Spanish Podiatrist Reflects on Her 2-Year Journey at NYCPM

The following memoir was written by Caterina Obrador, MSc:

Caterina Obrador

What would you do if you knew where to go, but not how to get there? As a Spanish podiatrist, I always wanted to be trained in the U.S., for the level of advancement of our profession in this country has yet to be surpassed anywhere else in the world. Unsure of how to make this dream possible, in 2011 I traveled from Dubai, where I was working, to Boston to attend the APMA Annual Scientific Meeting. After a dozen unsuccessful cold call introductions at university booths to identify an international program, I came across Dr. Michael Trepal and realized that such a long trip had been worth making, for I got to know an inspiring individual ready to move mountains to expand our profession beyond national boundaries. This is how, two years later and after overcoming the numerous subsequent administrative hurdles, ...

INTERNATIONAL PODIATRISTS IN THE NEWS
Jamaica Podiatrist Discusses Peeling Skin on the Feet

"Peeling skin on the feet is a very common problem that I see in my office. We define the condition as damage to and loss of the outer layer of skin (epidermis). The skin will often appear dry and flaky and is often accompanied by a rash, itching, and dryness. There are numerous causes of peeling skin on the feet, ranging from allergies, infection, auto immune diseases, edema, environmental factors, and other diseases," says podiatrist Angela Davis.

Angela Davis

"The most common cause I see is due to fungal infections or athlete’s foot. This can occur between the toes or on the sole and upper sections of the foot. Another common cause is contact dermatitis, where the patient has used a detergent, particular shoe material, or hosiery. Harsh environment factors like humidity, sun, heat, and wind can also cause peeling," says Davis.

PODIATRIC PRODUCTS IN THE NEWS
Cryopreserved Umbilical Cord is Effective in Treating DFUs: GA Podiatrist

Amniox Medical Inc. has released the results of a retrospective clinical study of cryopreserved umbilical cord and amniotic membrane (UC/AM) tissue for chronic wound management, a review of the effectiveness of Neox wound allograft in treating patients with chronic diabetic foot ulcers. The findings were presented at Superbones Superwounds East 2016, where the study’s author, Allen Raphael, DPM, was among many top speakers on faculty at the conference.

Dr. Allen Raphael

All wounds were debrided in an operating room or clinic before cryopreserved umbilical cord (Neox wound allograft) was placed directly over the wound and secured in place with sutures. The average time to healing was 13.79 weeks, and the average number of applications of Neox wound allograft was 1.68. Dr. Raphael concluded that cryopreserved umbilical cord is effective in promoting the rapid healing of chronic, diabetic foot ulcers, suggesting its usefulness as an advanced tissue treatment modality.

MEDICAL EDUCATION
Med Students: Exceptionally High Rates of Alcohol Abuse

Approximately one third of medical school students meet criteria for alcohol abuse or dependence, double the rate of their age-matched, non–med student peers with burnout and high educational debt primarily to blame, new research shows. "This is the first study to explore the relationship between alcohol abuse/dependence and burnout among medical students," senior author Lotte N. Dyrbye, MD, professor of medicine and medical education at the Mayo Clinic College of Medicine, in Rochester, Minnesota, told Medscape Medical News.

"In this national cohort of medical students, a third [of students] met criteria for alcohol abuse/dependence. That is a much higher prevalence than what has been previously reported in similarly aged U.S. college graduates," Dr Dyrbye said. The findings are from a national survey that the investigators sent to 12,500 medical students in 2012. The study was published online March 1 in Academic Medicine.

PRACTICE MANAGEMENT TIP OF THE DAY
Online Reputation Protection for Physicians - Part 1

The best way to deal with a few negative reviews is to understand what's driving them, to try to anticipate and correct problems, and to build up positive reviews from happy patients to create a more realistic picture. Here are some of our easiest-to-implement tips to improve your reviews:

1. Monitor rating sites

Healthgrades, Vitals, and Yelp are good places to start. A tech-savvy person with good judgment can take this on for your practice as "social media lead" — it may only take a few minutes per day to stay on top of reviews and respond as necessary (always encouraging the patient to call and never sharing personal health information online). This can be a great opportunity for a motivated staffer to branch out and improve skills.

This person should report back to your whole office on what (s)he finds. Reviews can be a wonderful resource to understand how patients see your practice. Applaud everyone's efforts to understand the strengths and weaknesses of your practice as it's presented online. Make sure to register as the practice owner on sites that allow you to respond to complaints. It's critical to respond quickly, emphasize privacy, and let the patient know how important it is to speak by phone or in person about the issue.

CODINGLINE CORNER
Query: Senior Housing POS & Coding

Has anyone had any experience with coding podiatry services to residents of senior housing? The place of service is not assisted living in any way. These are just apartments for seniors. The facility contacted our office about coming there and treating the residents. Would this be considered "home care", and is it covered?

Nancy, Biller, Podiatry Associates of Rocheser, Office of Michael Giordano, DPM, Mt Kisco, NY

Response: Place of service would be "12" which is "home." The scenario you presented is not "assisted living."

My comment on e-scribing: My numbers would add to more than 100% because we have learned to give patients a written script as well as e-script, especially on weekends! It's really a time saver.

RESPONSES/COMMENTS (PM NEWS QUICK POLLS) - PART 2A
From: Joseph A. Gershey, DPM

In response to Dr. David Samuel’s comment, I want to applaud the APMA Board and House for the actions that they committed to last week at the APMA House of Delegates. For the first time, the Board’s actions view the profession as a single organism. Our profession will fail to thrive if there are not sufficient residencies for graduates, sufficient graduates for healthy schools, or sufficient applicants for uncapped class size. At this House, the Alliance of Component State Associations drafted a resolution asking for an “all stakeholders” meeting at which all phases of the profession could be represented. The charge of the meeting was to draft an action plan to allow the profession to thrive once again.

The APMA Board indicated that this effort should not wait for a Resolution or a “White Paper report.” Dr. Dan Davis announced, to House applause, the scheduling of an “all stakeholders” meeting to address the student pool, residency availability, and growth in the profession. Pursuant to a Resolution of 2015, the APMA Communications arm will begin to educate the profession on PMSR residencies which are not dependent on the RRA credential. The result of this meeting should be an action plan which addresses all phases of the development of our profession in order to allow us to announce, in all coming years, that the growth of our profession has been restored and that our patients and our practices will be the beneficiaries.

RESPONSES/COMMENTS (PM NEWS QUICK POLLS) - PART 2B
From: Pete Smith, DPM

First, I would like to congratulate APMA and its leaders, who back in the early 1990s saw fit to move our profession forward by requiring three-year residency programs vis-a-vis Vision 2015, and who are currently working to move us forward. The recent graduates and current residents are getting a medical and surgical education on par with their MD/DO brethren and certainly much more extensive than most of us (Class of 1988) had. That being said, the move to these residencies has led to some unintended consequences including a shortfall of residencies, a capping of the school class size, and an over-reliance on rearfoot surgical residencies.

The Pennsylvania Podiatric Medical Association saw the need to boost the number of residencies 9 years ago when, in conjunction with a few other states, we formed our own 'Residency Genesis Program' which was later ceded over to APMA which has worked to close the shortfall gap. Residencies have been created, but unfortunately...
