conversations about their healthcare.
2. Listen before giving advice
Healthcare providers need to convey that their relationship with the patient is a partnership. This means taking the time to genuinely listen. While you may be the expert when it comes to medicine, the patient is the expert regarding their body and life. Let them know that you respect their experience and are taking it into consideration when making decisions about the plan of care.

Query: Deleting A Claim
If a patient refuses to pay their bill due to a high deductible, is it possible to request the insurance company (e.g. Anthem) to delete the claim? That way, the patient will still be responsible for paying their deductible amount elsewhere.

Response: First off, the deductible is the patient’s responsibility whether it's a medical claim, car damage repair or any other type of insurance claim that has a deductible. If they won't pay your claim because of a deductible, what makes you think they will pay another doctor’s claim? Your claim may be more expensive if x-rays were taken or a procedure was done versus seeing their PCP which may only be less expensive office visit type claims… but still they have to pay.
This becomes a business decision. At the beginning of the year, deductibles are always going to be an issue so you should have a policy in place. You could offer a payment plan, but if the patient refuses to pay, then you should follow your other office protocol for non-payment which might include collection agency, small claims court, etc. You can’t just write off everyone’s deductible unless you felt there was some unusual financial hardship.

RE: Recommendation for Billing Service for NY (Mitchell Rubin, DPM)

I use Practice Defenders here in St. Petersburg, FL and it has been a very positive experience. Not only do they understand all of the particulars of podiatry billing, but they do business in almost every state so they know the policies. Most of our claims are paid on the first submission, but when necessary, they are very focused on following up and making sure we don't lose anything. They do a very good job with DME and wound care and that's hard to find.

I am a user/consultant for the Synergy Global Group (SGG) for my billing for six+ years now. I partner with their DPM clients practicing out-of-network or hybrid (in- and out-of-network) and with their uncovered services billing. The best way I can explain SGG to podiatry is that they are a 10% billing company that charges 4% or less. SGG has built a platform that caters to small private practitioners as well as multi-location and grouped practices. They exclusively chase difficult claims for hospital and mega-providers. Their monthly statements and analytics are sophisticated yet understandable.

What is not being mentioned, and has not been mentioned when deciding whether or not to excise the neuroma or address the deep transverse intermetatarsal ligament (DTML), is the actual size of the neuroma. All neuromas are not created equal. If the neuroma is quite large and fills the interspace, a DTML procedure is doomed to failure and you will eventually need to go back again and excise it. That has been my experience. Conversely, if the lesion is small and the patient's symptoms are mild to moderate you should realize success by only addressing the ligament. I can’t tell you where to draw the line, but that comes with experience.
I also discuss both procedures with the patient and let them know that we may have to do a second procedure if the DTML is not successful. Some are okay with this and some are not. Some don’t mind rolling the dice, and some will say "just take it out doc" as they don’t want to go through a second procedure. I also will consider an invasive procedure after attempting sclerosing or steroid injections on small to moderate sized neuromas, and I have stopped injecting larger ones as for me, that has not been successful. Nor have pads or orthotics as patients do not wear them 24/7 and have pain barefoot. As always, evaluate your patient and do what is in their best interest.

As Dr. Kiel is commonly using sclerosing injections on patients, I’m curious as to what code he is using and what solution he is using for “sclerosing” a nerve (Sclerodex, polidocanol, sodium tetradecyl sulfate [sotradecol] or phenol). I have to assume that it is one of these agents, as these are actual sclerosing agents, whereas 4% absolute alcohol is not (as evidenced by the fact that you are doing more than one injection). Clearly, if you have “sclerosed” the nerve, you don’t have to do it multiple times.
