AT THE COLLEGES
Scholl Class of '18 Achives 100 Percent Residency Match

Rosalind Franklin University's Dr. William M. Scholl College of Podiatric Medicine celebrated a 100 percent residency placement during its Match Day event on March 23. "This is truly an accomplishment by members of the Class of 2018," said Scholl College Dean Dr. Nancy L. Parsley. "We are so proud of them. To have a 100 percent placement rate is truly special. It speaks to the hard work, endless effort, and unique talents of our students."

Shirley Chen (center) celebrates with Scholl classmates

Fourth-year students at the North Chicago-based college, joined by friends, family, faculty and staff, gathered in the Scholl Gallery to learn where they will complete their residency training for careers in podiatric medicine and surgery. Shirley Chen matched to her first choice, MedStar Washington Hospital Center in Washington, D.C., where podiatric residents earn an additional credential in reconstructive rearfoot/ankle surgery.

PODIATRISTS AND FOOTWEAR
Get Rid of Your Rubber Flip-flops: NY Podiatrist

Dr. Jacqueline Sutera is on a crusade against cheap flip-flops. The New York City–based podiatrist and board-certified foot surgeon could not warn me enough of the perils of walking for an extended period of time atop flimsy rubber slabs. When we spoke on the phone about how to choose a comfortable sandal for travel, Dr. Sutera was quick to point out what to avoid: stay away from sandals that are super thin and flat.

Dr. Jacqueline Sutera

“We all know that they’re really bad for your feet, especially for women,” she said. “Little girls can kind of get away with [wearing thin sandals] for a while. It doesn’t start to catch up with you until later on in life, but I have 20- and 30-year-olds walking around in them all summer and they wonder why their feet hurt in August. It’s not good.” So what does make for a good walking sandal? Dr. Sutera mentioned three main characteristics: a supportive sole, thick straps, and some sort of back.

PODIATRISTS AND THE LAW
MI Podiatrist Loses Medical License, Avoids Jail Time

St. Joseph podiatrist whose career is now over was sentenced to probation Monday and warned by a judge that his leash to remain out of jail is short. “This wasn’t just bad conduct and illegal conduct, it was abhorrent conduct. This cannot be attributed to anything but stupid behavior, and in your position, that’s unacceptable,” Berrien County Trial Court Judge Sterling Schrock told Craig Sherwood before placing him on probation Monday. He was sentenced pursuant to a plea deal in which he pleaded no contest to diverting opioids, and insurance fraud.

PRACTICE MANAGEMENT TIP OF THE DAY
Areas of Financial Waste to Cut at Your Practice - Part 3

Here are a few ways to reduce financial waste, according to practice management and accounting professionals from across the country:

3.  Shop for Better Prices.

"Medical facilities tend to overpay because they are stuck in their ways and stay loyal with a supplier without shopping around. You can join a group purchasing organization (GPO), which are all over the country, and pay 20 to 40 percent less. It's just a matter of familiarizing yourself with GPOs and what they can be used for. It's more than medical supplies. You can get discounts on cell phones and cable and Internet for you office." - Thomas Moore, CEO of healthcare management firm The Caduceus Group, Atlanta, GA

CODINGLINE CORNER
Query: Midfoot Amputation

The surgeon reported in the operative note that a “midfoot amputation” was performed. Please provide thoughts on CPT 28805 or CPT 28800 as the correct code.

Della Roes, Biller, Lake Havasu City, AZ

Response: CPT code 28808 is: Amputation, foot; midtarsal (eg. Chopart type procedure). A Chopart procedure is a midtarsal amputation of the foot between the calcaneus and the cuboid bones (Calcaneocuboid joint) and the talus and the navicular bones (Talocalcaneonavicular joint).

CPT code 28805 is: Amputation, foot; transmetatarsal. A transmetatarsal amputation, also called TMA, is surgery to remove all or part of your forefoot. The forefoot includes the metatarsal bones, which are the five long bones between your toes and ankle. It would probably be best to access the operative report in order to determine which of these two CPT codes best illustrates the procedure that was actually performed.

RESPONSES/COMMENTS (REVEVANT RESEARCH)
From: Paul Kesselman, DPM

Today, the unfavorable study on orthotics and plantar heel pain (PHP) hit Medscape. I agree that our students and residents are not properly trained in research, nor do we have a sufficient number of  trained clinical or academic podiatrists trained in research. Until such time that this changes, what else can we expect other than more stories and studies which may not accurately reflect the body of knowledge we as clinicians have either been taught and/or have come to believe.

I want to clearly state that podiatrists providing properly peer-reviewed research may not always translate into positive outcomes. And no one would expect all research to be favorable as there of course are more dead ends than not.

But podiatrists properly trained in clinical and academic research should, at the very least, be able to provide ample counterpoints to those negative outcomes in poorly written papers. Without either positive outcomes properly researched or properly prepared counterpoints to negatives, the bean counters at the insurance companies can continue to use unfavorable outcomes any way they wish to sway their reimbursement policies in order to favor their profit margins.

Paul Kesselman, DPM, Woodside, NY
